I am leading 18 different faculty and 48 different students from nursing, medicine, dental medicine, physical therapy, pharmacy, social work, and public health for a two day training and then 12 week outreach program to the homes of older persons, with a special focus on the care of veterans. 4. GOT Care! Geriatric Outreach and Training with Care Fall 2015 I am PI for this Interprofessional Geriatric Outreach and Training Program. I am leading 20 different faculty and 65 different students from nursing, medicine, dental medicine, physical therapy, pharmacy, social work, and public health for a two day training and then 12 week outreach program to the homes of older persons, with a special focus on the care of veterans. 5. Participated as faculty member from Nursing for the IPE Dean s Day Fall Presentation to new DNP student cohort in Scientific Underpinnings and Health Services Research Course (for Professor Juliette Shellman), August Describe the role of the DNP in the acquisition of large grant funding for a DNP/clinical practice project, and collaborative research experiences toward this effort. Discussion of possible palliative care DNP project opportunities. 7. GOT Care! Discussant for Undergraduate Student Development Program (PI Thomas Van Hoof) for 10 students November Participant in the Inter-track Procedure Day for Advanced Practice students with didactic lecture and lab time for instruction and practice of skills such as suturing, wound care, burns, subungal hematoma release, fish hook removal, monofilament test, foreign body in eye, and central line placement for acute care students September Interprofessional Collaborative Education Day September 2014 Representative from Nursing to carry out this day focusing on interprofessional leadership and clinical care training. Worked with a team of interprofessional faculty from Pharmacy, Physical Therapy, and the Leadership Office from UConn to plan and carry out a full day program for about 60 students from these various disciplines. 11. Interprofessional Geriatric Workshop Designed curriculum for didactic and clinical education, coordinated, administered and participated as clinician and faculty for an 8 week program of comprehensive geriatric assessment and management with 8 interprofessional faculty from nursing, medicine, pharmacy, physical therapy, and social work. Students of the program included physician residents, advanced practice nursing students, senior BSN geriatric capstone students, doctoral students in Pharmacy, and doctoral students in Physical Therapy. A full curriculum and training program was developed for approximately 66 interprofessional faculty and students from nursing, pharmacy, physical therapy, medicine, dental medicine, social work, and public health. All of the faculty and students participated in the training and then went on interprofessional home visits for older persons who have multiple chronic conditions and high emergency department use, as an effort to provide interprofessional feedback to primary care providers to improve these older persons quality of life and reduce this ED use.
