1 Pharmacy Technician Checklist ONE WEEK BEFORE CLASS STARTS, submit the following documentation to the department assistant in room 506 or the instructor in room 505 (address below): Copy of TABE assessment test results. Reading score of 10.0 required. Math Score of 9.0 required. Completed Student Survey (attached). Signed Technical Standards and Essential Functions document (attached). Copy of high school diploma or GED. Copy of criminal background check. Order your background check at American DataBank $59. (See backside of checklist for information on disqualifying offenses.) Copy of 10 panel drug test taken within two weeks of program start date available at Rocky Mountain Drug Testing $25 cash or Wiz Quiz quiz.com $45 cash Copy of resume. For resume assistance BEFORE the FIRST DAY of class, you should also have: Textbooks Available at EGTC student ID Available at the Treasurer s Office after payment FAFSA and financial aid file completed Official, sealed or electronic college transcripts submitted to Student Records if you are requesting to transfer credits into the program (see your advisor) Items required BEFORE CLINICAL PLACEMENT (additional items may be required) : Clinical Registration Approval Form signed by instructor (required to register) Copy of two doses MMR (measles, mumps, rubella) vaccination or Titer test results Copy of Varicella (chickenpox) vaccination, Titer test results, or physician's documentation of disease Copy of two Hepatitis B vaccinations or Titer test results. This is a series of three vaccinations. The second is administered 30 days after the first. The third is administered 6 months after the second. Copy of negative Tuberculosis (TB) test or negative chest x ray taken within one year of start of clinical. Test available for $20 at Denver Health TB Clinic at 605 Bannock Street or variable price with your healthcare provider. Costs are approximate. Keep a copy of all documentation for your own records. 2 [ NOTE: It is recommended that a prospective student, who is in the process of addressing a disqualifying criminal offense, resolve the issue prior to applying to a Health Sciences & Administration program. Students with unresolved disqualifying criminal offenses may participate in classroom activities but will not be allowed to register for courses that require a practice experience, such as clinicals and internships, in health care facilities. Students who resolve their issue related to a disqualifying criminal offense within one (1) month of completing the classroom component of the program will be allowed to register for clinical/internship courses. Registration will be based upon availability of the course and openings on the class roster.] Colorado Community College System Healthcare Programs Disqualifying Criminal Offenses The following criminal offenses appearing on a criminal background check will disqualify an applicant for admission to CCCS Nursing, Emergency Medical Services (EMS), and Allied Health Programs. Any felony conviction (no time limit). Crimes of violence (assault, sexual offenses, arson, kidnapping, any crime against an at-risk adult or juvenile, etc.) as defined in section C.R.S. in the 7 years immediately preceding the submittal of application. Any offense involving unlawful sexual behavior. Any crime, the underlying basis of which has been found by the court on the record to include an act of domestic violence, as defined in section C.R.S. in 7 years immediately preceding the submittal of application. Any crime of elder or child abuse, as defined in section C.R.S. Any crime related to the sale, possession, distribution or transfer of narcotics or controlled substances. Any felony theft crimes in the 7 years immediately preceding the submittal of application. Any misdemeanor theft crimes in the 7 years immediately preceding the submittal of application. Any offense of sexual assault on a client by a psychotherapist, as defined in section C.R.S. Crimes of moral turpitude (prostitution, public lewdness/exposure, etc.) in the 7 years immediately preceding the submittal of application. Registered Sex Offenders (no time limit). Any offense in another state, the elements of which are substantially similar to the elements of any of the above offenses. More than one (1) D.U.I. in the 7 years immediately preceding the submittal of application. Listed on the U.S. Treasury, Office of Foreign Assets Control (OFAC) or Specially Designated Nationals (SDN). Listed on Health and Human Services/Office of Inspector General List of Excluded Individuals/Entities or the GSA List of Parties Excluded from Federal Programs. If the investigation reveals information that could be relevant to the application, the designated individual responsible for background checks may request additional information from the applicant. The offense shall be reviewed on a case-by-case basis. Students who have successfully completed the terms of a deferred adjudication agreement will not be disqualified. If any applicant feels the criminal background check is inaccurate, they may appeal the decision and request a review with the specific community college applied at. It is the applicant s burden to produce substantial evidence that proves the crimes charged are incorrect.

4 Technical Standards and Essential Functions The following technical standards and essential functions outline reasonable expectations of a student in the Pharmacy Technician Program for the performance of common pharmacy technician functions. The pharmacy technician student must be able to apply the knowledge and skills necessary to function in a variety of classroom, lab and/or clinical situations while providing the essential competencies of pharmacy technicians. These requirements apply for the purpose of admission and continuation in the program. Please see Emily Griffith Technical College s (EGTC) ADA Policy in the college catalog or on the website: for additional information. A copy of requirements of essential functions required by local institutions and organizations is available upon request. The student must demonstrate the following abilities: Categories of Definition Essential Functions Observation Ability to participate actively in all demonstrations, laboratory exercise, and clinical experiences in the professional program component and to assess and comprehend the condition of all clients assigned to him/her for examination, diagnosis, and treatment. Such observation and information usually requires functional use of visual, auditory, and somatic sensations. Communication Motor Ability to communicate independently and effectively in English using verbal, non-verbal and written formats with faculty, other students, clients, families and all members of the healthcare team. Sufficient motor ability to execute the movement and skills required for safe and effective pharmaceutical practice Example of Technical Standard Visual (Corrected as necessary) Able to visually discriminate increment readings on syringes Able to read instrument scales Able to enter and review data during use of computer equipment Able to visually discriminate different colored and shaped objects Recognize and interpret facial expressions and body language Assess the environment at a distance Auditory (corrected as necessary) Recognize and respond to soft voices or voices under protective garb Recognize and respond to voices over the telephone, via a speaker, or from microphone speaker in a drive-thru Exercise excellent verbal and interpersonal communication work to impart information to clients or to the public, to convey instructions to patients or co-workers clearly and accurately Assess nonverbal communications Transmit information to clients, fellow students, faculty and staff, and members of the health care team Receive, write and interpret written communication in both academic and clinical settings Eye-hand coordination and finger dexterity required to achieve the psychomotor objectives (use of a spatula and tablet counter, mortar and pestle, balance and weight set, needles, syringes, and the skills of counting and pouring) Excellent fine motor skills and hand/finger strength, such as those required to perform sterile compounding using strict aseptic technique. The ability to manipulate needles and syringes while holding your arms out in front of you for extended periods of time. Lift up to 50 pounds Stand and/or walk for long periods of time (8-12 hours)

5 Intellectual Intellectual (continued) Behavioral and Social Attributes Ability to collect, interpret and integrate information and make decisions. Possess the emotional health and stability required for full utilization of the student s intellectual abilities, the exercise of good judgment, the prompt completion of all academic and patient care responsibilities and the development of mature, sensitive, and effective relationships with clients and other members of the health care team. Possess the ability to tolerate taxing workloads, function effectively under stress, adapt to changing environments, display flexibility, and learn to function in the face of uncertainties inherent in clinical settings with patients. Possess compassion, integrity, concern for others, and motivation. Read and comprehend relevant information in textbooks, prescriptions and medication orders, medical records and professional literature Measure, calculate, reason, analyze and synthesize Utilize intellectual abilities, exercise good judgment and complete tasks, within required time limits Retain information Apply knowledge to new situations and problem solving scenarios Manage heavy academic schedules and deadlines Perform in fast paced clinical situations Display flexibility Sustain professional activities for protracted periods under conditions of physical and emotional stress Demonstrate emotional health required for full utilization of intellectual abilities and exercise of good judgment Demonstrate integrity, concern for others, interpersonal skills, interest and motivations Accepts responsibility and accountability for one s own actions Develop mature, sensitive and effective relationships with clients and others Comply with the professional standards of the pharmacy profession Possess the ability to demonstrate professional behaviors and a strong work ethic. Health Sciences programs establish technical standards and essential functions to insure that students have the abilities required to participate and potentially be successful in all aspects of the respective programs, and to ensure that the program remains in compliance with their accrediting agency. In addition, student requirements related to the technical standards are further affected by the requirements of the externship sites which are regulated by the State Board of Pharmacy, the Joint Commission, and various other federal and state agencies. Students are required to meet technical standards and essential functions for the Pharmacy Technician Program as indicated above, if an applicant or student is unable to meet all of the outlined standards, he/she may be withdrawn from the program.
