3 Context The context for ths recommendatons report s qute dfferent from that of the frst report. The structural recommendatons advanced last year wll be used n future years to frame and poston our recommendatons and our work n collaboraton wth the Local Health Integraton Networks (LHINs). In the short perod of tme that elapsed between submsson of our report n March 2013 and ths second report (about sx months), our recommendatons served to gude the work and endeavours of the LHINs wth respect to French language health servces. On the bass of our recommendatons and of the prortes defned n ther respectve Integrated Health Servces Plans, the LHINs developed work plans. The work requred to respond to our prevous recommendatons s therefore stll ongong, and therefore the next Jont Annual Acton Plan between the LHINs and the Réseau du meux-être francophone du Nord de l Ontaro (RMEFNO) should ncorporate all recommendatons; whch wll n turn strengthen the collaboratve work wth the RMEFNO. The stronger role of the RMEFNO as a resource for health servce provders may be one of the greatest success stores snce the last recommendatons. Our last report concluded by focusng on the mportance of workng closely together n order to develop collaboratve workng habts. Ths collaboraton must contnue at all levels and especally n the feld where jont LHINs-RMEFNO vsts to health servce provders have made t possble to clearly defne our respectve roles n our relatonshp wth provders. It also clearly demonstrates the LHINs commtment to French language health servces. Context 3 of 26

4 Follow-up of the Recommendatons The followng sectons provde an overvew of the recommendatons n the last report and the work done to move them forward. They also draw attenton to the work that remans to be done and reaffrm the prortes dentfed by the RMEFNO through ts communty engagement actvtes and ts role as French language health plannng entty. We would once agan lke to emphasze that for a number of these recommendatons, because of the short tme between the submsson of the two reports, much of the work remans to be done and the actons proposed by the LHINs n ther respectve work plans are seen as evdence of ther nterest and ther ntenton to work on mprovng the offer of French language servces n Northern Ontaro. 1. Access Ponts Create, mantan and support the development of French language health care servce access ponts that are lngustcally and culturally approprate for Ontaro's Francophone populaton. The structural recommendaton on access ponts stresses the mportance of havng servce access ponts where there s an actve offer of health servces n French. The requrement to ensure safety and qualty of care across the contnuum of health servces avalable to the people of Northern Ontaro s all the more mportant because servces are scattered across the terrtory. Francophones n our area often have to travel to obtan care, and havng the opportunty to be treated n French outsde ther home communty s all the more mportant when they fnd themselves far from ther famly and communty support networks. 1.1 Develop a Jont Workng Mechansm for Desgnaton Develop a jont workng mechansm wth the LHINs for the desgnaton of nsttutons, as set forth n Regulaton 515/09 of the Act, Collaboraton on agency desgnaton has moved forward by leaps and bounds over the past year. The roles of the NE LHIN and the RMEFNO n the desgnaton process were clarfed and formalzed. A seres of 3 webnars was offered by the LHIN for the RMEFNO team to provde tranng on the Desgnaton Plan template and strateges to support Health servce provders through the process. Collaboraton on agency dentfcaton as well as the support provded durng the desgnaton request process and the updatng of desgnaton plans must therefore be contnued ths year. The NE LHIN s planned actons to answer ths recommendaton are n the process of beng completed. The NW LHIN has ndcated that t wll contnue to work wth ts dentfed health servce provders on ther French language servces plans. The LHIN s also preparng an annual stuatonal analyss for the RMEFNO. In lght of the RMEFNO s expandng role as a resource for health servce provders, we recommend to the NW LHIN to ntroduce us as an ally n ts endeavours n order to promote our ntegraton nto the process. 5 1.2 Promote an Actve Offer Promote an actve offer of French language health servces, by rasng awareness and by supportng organzatons. The NE LHIN has ndcated that actve offer s part of ts expectatons of provders who are preparng or updatng ther desgnaton plans. The French Language Servces Coordnator has also ssued nternal remnders of the mportance of ncludng the needs of Francophones n all plannng actvtes. The NE LHIN s planned actons to answer ths recommendaton are n the process of beng completed. The Northern Ontaro School of Medcne (NOSM) organzed a workshop on cultural and lngustc competences n Sudbury and n Thunder Bay. The RMEFNO helped to publcze the workshop and the NW LHIN encouraged all health servce provders to partcpate. Sx provders n the North West took part n the workshop. The RMEFNO beleves ths knd of tranng s essental n the work of rasng awareness on the ssue and recommends to the two LHINs that they promote actvtes lke ths one and that they encourage all of the health servce provders they fund to partcpate. The NW LHIN s planned actons to answer ths recommendaton are n the process of beng completed. 1.3 Support and Work Wth the Communtes of Thunder Bay and Tmmns Gven that gaps were dentfed n French language servces n Thunder Bay and Tmmns, and gven that these communtes have clearly shown ther commtment to fnd solutons for these gaps, we recommend that both LHINs work wth ther respectve communtes to fnd a soluton for provdng equtable access to French language health servces that address the communtes specfc needs. The LHINs responded to ths recommendaton by sayng they wanted to work wth these communtes. Tmmns The NE LHIN ndcated they would present the Health Lnks model to the Carrefour santé de Tmmns and ensure the ncluson of French Language servces plannng n the mplementaton of the Tmmns Health Lnk. Last year s recommendaton s stll vald and the RMEFNO recommends that the Francophone communty of Tmmns be ncluded n the health servce plannng process n order to ensure they have better access to health servces n French. The NE LHIN s planned actons to answer ths recommendaton have not started. Follow up of the Recommendatons 5 of 26

6 Thunder Bay In response to ths recommendaton, the NW LHIN s workng wth ts staff and wth health servce provders to fnd solutons for equtable access to French language health servces by focusng on ts Integrated Health Servces Plan prorty areas: enhancng access to prmary care; reducng wat tmes; reducng percentage of alternate level of care (ALC) days; mprovng access to specalty care and dagnostc servces; and mprovng access to mental health and addctons servces. The LHIN wll also work wth the RMEFNO to dentfy servce needs that fall wthn the access to care prorty area. The RMEFNO recommends that the Francophone communty of Thunder Bay be ncluded n the health servce plannng process n order to ensure they have better access to health servces n French. The NW LHIN s planned actons to answer ths recommendaton have not started. 2. Human Resources Increase the avalablty and mprove access to qualfed French-speakng human resources where they are needed. Ths nvolves a tranng component and a human resources plannng component. 2.1 Offer Tranng on Cultural and Lngustc Competences Develop and mplement an approach to contnuous tranng adapted for Northern Ontaro wth a vew to mprove cultural and lngustc competences n all organzatons provdng servces to Francophones The NE LHIN s response to ths recommendaton was to focus on communty engagement actvtes and the healthcare stores of Francophone patents. These stores can be used to rase health servce provders awareness regardng the real mpact of actve offer of French language servces on patents navgatng the healthcare system. The NE LHIN s planned actons to answer ths recommendaton have not started. A study on future health human resources needs was completed n the North West. The NW LHIN and the RMEFNO took part n ths project and a jont analyss of the results was undertaken n order to dentfy the needs of Francophones n the regon. Through collaboraton wth several health servce provders, t was possble to rase ther awareness of the mportance of actve offer and of French language servces n general for the Francophone populaton. Work on ths must contnue n order to support provders efforts to recrut blngual staff and to set up French language servces commttees and mplement mechansms that promote the tranng and awareness of staff wth regard to cultural and lngustc competences. The NW LHIN s planned actons to answer ths recommendaton are n the process of beng completed. Follow up of the Recommendatons 6 of 26

Preventon and Promoton Support the development and mplementaton of lngustcally and culturally approprate health promoton programs focused on determnants of health and desgned to mprove the health status of the Francophone populaton. 3.1 Use Technologes for Promoton and Preventon We recommend a greater use of vdeoconferencng and Web tools to mprove access to tranng and workshops on promoton and preventon. Ths approach would also make t possble to serve areas wth a small number of Francophones. The possblty of ncreasng the use of vdeoconferencng and the Ontaro Telemedecne Network (OTN) to provde workshops and servces to Francophones was dscussed. Each of the LHINs has ndcated that t s takng steps to move ths forward, however ths recommendaton has not yet produced any concrete results. The RMEFNO remans convnced that best practces n ths feld and lved experence n the North and elsewhere n the provnce support the vablty and effectveness of ths approach. The NE LHIN ndcated that t would brng ths recommendaton to the Prmary Care Advsory Councl. The NE LHIN s planned actons to answer ths recommendaton have not started. The NW LHIN has ndcated that t wants to dentfy provders offerng courses on Chronc dsease management and self-management and to work wth them to make these courses avalable n French. The NW LHIN s planned actons to answer ths recommendaton have not started. 4. Communty Engagement and Servce Plannng Ensure that Francophone communtes have a more actve role n the plannng of French language servces and n self-management of ther health. 4.1 Ensure the Collecton of Pertnent Data To mplement or adapt the nformaton systems as soon as possble to enable data collecton on Francophones n the followng areas: servce utlzaton; health status of the populaton; development and presence of Francophone human resources. The NE LHIN has suggested that t wll share the data of ts communty profles wth the RMEFNO once they have been completed. The NE LHIN s planned actons to answer ths recommendaton have not started. Smlarly, the NW LHIN s proposng to share nformaton on geographc nformaton systems wth the RMEFNO. It has shared the percentages of Francophones by Integrated Dstrct Network (IDN) wth the RMEFNO. The NW LHIN s planned actons to answer ths recommendaton are n the process of beng completed. 4.3 Use a Francophone Lens Recommend (requre f possble) the use of Health Equty Impact Assessment (HEIA) tool n all LHINs and health servces organzatons ntatves, to serve as a Francophone lens. The NE LHIN has ndcated that t wll share nformaton on the Health Equty Impact Assessment (HEIA) tool wth the Health Lnks. French language servces should also always be ncluded n the LHIN s nternal assessment tools. The NE LHIN s planned actons to answer ths recommendaton are n the process of beng completed. The NW LHIN s usng the Health Equty Impact Assessment tool (HEIA) when preparng ts plannng projects and has proposed that ths be added as a step n the project charter development. 9 4.4 Include French Language Servces n Agreements wth Provders Where relevant, nclude, n agreements wth provders, the duty to offer French language servces and assess these servces. The NE LHIN has ndcated that t dscusses French language servces at ts quarterly meetngs wth health servce provders. French language servce ndcators are also ncluded n the Hosptal Servce Accountablty Agreement (H-SAA), the Mult-Sector Servce Accountablty Agreement (M-SAA) and the Long-Term Care Servce Accountablty Agreement (L-SAA). The NE LHIN s planned actons to answer ths recommendaton are n the process of beng completed. The NW LHIN has ncluded an ndcator n the M-SAA for provders to complete the French language health servce mplementaton plans and the French language servces annual report. The NW LHIN s planned actons to answer ths recommendaton are n the process of beng completed. 5. Mental Health and Addctons Create and partcpate n a jont commttee between the RMEFNO and the LHINs to assess the state of French language mental health and addctons servces, as well as the needs n ths area and the possble solutons. Ths commttee would examne, among others: The mportance of encouragng the Mnstry of Health and Long-Term Care, fundng agences and Health Servce Provders to ncrease the avalablty of culturally and lngustcally approprate promoton and preventon programs for Francophones. The mplementaton of methods for collectng data on French language mental health and addctons servces. As actons n response to ths recommendaton, the NE LHIN has proposed that the RMEFNO partcpate n mental health and addctons plannng tables and that the Executve Drector take part n the Mental Health and Addcton Network of Care. It s also plannng to revew access to mental health and addcton servces n addton to mprovng coordnaton and ntegraton of servces n ths sector. The NE LHIN s planned actons to answer ths recommendaton are n the process of beng completed. The NW LHIN ndcated that t wll work wth the RMEFNO to dentfy needs for servces n French that fall under the access to care prorty. The NW LHIN s planned actons to answer ths recommendaton have not started. The RMEFNO would lke to cauton the LHINs about the danger of loss of servces durng servce ntegratons or transfers. It s mportant that when servces are transferred to a locaton wth fewer Francophones, the same level of servce avalablty n French be mantaned f the same populaton contnues to be served from a centralzed or ntegrated locaton wthn another organzaton. 10 Follow-up Tables of the Recommendatons Report The followng tables refer only to actons undertaken snce the last report. It s clear that a number of the recommendatons n the report wll requre ongong work over the next few years. Subsequent work plans must ndcate how the LHINs ntend to contnue ths work n order to meet the objectves set n earler recommendatons that have not yet been acheved. Because of the complexty of several of the structural recommendatons, t goes wthout sayng that the objectves set n past reports are stll as mportant for the mprovement of access to French language health servces. The NW LHIN, n collaboraton wth the RMEFNO, revewed the study on Health Human Resources n order to dentfy the obstacles and mportant elements for Francophones n the North West. As noted n pont 1.2: A sesson was hosted n Thunder Bay on May 23, 2013 by the Northern Ontaro School of Medcne (NOSM). The RMEFNO helped NOSM wth the dstrbuton of the nformaton regardng ths workshop. The NW LHIN ncluded n the Hosptal Servce Accountablty Agreement (H-SAA) and n the Mult-sector Servce Accountablty Agreement a French language servces ndcator to be completed n the desgnaton plans and n the French language servces Annual Report. 5. Mental Health and Addctons Create and partcpate n a jont commttee between the RMEFNO and the LHINs to assess the state of French language mental health and addctons servces, as well as the needs n ths area and the possble solutons. Ths commttee would examne, among others: The mportance of encouragng the Mnstry of Health and Long-Term Care, fundng agences and health servce provders to ncrease the avalablty of culturally and lngustcally approprate promoton and preventon programs for Francophones. The mplementaton of methods for collectng data on French language mental health and addctons servces. 19 In-Year Recommendatons for The RMEFNO s gudng prncples were offcally presented to the LHIN Boards of Drectors. The RMEFNO was consulted durng development of the NW LHIN s Integrated Health Servce Plan (IHSP) III ( ). The Executve Drector presented recommendatons on ncluson of Francophones n the IHSP III ( ). An RMEFNO Communty Engagement and Plannng Offcer helped organze the Mental Health Forum organzed by the NE LHIN n Sudbury. She asked that the speakers retaned be blngual. Pror to the NW LHIN s Consultaton Forum, the RMEFNO asked that all of the documentaton be made avalable n French. The RMEFNO ponted out to the NE LHIN that ts Request for Proposals (RFP) for exercse and falls preventon classes was n Englsh only. In-Year Recommendatons for Actons and Follow-up by the NW LHIN n The NW LHIN was to consult ts legal counsel and follow up regardng ther response to the gudng prncples. Follow-up from the board of Drectors s expected. One table was changed to nclude the percentage of Francophones n each Integrated Dstrct Network (IDN). Other wordng changes and recommendatons were not ncorporated nto the IHSP text, however the LHIN ndcated plannng ntatves would nclude the francophone perspectve. All of the documentaton was avalable n French for the event and the LHIN receved a number of postve comments on the qualty of the documents, whch were much apprecated. Actons and Follow-up by the NE LHIN n The LHIN Board of Drectors has acknowledged recept of the RMEFNO s gudng prncples. All of the speakers retaned were blngual. The LHIN delayed the RFP untl all of the documents were avalable n both languages. The RMEFNO worked on preparatons for the Francophone communty consultaton on mental health organzed by the Mnstry of Health and Long-Term Care (MOHLTC). 22 New Recommendatons for The structural recommendatons presented n our frst recommendatons report ( ) are repeated n ths table to serve as the man categores for the new recommendatons. The specfc recommendatons n the frst report are shown n pale gray to provde context for the new recommendatons, whch are often lnked to them. Our work n the feld and our collaboraton wth the LHINs n the plannng of French language health servces have enabled us, snce the last report, to refne or add to the recommendatons n the frst report. As we ndcated n the background at the begnnng of ths document, the lmted perod of tme between submsson of the frst and second recommendatons reports has not allowed the LHINs to show results for all of our recommendatons. Our frst report was desgned to lay the foundatons for the work to come n the next few years and the focus was on presentng the context and the challenges for French language health servces n Northern Ontaro. We wll contnue to work wth the LHINs to support them n ther efforts to mplement the recommendatons from the frst report. Moreover, as ndcated above, each LHIN quckly developed a work plan wth proposed actvtes and objectves n response to the recommendatons Recommendatons 1. Access Ponts Create, mantan and support the development of French language health care servce access ponts that are lngustcally and culturally approprate for Ontaro's Francophone populaton. 1.1 Develop a Jont Workng Mechansm for Desgnaton Develop a jont workng mechansm wth the LHINs for the desgnaton of nsttutons, as set forth n Regulaton 515/09 of the Act, Promote an Actve Offer Promote an actve offer of French language health servces, by rasng awareness and by supportng organzatons Gven ther remarkable effectveness as an educatonal tool, the RMEFNO recommends that the LHINs use the healthcare stores of Francophone patents provded by the RMEFNO as a best practce to rase health servce provders awareness of the mportance of French language servces. 1.3 Support and Work wth the Communtes of Thunder Bay and Tmmns Gven that gaps were dentfed n French language servces n Thunder Bay and Tmmns, and gven that these communtes have clearly shown ther commtment to fnd solutons for these gaps, we recommend that both LHINs work wth ther respectve communtes to fnd a soluton for provdng equtable access to French language health servces that address the communtes specfc needs Last year s recommendaton s stll vald and we recommend that the Francophone communty be ncluded n ths process and that work be done on fllng the dentfed gaps n French language health servces. 23 1.4 In lght of the partcular vulnerablty of the elderly Francophone populaton, the RMEFNO recommends to the LHINs that they work wth health servce provders delverng long-term care to revew the process for allocatng places n long-term care facltes. We recommend that patents language be consdered n the allocaton of places n facltes offerng servces n French or n dentfed facltes. 1.5 The RMEFNO recommends to the LHINs that they dentfy health servce provders who demonstrate best practces n the actve offer of French language servces and encourage them to share these practces and success stores wth other provders. 1.6 The RMEFNO recommends that t be a stakeholder n the Health Lnks from the start of ther plannng and mplementaton so that t can act as the Francophone lens. We also recommend to the LHINs that they present us as an ally to health servce provders n order to promote our ntegraton nto the process. 2. Human Resources Increase the avalablty and mprove access to qualfed French-speakng human resources where they are needed. Ths nvolves a tranng component and a human resources plannng component. 2.1 Offer Tranng on Cultural and Lngustc Competence Develop and mplement an approach to contnuous tranng adapted for Northern Ontaro wth a vew to mprove cultural and lngustc competences n all organzatons provdng servces to Francophones Recognzng the mportant role of tranng on cultural and lngustc competences n rasng awareness of the mportance of the actve offer of French language servces, we recommend that the LHINs work wth those who organze such tranng or workshops, that they promote them and that they encourage health servce provders to partcpate. 2.2 In vew of the gaps dentfed n the avalablty of French language health servces n the Algoma regon and untl such tme as better solutons can be put n place, the RMEFNO recommends to the NE LHIN that t look nto the possblty of settng up a professonal nterpretaton servce. The use of vdeoconferencng (OTN) to provde ths servce to the populaton should also be consdered. 2.3 The RMEFNO also recommends that the benefts of OTN be promoted to healthcare professonals and patents. We recommend usng OTN to ncrease the offer of French language servces for patents lvng n communtes where there s no access to the servces of blngual professonals or specalty servces. Ths approach would make t possble to reduce costs by avodng travel whle ncreasng the rate of consultaton wth the approprate servces, reducng vsts to the emergency room and fosterng complance wth doctors orders by patents who would receve servces n French. 3. Preventon and Promoton Support the development and mplementaton of lngustcally and culturally approprate health promoton programs focused on determnants of health and desgned to mprove the health status of the Francophone populaton. 3.1 Use Technologes for Promoton and Preventon Increase the use of vdeoconferencng and Web tools to mprove access to tranng and workshops on promoton and preventon. Ths approach would also make t possble to serve areas wth a small number of Francophones Gven the mportance of people s actve partcpaton n the management of ther health and chronc dseases, the RMEFNO recommends to the LHINs that they stress to health servce provders the mportance of offerng programs n French n the areas of promoton and preventon and the self-management of chronc dseases. 4. Communty Engagement and Servce Plannng Ensure that Francophone communtes have a more actve role n the plannng of French language servces and n self-management of ther health. Communty Engagement and Servce Plannng for of 26

We recommend that ths work be contnued n order to mprove the process for dentfcaton of blngual staff and standardzed evaluaton of ther level of lngustc competence. 4.6 Gven the lack of evdence on the health of the Francophone populaton of our regon, the RMEFNO recommends that the LHINs support the research: A Regonal Health Needs Assessment of the Francophone Populaton n Ontaro, submtted to the Partnershps for Health System Improvement program of the Canadan Insttutes for Health Research by a team from the Unversty of Ottawa n collaboraton wth the RMEFNO for Northern Ontaro. If the project s funded, the RMEFNO recommends to the LHINs that they partcpate and help wth ths research The RMEFNO recommends to the LHINs that they endorse the Jont Poston Statement on the Lngustc Varable of the Regroupement des Enttés de planfcaton des servces de santé en franças de l Ontaro and adopt the questons t proposes as a means of dentfyng Francophones We recommend that the LHINs support the health servce provders they fund n the mplementaton of these questons n order to dentfy Francophone patents. 4.7 Wth the objectve of facltatng the dentfcaton of Francophone patents, we recommend to the LHINs that they work wth the RMEFNO to move forward on the Jont Poston Statement on the Lngustc Varable of the Regroupement des Enttés de planfcaton des servces de santé en franças de l Ontaro as a means of dentfyng Francophone patents at the provncal level. Other 6.1 Recognzng the essental role of the 911 emergency servce n the health servce contnuum, even f t does not come under the jursdcton of the LHINs, we recommend to the LHINS that they work wth the RMEFNO to dentfy potental solutons to remedy these gaps. 26 Next Steps The next step n mplementng these recommendatons wll be the development of the Jont Annual Acton Plan to ensure that the work plans of the LHIN- RMEFNO workng commttees can provde solutons to move forward on these recommendatons. These plans wll enable us to map out the plannng work that wll need to be done untl the next report perod. Throughout the year, formal workng commttee meetngs wll be used to update the acton plans and dscuss better ways of collaboratng to meet the objectves. In the feld, collaboraton between RMEFNO Communty Engagement and Plannng Offcers and LHIN Lason Offcers durng vsts to health servce provders, on the commttees to whch they belong and n the organzaton of actvtes wll soldfy the commtment to French language servces. The partcpaton of the LHINs and health servce provders as guest speakers n Carrefours santé meetngs s another effectve way for Francophones to express ther needs and for provders to present ther servces and provde useful nformaton for navgatng the healthcare system. The RMEFNO wll contnue to play an mportant role as support and resource for the LHINs and health servce provders wth regard to communty engagement and French language servces. The progress made thus far and the wll and ablty to work together wll enable us to move forward on prorty ssues to mprove the plannng and offer of French language health servces for the populaton of Northern Ontaro.
