This year s conference will feature great speakers, opportunities to learn about the latest and greatest products and services in IV and venous access treatment, and the ability to network with fellow infusion therapy professionals. LITEVAN 2019 also will feature a poster area to share information and best practices. Registration Information All conference registrations will be completed online. Accepted forms of payment are Visa, MasterCard, American Express or check. If you wish to pay by check, please print out your online confirmation page to use as an invoice and mail with your check to the LITEVAN Office. More registration details can be found on page 4 of this brochure. Registration Specials LITEVAN is again giving an incentive to register early with discounted early bird registration rates! Save up to $50 by registering before the March 29 Early Bird registration deadline. LITEVAN also continues to offer group discounts for three or more attendees from the same facility, making it easier for your entire staff to gain new knowledge at LITEVAN 2019, as well as a discount to AVA members. Opening Reception This year s Opening Reception will take place on Thursday, April 25 from 5:00 p.m. to 6:00 p.m. in the Exhibit Hall. Registration Cancellation, Substitution and Emergency Illness or Death Policies Registration Cancellations Policy All registration cancellations and refund requests must be made in writing by March 29, A refund of the full conference fee, minus a $75 administrative fee, will be given for cancellations received by that date. No refunds will be granted for requests received on or after March 30, Submit all requests to Caitlin Price at cprice@cmemanage.com. LITEVAN regrets that refunds will not be given for no-shows. Refunds will not be processed until after the conference. Substitutions You may substitute registrants (from the same office/ practice) prior to the conference. Please contact the LITEVAN Office at x 110 for assistance. LITEVAN does not accept responsibility for no shows at the conference. Emergency Illness or Death of Registrant or Immediate Family Member Registrants who are unable to attend the LITEVAN Annual Educational Conference because of a serious illness or death may receive a partial refund (amount paid minus the $75 processing fee) or full credit for a future meeting (as applicable) in the following instances: Personal illness or death of the attendee; or Illness or death in the immediate family of the attendee. Immediate family refers to a person s parents, spouse, children, and siblings and will also include the parent s spouse. 4 Requestors must submit the request in writing to the LITEVAN Office at cprice@cmemanage.com along with written documentation from a healthcare provider The Executive Director and LITEVAN President will review the request to ensure it is in accordance with written policy. Register Now Online! All LITEVAN 2019 registrations must be completed online at LITE.org/conference. The system allows you to register quickly and easily, and you can choose to pay via credit card securely through the site or to be invoiced and send a check. If you are a member of LITEVAN, log into the website and the discounted rates will be provided on the conference page. If you do not know your password, visit LITE.org and click Login at the top. Choose the Forgot Password option and follow the steps to reset your password. Group Registrations Need to register multiple attendees? You can enter group registrations together and pay for them all at once. On the conference registration, choose the Group Discount option. Complete the registration information for the first person, and at the bottom of the page, click Add Guest under the Guest List header. Complete the information for the second registrant and click Done. This will bring you back to the page with the initial person s registration. You can continue to add registrants by following the same steps. Once you ve entered information for all registrants, click Next to continue and complete the registration process.

Nemacolin has the best of both worlds: meeting rooms that allow attendees to engage fully in the educational sessions as well as an array of activities to enjoy at your leisure. Some of Nemacolin s activities include: an indoor pool and fitness center; championship golf course Mystic Rock; exquisite spa; shopping; and the Lady Luck Casino Nemacolin (located approximately three miles from Nemacolin s main entrance).
