2 MESSAGE Overview from of the Programs CEO & Services Dear Friends, Providing community benefit is an important part of our Mission. It represents a vital link to our community & neighbors. Our strength is in our history and Mission. The Mission and tradition of Inspired Care goes on; as it has since Every year we learn the importance of depending upon each other in times of need. We are concerned that the numbers of uninsured and underinsured continue to rise. We take seriously our stewardship to provide access and coverage to quality health care services. This year access to primary care was expanded through the hiring of eleven primary care providers, two surgeons, and the expansion of walkin services at our Endicott primary care practice. We expanded our hours for outpatient laboratory services to better accommodate the needs of our community. And we expanded access to emergency room services through the opening of our new emergency room. Throughout the year we continued to press forward to address access needs for children s oral health, pharmaceuticals, and behavioral health services for youth. Last year we recognized continued need for access to dental services. Increasing hours of dental services resulted in 8.3% more dental visits. The Hope Dispensary of the Southern Tier opened in August of marked the first full year of this free pharmaceutical assistance program. During 2010, together with Pharmacy Assistance Programs, Lourdes served over 1,800 persons and filled over 6,100 prescriptions which resulted in over $932,000 in free pharmaceuticals being provided to the community. 6% of those served by the Hope Dispensary were linked to a medical home. And Lourdes facilitated the enrollment of nearly 100 persons in a health insurance program. During the year we collaborated with the Broome County Office of Mental Health as they closed their children s mental health program. As part of Lourdes commitment to address the behavioral health needs of children and youth, Lourdes applied for and was approved by the New York State Office of Mental Health to be licensed as a children and family mental health clinic. Lourdes Center for Mental Health will provide a wide range of mental health services which include psychiatric assessment and treatment, medication management and psychotherapeutic counseling for individuals, families and groups. Our associates extended our ministry into the community by providing supplies to elementary schools, participating in food drives, and ensuring that over 600 persons were able to enjoy the blessings of the holiday season. I express my heart felt gratitude to them for living our Mission. As a leading health care provider, each year we provide millions of dollars in charity care & health education programs to our community. Lourdes is pleased to report to the community the services provided during 2010 which address the Community s Health Status and Health Priorities. We remain committed to our call to provide Health Care that Works, Health Care that is Safe, and Health Care that leaves No One Behind. 3 Overview Priorities for of the Programs Year & Services During , Lourdes collaborated with community agencies under the direction of the New York State Health Department and County Health Departments to address public health issues and underlying causes and contributing factors to health status and chronic disease. This report to the community demonstrates how Lourdes has worked over the last year to help the Greater Binghamton community in the following community health priority areas: Community Health Priorities Access to Healthcare Increase health care coverage Increased access to behavioral/mental health and substance abuse Increase access to prescription drug medication Preventing & Managing Chronic Disease, with a focus on decreasing obesity, diabetes, and reducing hospitalizations due to heart failure. The following chart gives an overview of some of the programs and services Lourdes offers that impact the community s health status. 5 Overview of Programs & Services COMMITMENT TO COMMUNITY HEALTH STATUS Access to Health Care Increased Health Care Coverage. Lourdes made a significant investment in helping to provide healthcare access and coverage to help the uninsured. Lourdes continued to employ six financial counselors to assist uninsured individuals to determine eligibility for healthcare coverage. Outreach was expanded by implementing an outbound call program to assist the uninsured in Lourdes primary care practices in accessing health care coverage. The number of persons who were enrolled in the Lourdes Patient Financial Assistance Program increased 17.3%.A 25% discount was provided to 26,000 accounts. Through Lourdes Dispensary of Hope of the Southern Tier, over 100 persons were linked to a medical home and nearly 100 were assisted in enrolling in a medical insurance plan. Increased Access to Primary Care, Prevention and Specialty Services. Lourdes expanded access to Broome and Tioga County residents by expanding outpatient laboratory hours at primary care locations and the hospital campus; expanding walk-in services at the Lourdes Endicott Primary Care Practice location; and through the opening of a newly renovated and expanded emergency room designed meet the increased needs of the community. Lourdes assisted the community in ensuring access to primary care by employing eleven primary care providers. Three surgical specialty physicians were hired or recruited to help with cancer and obesity prevention, early detection, and treatment. Pulmonologists will join the Lourdes team in Their expertise will aid the community in managing chronic disease and reducing readmissions. Increased Access to Behavioral & Mental Health, Substance Abuse, and Maternal, Child and Adolescent Health Lourdes Youth Services reached thousands of children and families in 2010 by providing evidenced based programming and clinical services. Funding for these services was provided by a variety of sources including federal and state funds and grants. Lourdes Youth Services administered 4.8 million dollars in 2010 to provide these services to the youth and families in our community. Ways in which these resources were used to address community health priorities were: Lourdes Parents and Children Together program (PACT) received an increase in funding from Healthy Families New York. This allowed PACT to increase the number of families served by this program. Lourdes Student Assistance Program received an increase in funding from Broome County which enabled them to add a counselor to Broome Tioga BOCES. Lourdes Center for Mental Health received approval to operate an Article 31 New York State Office of Mental Health Children and Family Mental Health Clinic. Lourdes Family and Schools Together (FAST), Mental Health Juvenile Justice (MHJJ) and PACT programs completed their first year of providing evidenced based programming to students throughout Broome County through their collaboration with the federally funded Safe Healthy Attitudes Require Education (SHARE) initiative. Increased Access to Prescription Drug Medication. Lourdes increased Pharmaceutical Access in the community through the Dispensary
6 of Hope of the Southern Tier marked the first full year of this free pharmaceutical assistance program. During 2010, together with Pharmacy Assistance Programs, Lourdes enabled over 1,800 persons to be served, over 6,100 prescriptions filled, which resulted in over $932,000 in free pharmaceuticals to be provided to the community. The total number of prescriptions which were filled directly through the Hope Dispensary of the Southern Tier grew 27.2% over the prior year. 6% of those served by the Hope Dispensary were linked to a medical home. Lourdes facilitated the enrollment of nearly 100 persons in a health insurance program. Chronic Disease Lourdes Regional Cancer Center provided outreach and education services for breast, cervical and colorectal cancer through the Cancer Services Program, community health fairs, and support groups. Lourdes partnered with Senator Tom Libous and United Health Services in a Time for a Tune Up prostate cancer program, which served nearly 100 men. Working on Wellness (WOW), a community partnership designed to address childhood obesity issues and their relationship to health, disease, and Type II diabetes. Strategic Alliance Grant partnered with the Broome County Health Department to address obesity within the poor and vulnerable pediatric population. Reducing Heart Disease through activities such as Mission Meltaway; maintaining a smoke free environment. Smoking cessation programs continue throughout the Lourdes Primary Care Network with the local Team Act coalition. Frail & Elderly Lourdes addressed the needs of the frail and elderly and their families through many venues, among which were participation in the Aging Futures community-wide coalition whose mission is to address the needs of the senior population in the community. Lourdes at Home served nearly 2,200 persons and provided more than 59,000 home health visits to Broome County residents. Lourdes expanded it s at Home tele-health monitoring program from 26 to 56 monitors. This evidenced based program has decreased Acute Care Hospitalization rates significantly for patients with Congestive Heart Failure (CHF). This expansion allowed Lourdes to expand its at home Chronic Disease Management program to include Chronic Obstructive Pulmonary Disease (COPD) as well as CHF. Lourdes continues to be the only Home Health Care Agency to service pediatric patients. Lourdes partners with Lourdes Health Support in the care of home ventilator patients. Lourdes provides the area s only palliative care services, and is the only hospice provider for Broome and Tioga county residents. Commitment to Health Care that is Safe Lourdes is committed to providing quality care to everyone we serve. By instituting a number of quality improvement initiatives, Lourdes continues to offer improved community health status. Cardiovascular disease, including stroke and heart failure, remains a significant health concern locally and nationally. Through Lourdes heart failure disease management programs and home care heart failure program, Lourdes met or exceeded local
7 Overview of Programs & Services and New York State clinical outcomes for Heart Failure, Pneumonia, and Surgical Site Infection. Public and professional education on stroke awareness, prevention and treatment was provided throughout the year. Lourdes continues to provide low cost screening for stroke prevention along with an annual stroke awareness day that is free to the public. Lourdes maintains its New York State Health Department Stroke Center designation. Infectious Disease & Pneumonia. Lourdes participated in the Institute for Health Care Improvement (IHI) 100k Lives Campaign, and is a designated mentor hospital, in recognition of efforts and results with the campaign. Issues being addressed through the campaign included ventilator acquired pneumonia, acute myocardial infarctions, preventing central line infections and preventing adverse drug events. Patient Safety Huddles. As part of Lourdes commitment to clinical excellence, Lourdes holds daily safety huddles with Lourdes associates from every discipline. At this huddle, Lourdes associates discuss the care we provided the previous day to learn how we can make it better for the patients we are caring for each day moving forward. ABOUT Lourdes For over 80 years, Lourdes has been a cornerstone of our community, offering resources that have made a difference. With our spiritual, Catholic faith tradition, Lourdes Hospital provides the compassionate care that every patient deserves. Lourdes is more than an inpatient acute care hospital we are an outpatient ambulatory surgery center we are a Regional Cancer Center. We provide Palliative Care and Hospice Care, an orthopedics program, diabetes care, home health care, and much more. Reaching beyond the boundaries of our main campus, Lourdes has established a network of primary care physician offices at convenient sites throughout the region. The Mobile Mammography and Mobile Dental Care programs provide women s health and mammography services and dental care through two mobile medical vans, making health care accessible to rural populations and children in need. Lourdes is also a member of Ascension Health, the largest, Catholic, nonprofit healthcare system in the nation with 113,000 associates serving in more than 500 locations in 20 states and the District of Columbia. Much has changed in the community and at Lourdes since the Daughters of Charity opened a small 25-bed hospital on Riverside Drive over 80 years ago, but one thing remains constant: the commitment and Mission of caring found here each and every day.
