Why Does ICD- 9 CM Need to Be Replaced? 30 years old Terminology and classification of some conditions are outdated and obsolete Running out of room Increasing lack of specificity Outdated codes produce inaccurate and limited data 13 Why Does ICD- 9 CM Need to be Replaced? Anticipate full benefits of the Electronic Health Record (EHR) can not be achieved without transitioning to ICD-10 Comparison with international data is hindered 14 7

ICD-9 CM vs. ICD-10 CM Similarities Same hierarchical structure Same conventions/notes/instructions Availability of not otherwise specified and not elsewhere classified options Differences Structural changes Organizational revisions New features 19 ICD-10 CM Format X X X X X X X Category Etiology, Anatomical site, Severity Extension 20 10
Code Set Comparison: Diagnosis Codes ICD-9-CM Diagnosis Codes 3-5 characters in length Approximately 14,000 codes First digit may be alpha (E or V) or numeric. Digits 2-5 are numeric Limited space for new codes Lacks detail Lacks laterality Difficult to analyze data due to nonspecific codes Codes are non-specific and do not adequately define diagnoses needed for medical research Does not support interoperability because it is not used in other countries ICD-10-CM Diagnosis Codes 3-7 characters in length Approximately 68,000 codes Digit 1 is alpha; digit 2 and 3 are numeric; digit 4-7 are alpha or numeric Flexible for adding new codes Very specific Has laterality Specificity improves coding accuracy and depth of data for analysis Detail improves the accuracy of data used in medical research Supports interoperability and the exchange of health care data between other countries and the United States 21 Example of Greater Specificity Chapter 12 - L00-L99 Diseases of the Skin and Subcutaneous Tissue and Other Diseases of the Skin and Subcutaneous Tissue L89 Decubitus ulcer L89.2 Decubitus ulcer with necrosis of the muscle L89.3 Decubitus ulcer with necrosis of the bone L97.01 Ulcer of the lower limb limited to breakdown L97.12 Ulcer of the lower limb with fat layer exposed, left limb L97.21 Ulcer of the lower limb with necrosis, right limb L98.42 Chronic ulcer of the skin with fat layer exposed L98.44 Chronic ulcer of the skin with necrosis of the bone 22 11
Example of ICD-10 CM Laterality 23 ICD-9 CM 880 Open wound of shoulder and upper arm Without mention of complication Complicated With tendon involvement ICD-10 CM S41.0 Open wound of shoulder S41.01X Laceration without foreign body of shoulder S41.02X Laceration with foreign body of shoulder Final character of the codes indicates laterality Right side is always 1 Left side is always 2 Bilateral is always 3 ICD-10-PCS 24 12
ICD-10-PCS Smart codes: each character in a given code string represents a separate data description set with a specific meaning, and provides additional information Each character can be reported on separately The complete and final code description will have multiple subdescriptions in its final and complete form Section Body System Root Operation Body Part Approach Device Qualifier 25 Code Set Comparison: Procedure Codes ICD-9-CM Procedure Codes 3-5 numbers in length Approximately 3,000 codes Codes are all numeric Based on outdate technology Limited space for new codes Lacks detail Lacks laterality Generic terms of anatomy Lacks description of methodology and approach for procedures Limits DRG assignment Lacks precision to adequately define procedures ICD-10-PCS Procedure Codes 7 alpha numeric characters in length Approximately 87,000 codes Codes are alpha numeric (no use of I and O) Reflects current usage of medical terminology Flexible for adding new codes Very specific Has laterality Detailed anatomical descriptions Provides detailed descriptions of methodology and approaches for surgical procedures Allows DRG definitions to better recognize new technologies and devices Precisely defines procedures with detail regarding body part, approach, any device used and qualifying information 26 13
Example of ICD-10 PCS Code Insertion of a tissue expander vs. Insertion of a tissue expander in the right breast, open approach ICD-9 Procedure Code ICD-10-PCS 0HHT0NZ 27 ICD-10-PCS Example: Repair of a fracture, radius w/ internal fixation device ICD-9-CM ICD-10-PCS ØPQJØ4Z 1 (Ø) 2 (P) 3 (Q) 4 (J) 5 (Ø) 6 (4) 7 (Z) 28 14
ICD-10-PCS Here is a breakdown of the multi-axial code assignment for the previous example using the code: ØPQJØ4Z 1 Section (such as Medical/Surgical - Ø) 2 Body System (Upper Bones - P) 3 Root Operation (Repair - Q) 4 Body Part (Left Radius - J) 5 Approach (Open - Ø) 6 Device (Internal Fixation Device - 4) 7 Qualifier-(none - Z) 29 Mappings How can we cheat and get from ICD-9 to ICD-10 and from ICD-10 to ICD-9? 30 15
