MISSION Our Mission is to provide health and community services that will best meet the needs of our community. In doing so we will focus on: Supporting community identified need and genuine community participation Placing our clients/patients at the centre of our work Supporting individuals, groups and communities to maintain and improve their health and well-being and minimise the negative impact of chronic disease and injury Integrating and coordinating our services within an interdisciplinary service delivery model Allocating and using our resources effectively and efficiently Achieving through collaboration and partnerships Being creative, innovative and open to discovery. CORE VALUES In achieving our goals and objectives we will develop an organisational culture that supports: Social Justice - Equity of Outcome. 4 KEY AREAS, GOALS (OUTCOMES) AND OBJECTIVES Key Area 1 Accountability and governance Goal (Outcome) 1.1 CGHS performs efficiently and effectively and responds strategically to changing demands Objective Develop and continuously improve transparent governance and accountability systems that are consistent with agreed values and support high performance. Strategies Complete the implementation of our detailed governance accountability framework onto lnterplan integrated planning system Continue to apply for home care packages through the Aged Care Approvals Round, in order to support care coordination and service integration within the Wellington LGA Continue to gather timely and accurate intelligence on the impact of the aged care reforms in the community sector and the transfer of responsibility for HACC services for older people to the Commonwealth in order to assist CGHS to respond effectively Participate in the Department of Health sponsored VHA project designed to support Victorian Public Sector Residential Aged Care Services (PSRACS) providers to prepare for the implementation of the Living Longer Living Better reforms in order to effectively address the reforms in a manner consistent with our values and service delivery priorities Progress the Beyond Compliance framework across all affiliated CGHS residential aged care services Provide a coordinated response, from CGHS networked aged care services to the increasing demand for residential and community aged care Support Heyfield Hospital and Stretton Park Boards to determine their strategic directions as part of the CGHS Network Undertake a comprehensive redevelopment of our risk management framework in order to implement an enterprise risk management model consistent with Board approved recommendations of the 2013 VMIA external review. Key Area 2 People Goal (Outcome) 2.1 People are as healthy as they can be Objective Develop a system that is responsive to people's needs Strategies Implement the recommendations of the Gippsland Paediatric Allied Health Reform Project. 5 Reduce the number of Aboriginal people leaving the emergency department at their own risk or prior to treatment, including follow-up by the Aboriginal Liaison Officer and analysis of trends that can identify key issues for our clients and strategies to overcome them Implement, evaluate and continuously improve a Residential In-reach Program as part of CGHS heath independence programs Adopt the principles of Recovery Orientated Mental Health Services across all care settings Apply CGHS values & DH Integrated HP principles in decision making and service & program development activities Build on the work already done to provide a welcoming environment for Aboriginal people, through the display of artwork, flags, acknowledgement plaques and cultural celebration's. This will include the Aboriginal flag being on display in all facilities and participation in the NAIDOC celebrations Continue to prioritise active participation in the Wellington PCP and to benefit from PCP staff and membership involvement in our care coordination and service integration approaches and strategies Develop a CGHS definition for person centred and inclusive health care Engage with local providers of drug and alcohol programs and develop collaborative communication and referral pathways between key stakeholders i.e. Care Coordinators, CGHS GP Unit, GP practices, CGHS emergency department and LRH mental health services and LCHS alcohol and other drug services Improve the health literacy of our population with a focus on the most disadvantaged to support engagement and shared decision making Participate in the Victorian Government Koolin Balit Program, including participation in the Closing the Health Gap Regional Advisory Committee and the sub regional consortium Place a high priority on the implementation of the Beyond Compliance initiative across the CGHS aged care network Prevent or reduce unnecessary hospital admissions through early intervention and by promoting the provision of care in community settings Page 5 of 11
6 Support the development a Gippsland bariatric strategy that details the capability of each health service and describes pathways for people requiring both elective and emergency treatment.the pathways and local protocols should include: a plan based on the patient s needs door to door ; protocols and algorithms for safe handling, equipment, staffi ng, and preoperative and postoperative needs; Evidence based recommendations for the purchase and use of proper assistive equipment Implement the recommendations of the Department of Health Gippsland Paediatric Allied Health review as they relate to CGHS and recommended increases in capacity and capability Take action on reconciliation Undertake a a reablement pilot program as an adjunct to our Care Coordination and Home Support workforce Redesign Projects and as part of CGHS Health Independence Programs (HIP) Build a detailed understanding across our workforce of the principles underpinning person centred, inclusive health care and build the capability to embed these principles into the way we work Continue to develop and implement a program of cultural competency for all staff and volunteers. Goal (Outcome) 2.2 People are managing their own health better Objective Improve everyone's health status and health experience Strategies Draw on the Victorian mental health reform strategy and in particular focus efforts on reducing social isolation for older people and fostering active community participation Encourage broader representation on the CGHS Consumer Liaison Group to include representation from the 15 to 24 age group Accept shared responsibility for, and develop strategic partnerships to implement the National Preventative Health Strategy and the Victorian Public Health & Wellbeing Plan Complete implementation of whole of agency, transformational, care coordination project including minor capital works to integrate patient services and central intake services Maintain a consultant physician workforce with the mix of capabilities required to maintain our clinical service capabilities including: general medicine; rehabilitation; gerontology; gastro-enterology; endocrinology; critical care; coronary care and cardiology. 7 Maintain and enhance our relationship with Wellington Shire Council to enable effective sharing of information relating to our awareness of the needs of older people and our working together to enable access for older people to facilities and services and to support their participation in community life Provide cultural sensitivity training for all staff and Board of Management and frequently measure cultural competence In partnership with Gippsland Region Integrated Cancer Service (GRICS) establish and maintain a Multi-disciplinary Care (MDC) team meeting for all newly diagnosed lung cancer patients (including mesothelioma) within the Gippsland region. Goal (Outcome) 2.3 People have the best healthcare service options possible Objective Expand service capability Strategies Increase our capability and capacity to implement dementia friendly practices across our care settings Maintain a high capacity/capability for medical imaging, including a minimum 128 slice CT Maintain our current capability with regard to emergency and critical care, general medical and general surgery, including related support services such as pathology and medical imaging Redevelop our palliative care suite to provide a more homelike environment with facilities to enable carer and family involvement in patient care and or support Work with DH and other Gippsland service providers to progress the implementation of the Gippsland Subacute Services Review Build our capability for palliative care consistent with the National Palliative Care Strategy: Maintain a close and collaborative relationship with the Gippsland Palliative Care Consortium. Provide information for the general community and General Practitioners to encourage and destigmatise the use of palliative care services. Develop and retain a dedicated palliative care workforce and build the capability of the district nursing workforce to support palliative care Build our capability and capacity to be a health promoting health service and support Wellington Shire Council's Heath & Wellbeing Plan actions relating to healthy lifestyle programs Complete a feasibility study into a fixed MRI service at CGHS to better support our community and with the view of increasing service capability within the health service Maintain and increase CT angiography capability Objective Expand workforce capability Strategies Page 7 of 11
8 Build support for collective action within the health service sector to encourage high levels of articulation between VET and university qualifications, including guaranteed places for students progressing to higher qualifications and in particular for allied health training Expand the role of our home and community care workforce. Specifically we are seeking to support targeted capability development for our home and community care workforce to enable these workers to: Provide a range of health promotion, health support, and therapy for people in their homes; Have available to them a number of pathways to career development including pathways to vocational qualifications in allied health and nursing Complete implementation of CGHS workforce capability development framework, incorporating leadership, professional and technical capabilities and addressing contingency and succession planning and knowledge management Implement Partnering for Performance Program (P4P) and integrate program with overarching Workforce Capability Framework Undertake workforce modelling to reflect approved service capability and future demand identified through the health plan and in particular for aged care Utilise CGHS education and employment support services network to support the development and implementation of workforce development strategies to meet current and projected workforce demand. Objective Increase the systems financial sustainability and productivity Strategies Improve acute care services efficiently and achieve hospital performance targets within designated funding with a focus on developing a highly coordinated approach to admission, treatment and discharge of patients and clients based on the core principles of care coordination and redesigning care Improving theatre productivity by reducing unproductive theatre time and continuing to improve waiting list management Goal (Outcome) 2.4 People management practices are aligned to the strategic and operational goals of CGHS Objective Develop strong linkages between individual, service/divisional performance and overall CGHS strategy. Strategies Individual staff, departmental and organisation level capability development plans will support service delivery capability consistent with CGHS strategic objectives and service delivery priorities Support all departments to develop and regularly update business plans to enable the achievement of CGHS strategic objectives and service delivery priorities. Objective Documented practices support continuous improvement and enable business continuity Strategies Allocate resources to support the CLG and CDNG to support cultural responsiveness and enable the CLG and CDNG to continue to develop new and improved initiatives and resources for cultural responsiveness Formalise, document and evaluate defined referral processes and pathways for maternity services across the Wellington East Gippsland planning area Develop a procedure to detail the process for negotiating and reporting against our HACC funding agreement with the Wellington Shire Council Key Area 4 Knowledge Goal (Outcome) 4.1 Care is appropriate and cost effective and delivered in the most appropriate, cost effective settings Objective Increase accountability and transparency Strategies Continue to provide opportunities to engage individuals and consumer groups in decision making and strategic planning Incorporate community led community consultation as part of population health plan and report CGHS response back to the community Objective Develop a knowledge based culture Strategies Establish a coordinating mechanism for data, information and knowledge management, including internal and external reporting and aquitals Establish a data warehouse Investigate knowledge management approaches at other health services Map the structures, processes and technology that support knowledge management Prioritise and coordinate the implementation of CostPro (clinical costing and decision support system) and Health-e Workforce Solutions application (workforce planning, modelling and budgeting system). Goal (Outcome) 4.2 CGHS is highly productive and sustainable Objective Implement continuous improvement and innovation Strategies Expand hardwiring excellence project to all CGHS wards Implement the Embedding continuous improvement project including the introduction of continuous improvement whiteboards into all departments Implement the Redesigning Hospital Care Program across CGHS. 10 Progress redesigning care projects as agreed with DH Undertake continuous improvement project to improve service integration across the care continuum for maternity services. Key Area 5 Facilities and equipment Goal (Outcome) 5.1 Physical facilities and equipment enable CGHS service delivery. Target Infrastructure upgrades and equipment replacement or acquisitions support agreed service delivery priorities Objective Investments in infrastructure and equipment match service delivery capability priorities Strategies Develop a comprehensive strategy for land and building use consistent with the service delivery priorities and capabilities in the CGHS Health Plan Consider the reactivation of the strategic land use committee to support this work Factor the needs of older people into strategic land use and facilities planning Undertake urgent redevelopment of oncology in order to cope with significantly increased patient demand and provide appropriate facilities for patient, staff and visitors Work with Heyfield Hospital and Stretton Park to determine the feasibility of expanding their residential aged care services consistent with the expected under supply for their catchment, taking into account expected increased demand for accommodation suitable for people with dementia. Key Area 6 Information and communication technologies Goal (Outcome) 6.1 Information and communication technologies faciltate CGHS service delivery Objective Increased and more effective utlisation e-health and communication technology Strategies Link our centralised intake system including services directory in Sale to all reception and intake services across the CGHS network Implement the community module of ipm to support integrated patient services and centralised intake Implement Management Advantage residential aged care management system consistent with GHA implementation program Increase our telehealth capability to support communication across rural, regional and metropolitan Victoria and support the delivery of clinical advice, direct patient care and clinical training Page 10 of 11
