2 GENERAL INFORMATION The Associate Degree Nursing Program is designed to prepare qualified students for beginning employment as staff nurses giving direct care to clients. Upon successful completion of the program, students will be awarded an Associate of Applied Science Degree in Nursing and be eligible to make application to sit for the National Council Licensure Examination for Registered Nurses (NCLEX-RN). Accreditation The program is fully approved and accredited by: Arizona State Board of Nursing Accreditation Commission for 1740 W. Adams, Suite 2000 Education in Nursing (ACEN) Phoenix, AZ Peachtree Road, Suite 850 (602) Atlanta, GA (404) Locations Offered: Prescott and Verde Valley Campuses The lecture component of nursing classes is taught on campus. Students may be required to travel to either of the Yavapai College Nursing Program campuses for occasional selected experiences. All students are required to provide their own transportation to off-campus clinical activities. The College does not offer nursing courses in extension locations. The Nursing Department maintains a file for each applicant and student. The file contains documentation related to admission and progression in the program. See the Yavapai College General Catalog for information regarding Student Record Disclosure and compliance with the Family Educational Rights and Privacy Act of Credit Hours: One lecture credit hour generally requires 2-3 hours weekly of study in addition to class time. One lab credit hour requires 3 hours weekly in lab (including nursing clinical practica, preceptorships, or internships). Nursing labs also require considerable preparatory and post- clinical work. See Yavapai College catalog or class schedule for each course lecture and lab credits. The Nursing Program is very demanding and time intensive. In addition to classroom presentations, there are activities in the campus lab, clinical agencies, and discussion groups. Homework, multimedia, and out-of-class assignments are also required. Students who have work and/or family responsibilities are strongly urged to complete general studies courses, especially Anatomy and Physiology, before entering the first semester of the Nursing Program. All nursing students are required to meet with an academic advisor to create an educational plan. 2
3 IMMUNIZATION STATUS REQUIREMENTS The Nursing Program at Yavapai College follows the CDC Guidelines for Healthcare Providers when requiring and requesting documentation of a student s immunization status. CPR certification, and TB status are a requirement of all healthcare employers and a contract requirement between clinical agencies and the Yavapai College Nursing Program. Any student not having current documentation by the deadline for readmission will not be considered for readmission. Students should maintain copies of all their records since they are required by healthcare employers as a condition of employment. 1. MMR (Measles, Mumps, Rubella): Adults entering health care professions must have documented proof of immunity (titer) or proof of two MMRs in lifetime. Persons born prior to 1957 may be considered immune. 2. Tuberculosis: negative skin test, chest x-ray, or statement that student does not have active tuberculosis, signed by health care professional. 3. Hepatitis B: Documented evidence of completed series or positive titer. 4. Varicella: Documented evidence of two immunizations or positive titer. 5. Tetanus-Diphtheria-Pertusis: Current Tdap or TD booster upon expiration of Tdap. 6. Influenza: Immunization to be received annually in the fall on or before October 15 for the current flu season. CARDIOPULMONARY RESUSCITATION (CPR) CERTIFICATION REQUIREMENT BLS (Basic Life Support) certification for Health Care Providers must be current for the entire Nursing Program. CPR certification status is reported annually while students are enrolled in the program. The Nursing Program only accepts BLS Provider certification (CPR & AED) through the American Heart Association (AHA). An in-person course or a course with an in-person check off is required. A student without current documented BLS Provider certification (CPR & AED) will be dismissed from the Nursing Program. It is the responsibility of the student to maintain current certification. CASTLEBRANCH ACCOUNTS Students must update their Castle Branch immunization tracker prior to application submission. Other Castle Branch items including background check and random drug screening may be required if the student has been out of the nursing program for one semester or more. Payment for Castle Branch requirements will be the responsibility of the student. 3
4 TESTING FOR SUBSTANCE ABUSE Screening tests for alcohol and drugs are required for all students admitted to the Nursing Program prior to any actual clinical assignments. Random and for cause testing will also be done. The Nursing Program will make the arrangements. No advance notice will be given. FINGERPRINT CLEARANCE Federal law and agency accreditation rules require a DPS Fingerprint Clearance Card for nursing students. Please visit to determine how to obtain an Arizona DPS Fingerprint Clearance Card. For a person with no state or federal criminal record, the processing time is approximately one to two weeks from the date Arizona DPS receives the completed application packet. For those persons who do have a criminal record, the process may take six to thirteen weeks. ARTICULATION AND TRANSFER Students who graduate from the Associate Degree Nursing Program at Yavapai College also have the opportunity to transfer into a baccalaureate (bachelors) program at another institution. Students intending to transfer courses toward a baccalaureate degree in nursing should consult the catalog of the school to which they plan to transfer. If you would like assistance in selecting equivalent courses, please see an academic advisor. LICENSURE Graduation from the Yavapai College Associate Degree Nursing program is not the sole criteria for obtaining a license to practice nursing in Arizona. Licensing requirements are the exclusive responsibility of the Arizona State Board of Nursing (Nursing Practice Act A.R.S. Sections , et seq.), and students must satisfy those requirements independently of their satisfaction of any requirements for graduation from Yavapai College. The Arizona State Board of Nursing will deny licensure if the applicant has one or more felony convictions that have not previously been disclosed to the Board and has not received an absolute discharge from the sentences for all felony convictions three or more years prior to the date of filing an application. If this law pertains to you, your application will not be processed, and denial of your application will be instituted by the Board of Nursing. 4
5 Student Readmissions 1. Students who leave the Nursing Program at any level due to withdrawal, failure, or dismissal cannot be guaranteed an opportunity to return. Placements in each nursing course are on a space available basis. Students who exit the program in the first semester must re-apply to the Nursing Program. 2. The maximum allowable time lapse since last enrollment in nursing courses is one year indicating the student must apply for acceptance to the next beginning cohort group (start the program over). 3. Any student who seeks readmission to the Nursing Program must follow application procedures and program requirements in effect at the time of re-entry including but not limited to clinical partner requirements. Because nursing courses must be taken as a whole (i.e., theory and clinical must be taken concurrently), re-entering students are required to meet all requirements for the course of re-entry. Please be aware that there may be specific deadlines related to clinical course readmission. a. Clinical course credential deadline examples: VVMC & YRMC: 2-4 weeks prior to start of clinical rotation NAVAHCS: 5-10 weeks prior to start of clinical rotation myclinicalexchange account 3 8 weeks prior to semester start b. The Readmission Application should be submitted by the end of the current semester, up to the last day of the course 4. Based on reasons for leaving the Nursing Program, the following guidelines are used in making decisions regarding students seeking readmission to clinical nursing courses: a. Students who are dismissed because of academic dishonesty are ineligible to reenter the Nursing Program. b. The program generally runs at capacity, and due to strict accreditation guidelines, the program must adhere to these capacities. Readmission is only available to any student if space is available. Further, the location where a space may become available is not guaranteed. Students need to understand that they may be offered a location not of their choosing. c. Students who are not successful or exit a course will be permitted one re-entry if space is available from the course which they exited. Any subsequent failure to successfully complete the same course will make the student ineligible to reenter that course another time. A re-entry is defined as being registered for the course after the Yavapai College drop deadline (see academic calendar at for semester dates). 5
6 d. Students who drop a course or courses, and seek readmission due to an illness or injury will be placed on a waiting list after submitting a physician s release with the application for re-entry. The physician s release from treatment must state that they are able to return unencumbered from the reason that caused them to leave. The student will not be placed on a waiting list until documentation is complete. Students in this category will receive preference over students seeking readmission due to academic and/or clinical failure. When space becomes available, applicants are offered admission in the order that they appear on the waiting list. e. Students seeking readmission due to non-health related circumstances or program compliance issues will be required to provide additional documentation and/or complete additional requirements depending upon the nature of the withdrawal or dismissal. f. Students seeking readmission must complete a remediation specific to the withdrawal or dismissal including proof of current program compliance. The student must document that any recommendations or requirements for re-entry made at the time of program exit have been satisfied. The remediation plan is part of the readmission application requirements to be submitted to the Yavapai College Nursing Program Admissions Committee. g. For students who are placed on the Wait List, the prioritization of candidates will be as follows: Those who withdrew for a health-related issue will be placed first on the list. Students with non-health related circumstances other than program compliance issues or academic failure will be considered in this category after any remediation recommendations and requirements have been met. Should re-entry not be available after two semesters, the student may apply to the Nursing Program as a new applicant. Students with academic failures or program compliance issues will be ranked in order of re-entry using a ranking rubric. Re-entry is dependent upon seat availability. The final course grade will be a top criterion for students with academic failure. Time on wait list will not be a consideration for readmission. Ties for students in equal standing on the wait list will be determined first by GPA of course failure or withdrawal (as applicable), followed by the admissions matrix score (YC Nursing Program Selection Criteria) and current overall GPA. 6
7 5. Readmission Committee and Student Process a. Readmission Committee Members Advising Disability Resources Faculty member who does not know student YCNP Admissions Committee Member(s) b. Student Process Student meets with an academic advisor to go over readmission process. Application form must be signed and dated by the advisor to be valid for submission. Student submits a written request for readmission to the Nursing Program to notify the Nursing Director and Admissions Committee of their intention to seek readmission. The course title and semester to be considered for readmission must be included in the request. The Nursing Program Director notifies the Yavapai College Nursing Program Admissions Committee and provides documentation submitted by the student. Student completes a remediation specific to the withdrawal or dismissal The student is responsible for submitting a complete including the following: 1. Cover letter outlining success plan for the semester the student is reapplying for 2. Letter of recommendation from course instructor, clinical instructor or SNA advisor stating the student s ability to be successful in the course student is seeking readmission to 3. Copy of remediation plan with proof of current program compliance 4. Completed Readmission Application Form dated and signed by academic advisor 5. Readmission Agreement signed and dated by student 6. Receipt for $50 Readmission fee (payable at college s cashier office or online at 7
8 Student meets with committee at set review date. Notification of review date and time is sent to the student s YC scholar account. Failure to meet with the committee will forfeit consideration for seat placement. Committee meets to make decisions based on standing and seat availability. c. Students offered readmission must submit documentation to the Nursing Director that any recommendations or requirements made by the Readmission Committee have been satisfied. 6. Re-admitted students who repeat courses in which they previously achieved a passing grade must again achieve a passing grade in that course to progress in the program following re-entry. 7. 11 Appendix A Yavapai College Nursing Program Selection Criteria Admission to the program is based on a 1000 point system, utilizing the criteria listed below. Students will be required to provide proof of residency (tuition residency is defined by the Arizona State Legislature and is outlined in the Yavapai College catalog). Proof of course work and registration history at Yavapai College will be established by the records on file in the Registrar s office. Final selection and placement is determined by the Program Admissions Committee. Proof of attendance at a Pre-Nursing Workshop within the last 12 months of application submission is required. Applicants are ranked using the Yavapai College Nursing Program Selection Criteria. The 48 highest ranked applicants will be offered seats, based on space availability. The next 10 applicants on the ranking will be offered alternate status. When several applicants have the same ranking score, the following criteria will determine tie breakers: 1. Overall GPA (Category 1) 2. HESI Cumulative Score (Category 2) 3. Number of times Pre-requisite courses from Category 8 were taken All applicants will be notified by mail after final determinations have been made.
