2 What are we doing? Three years ago we set out on an improvement journey (Coordinated Community Health Services) with a vision. Whilst we have made significant progress, there is much more to do and we have used your feedback to identify the key priorities for the next phase of our service development. This vision has not changed. It remains central to improving the quality of our services, improving outcomes for our patients and carers, and making LPT a great place for people to work. We know that you are incredibly busy right now and are going above and beyond to provide the best outcomes and best patient experience. We want to ensure that: we deliver integrated, coordinated care which places the patient and carer at the centre, and this care is delivered by happy and supported staff. We want to put you at the centre of designing the best way to do this. Your contribution is vital. Please get involved and help us to design and implement changes to the way we work for the benefit of our patients and our staff. Rachel Bilsborough (Director of CHS)

3 Why are we doing this project? For staff to be able to provide the care they want to, and for patients to receive the care they deserve, we know our service needs to change. There are a growing number of frail older people in the community and we are supporting people with increasingly complex needs, so we have to evolve our service to meet those needs. We need to focus our attention on the patients that really need us and promote self care so that we can improve the resilience of the whole service. As well, we want our staff to feel motivated and supported to do the best job they can, and we want to empower our leaders to make the right decisions in the best interests of patients. We know things are challenging at the moment, and this project is about trying to fix that. We need to improve from where we are right now and ultimately we re too busy not to do this project. What is the project? This work sees CHS leading on redesigning improvements to tackle the challenges facing our planned care teams. We are supported by Newton Europe, a company with experience of working in the NHS and practical on-the-front-line experience of implementing changes. The programme will look to tackle the activities that take up a lot of your time when not seeing patients and we have used your feedback to identify these key activities. We will look to design solutions alongside clinicians, piloting any changes before rolling them out across all teams. There will be several areas of focus for the programme: Planning your workload effectively The right record-keeping for you The right skills for your caseload Caseload Accuracy

6 Programme Timelines

7 What to expect? We will be working with each locality hub one at a time. The first pilot area will be the Warrens. The staff in this area will supporting designing the changes and piloting them with their patients, but we will need all of our staff in our community nursing services to be involved in thinking about solutions. We will be holding regular monthly engagement sessions with all staff, whilst regularly communicating out the progress of our workstreams during design phase. How to get involved? The matrons will be setting up and leading workshops for the workstreams over the next month.
