National Health Mission, J&K invites applications from Ayurvedic Graduates and Staff Nurses (only Fresh candidates) who are willing to serve in rural/ hard to reach area as Mid Level Health Provider (MLHPs). This course is a part of an initiative to strengthen Sub-Centres as Health & Wellness Centres (HWCs) for implementation of public health programmes and to enable Comprehensive Primary Health Care Service delivery, including disease prevention and health promotion. Candidates who will successfully complete the said course will be posted at identified Sub-Centres as Mid Level Health Providers (MLHPs) to lead primary health care team. The certificate course will be implemented through IGNOU and District Hospital Gandhi Nagar, Jammu, District Hospital Udhampur in Jammu Division and District Hospital Anantnag, District Hospital Baramulla in Kashmir Division will be the Programme Study Centres for the programme. The interested eligible candidates are advised to submit the application forms complete in all respects by or before 15 th December, 2018 till 4:00 pm in the office of State Health Society, at Regional Institute of Health & Family Welfare, Nagrota, Jammu for Jammu division and Divisional Office, NHM, Chanapora Srinagar for Kashmir division. The screening test of eligible candidates will be held on 22 nd December Time and venue of examination will be uploaded on the website jknhm.com. Eligibility criteria:- S. No. 1 Name of Certificate Course Bridge Programme (6 months certificate course) in Community Health for Ayurvedic Graduates. Eligibility BAMS from a recognized Institute/ University and willing to serve at Sub-Centre level/ HWCs in rural & hard to reach areas of State. Maximum age limit 40 years. At least 2 years of relevant post qualification, working experience in healthcare services from recognized Course Duration Six Months

2 Public/ Private Health Institutions or NGOs. 2 Bridge Programme (6 months certificate course) in Community Health for Staff Nurses (GNM/ BSC Nursing). Staff Nurses with Diploma/ Degree in GNM/ BSc Nursing from a recognized institute and are willing to serve at Sub-Centre level/ HWCs in rural & hard to reach areas of State. Maximum age limit 40 years At least 2 years of relevant post qualification working experience in healthcare services from recognized Public/ Private Health Institutions or NGOs. Six Months Note: 1. All applications will be subjected to a screening process and only shortlisted candidates will have to undergo a written examination conducted by State health Society. 2. All essential documents will be verified on the day of written examination/ counseling. 3. Please note that 1 st December 2018 shall be taken as reference date for computing, experience, age, qualification etc. 4. Details of number of vacancies available in each district is uploaded on the official website and also in NMH office Jammu/ Srinagar. Vacancies indicated are tentative and subject to change which shall be notified from time to time. Terms & Conditions: 1. The engagement shall be purely on contractual basis for one year and the contract may be renewed further based on the performance of the candidate and approval of post in PIP of NHM for the next financial year. 2. The candidate should be permanent resident of J&K State. 3. Preference will be given to local candidates based on the district merit. 4. List of documents to be attached with the application: - i. Date of Birth Certificate. ii. Permanent Residence Certificate, in case of married female candidates will submit either her own PRC (post marriage) or the PRC of her husband. iii. Marks certificates of all years/profs of the requisite qualification. iv. Degree Certificate from the recognised University/ institution. v. Internship Completion Certificate. vi. Registration Certificate from Board of Ayurvedic System of Medicine, J&K.

3 vii. Diploma/ Degree recognised from State Medical Faculty. viii. Nursing counsel registration Certificate/ SMF registration Certificate. ix. At least 2 years of relevant post working experience in healthcare services from recognized Public/ Private Health Institutions or NGOs issued by competent authority bearing name/ designation/ date and stamp of the issuing authority. 5. Applications not falling in the prescribed criteria or without aforesaid documents shall be rejected out-rightly. 6. Final selection of the candidates will be done based on the district wise merit in written test and interview/ skill assessment. The number of candidates for each district will be as per the number of available vacancies as per the list. 7. All the selected candidates shall have to submit a bank guarantee of rupees Sixty Thousand (Rs.60000) in favour of FA & CAO, NHM, J&K for a period of one (1) year. 8. At the time of engagement the selected candidate shall also submit an affidavit/ undertaking duly attested by First Class Judicial Magistrate to the extent that mere completion of training course shall not have any claim for hiring in the State on Contractual basis under NHM.
