This is a designated senior nurse position Primary Purpose The Nurse Practitioner will be expected to deliver, independently, accessible and high quality effective nursing care that is fully coordinated and integrated with other mental health services within the primary sector and the Provider arm services.

2 The primary goal for the Nurse Practitioner will be to assist in improving client health and reducing health inequalities and clinical risk for service users with identified high and complex mental health needs by:» Direct provision of specialist consultation and liaison services to support NGO and other community services to deliver high quality care to mental health service users» Development of alternative models of care for service users with identified high and complex needs» Demonstrating advanced nursing leadership and mentorship to RN colleagues, multidisciplinary teams, and community health workers» Delivering on the objectives of the Mental Health Te Kokiri Strategy by improving access, outcomes, reducing health disparities and reducing duplication of services Principal Accountabilities 1. Clinical Practice Works with patients, families and other members of the health team to improve health outcomes. Provides culturally safe practice based on the principles of the Treaty of Waitangi Utilises professional judgment, and advanced clinical assessment skills, to support effective decision-making in the assessment, plans and treatment of patients Utilises laboratory and diagnostic tests and clinical information systems to support clinical decision making Manage a caseload of high and complex mental health service users in collaboration with medical, nursing and other relevant members of the MDT, with a particular emphasis on rural service users Works in partnership with clients and whanau to plan care by identifying health promotion and disease management goals; and providing accurate information and education about relevant interventions Communicates, collaborates and coordinates care with other health professionals to ensure best outcomes Operates within a framework of current best practice and inter-professional clinical case review Documentation is professional, accurate and timely and consumer involvement in decision making is visible Demonstrates an ability to limit and manage adverse reactions/ emergencies/crises Recognises situations of risk for consumer and acts appropriately Key Performance Indicators Assessments and treatment plans are appropriate and demonstrate advanced knowledge and skills Evidence of health promotion and client education provided Nurse led clinics provided which are appropriate to the context of mental health and addiction service clinical management 80% of feedback confirms use of NP as an advanced resource for mental health and addiction service delivery Treaty partners confirm appropriate consultation and liaison

Professional Development and Leadership Maintains and advances own specialist clinical competence and professional development Contributes to ensuring practice standards are maintained within mental health and addiction nursing services. Actively involved in the development, provision and evaluation of educational programmes for other health professionals Represents mental health and addiction nursing specialty perspective at an organisational and national level Develops and maintains key partnerships crucial to the success of the role Communicates effectively, positively and courteously role modeling effective conflict resolution Works in partnership with other nurses within mental health and addiction service to develop nursing practice and clinical team processes within the service Advises on new legislation/guidelines and developing appropriate changes to policy and protocol within mental health and addiction services Key Performance Indicators Evidence of participation in review and development of service processes and policies/protocols. Evidence of ongoing professional development appropriate for maintenance of specialty knowledge Required certification skills are current Evidence of successful provision of education sessions for clinical team Evidence of ongoing maintenance of professional portfolio 3. Management of the Environment Proactively maintains health and safety for clients and staff Maintains infection control standards Actively manages clinical risk and contributes to quality and risk planning for the mental health and addiction service Participates as appropriate to the investigation of incidents related to mental health and services Able to respond appropriately in an emergency/crisis situation. Key Performance Indicators Evidence of participation in risk identification and mitigation strategies. Evidence of attendance at all relevant/mandatory training sessions and application of knowledge to clinical practice. 4. Continuous Quality Improvement/Research Leads quality improvement activities based on best practice, involving staff where appropriate Identifies and leads research initiatives within the specialty Identifies and is actively involved in clinical audit activities for the mental health and addiction service

4 Key Performance Indicators Evidence of quality improvements identified and implemented Evidence of audits identified, completed and remedial actions progressed. A function of BOPDHB is to provide a 24-hour service. This may at times necessitate you being required to change duties or transfer to another ward or department to ensure adequate coverage. This position description is not exhaustive and the incumbent may be requested to perform any reasonable task within the scope of the position as requested by the Line Manager. This position description will be reviewed from time to time in consultation with the incumbent. The key performance indicators are a guide only and the relevant indicators should be agreed at annual performance appraisal. Significant clinical experience in mental health and addiction services. Worked at senior nursing level (e.g. CNS or SCN) within a specialist mental health team for a minimum of 3 years. Community/Primary Mental Health Care experience Competencies What am I capable of? Computer competent Clinical leadership and attainment of high standards of evidence based care for patients Well-developed problem solving and analytical skills Skilled at prioritising work load and patient acuity requirements based on information systems and experience Knowledge and understanding of research findings to support evidence based practice Well-developed written and verbal communication skills Commitment to Treaty of Waitangi Committed to providing a culturally safe environment for clients and whanau Ability to prioritise and cope with high and varied workload Demonstrates a commitment to quality Flexible, adaptable and embraces change Professional demeanour and high level of personal integrity Knowledge What do I know? NCNZ registration as a Nurse Practitioner with current practising certificate Evidence of expert practice Current valid driver s licence

6 You agree to demonstrate flexibility and a willingness to perform a variety of tasks to promote and support BOPDHB initiatives. You are required to meet the Health and Safety at Work Act 2015 requirements as set out in the BOPDHB Health and Safety policies and protocols. This includes completing successfully any health and safety training provided by the BOPDHB. You are required to maintain a standard of health which will allow for the performance of all duties and functions of the position. All BOPDHB sites are smokefree environments. Health Practitioners Competence Assurance Act You are required to maintain your current competency based practicing certificate. 2. You must notify your Manager of any changes to scope or conditions on practice (determined by Regulatory Authority). 3. You must complete the requirements of any competency programme. 4. You must notify your employer of concerns relating to the risk of harm to the public of another health practitioner practicing below the required standard of competence. 5. Know the provisions of the HPCA Act as the governing legislation. Vulnerable Children Act 2014 Due to this position having contact with children and the BOPDHB s commitment to child protection, you will be subject to safety checks under the Vulnerable Children Act at the time of hire and thereafter as per the relevant legislation.
