5 Q26: Please describe the activities that you are doing to address disparities: The NFMMC Health Home is located in the hospital's Medical Office Building and connected to primary care for better collaboration of cases with the primary care providers. The Health Home also has a kiosk station embedded within the Niagara Wellness Connection Center to meet with behavioral health and primary care clients before and after appointments to assist with coordination of care between the client and primary care provider. In addition the Health Home has opened an office in Lockport to improve accessibility for clients in the eastern end of Niagara County. Weekly case rounds are held to discuss cases with medical professional staff and care managers for better coordination of care. Non-NFMMC providers are contacted face to face and by phone letter and fax to obtain medical orders related to the client. Medical treatment plans are electronically embedded in the clients' individualized care plans. A paper copy is shared with the client. The care manager plays a pivotal role in ensuring the client receives education and supportive services to promote healthy outcomes.
