While we each celebrate different holidays, I have to confess that my favorite is Thanksgiving. It is a time of reflection and gratefulness. As nurses, each of you makes a profound difference in the lives of others, and together we make an incredible impact in our society. Thank you for your hard work and dedication, especially during a year of many changes. One of those changes is the new nursing strategic plan. The Nursing Executive Board and I have been working to develop a threeyear plan to enhance, improve and build a STRATEGIC PLAN: Preparing for Tomorrow s Needs Nursing practice at Medical Center is about to change in a profound way, as executive leadership prepares for the final stages of the new strategic plan. We were not trying to do a plan for nursing in one entity or one facility. We were looking at a plan for nursing as a whole across the institution, said Marilyn Dubree, R.N., M.S.N., executive chief nursing officer. We wanted something that was really aspirational and would take us to a different place several years out from where we are today. The key word is aspirational, Dubree said. This strategic plan does not contain the nuts and bolts for everyday practice, but rather highlevel goals that are intended to challenge and enhance every nurse at the Medical Center. The first goal relates to the model of care with the hope of creating a strong team that practices personalized care. 2 STRATEGIC PLAN continued from page 1 The best work we can do for patients and families is related to how we coordinate ourselves on their behalf. If you want to have a strong team, every single person who plays a role on the team has to be strong in their own right. By sheer number our 5,000 nurses we are the biggest players on the team, and we need to know how to play well on that team. The second goal is related to Evidence- Based Practice. Dubree said VUMC nurses use evidence well, but need to take it further by developing new knowledge and sharing it in a meaningful way outside the institution. We ve been very involved in the doing of good work. We have sometimes neglected sharing it outside our Vanderbilt community because we ve been so busy doing the good work. We don t write, we don t go out and tell Goals of the Strategic Plan Personalized Patient Health Care Model Transform the way in which health care is delivered in order to ensure highly reliable personalized care, taking advantage of the capabilities and unique contributions of the entire care delivery team. Evidence-Based Practice and Effective Processes Lead the nation in producing evidence that will drive nursing practice, recognizing and legitimizing the evolution of knowledge in a rapidly changing environment. Create passion and discipline for the translation of evidence into practice that will optimize patient outcomes. Transformational Leadership and Professional Development Provide current and future health care leaders and care providers the environment, tools, evidence and skill development to lead during a time of health care reform and transition. people, we don t look at ways we could compare what we were doing before, Dubree said. The third goal rethinks the leadership structure and provides leaders with proper resources. For example, the Nurse Residency Program is attracting outstanding nurses from throughout the country, and the current leadership structure needs to expand and usher in the future of VUMC nursing. I don t want to imply that I think we have anything other than the finest leaders. But I know that we re going to come up short if we re not prepared to think about how we do this differently, she said. It is a realization that the work is going to change in complexity, acuity and variety, and we leaders need to challenge the way we do things. We need leaders who deal with generational issues, leaders who are comfortable with new technology and innovation, leaders who will know how to lead interprofessional teams differently than we have in the past, leaders who might not be nurses. The strategic plan is expected to be in place for three years, but could change as the goals evolve. The plan was developed by members of the Nursing Executive Board, which is charged with leading strategy for continued on page 3 2
3 continued from page 2 VUMC nursing. Planning began in early 2009 when nurse leaders were in place for all entities. When Jeff Balser, M.D., Ph.D., became Vice Chancellor for Health Affairs in June, the Executive Board ensured their developing plan matched Balser s goals for VUMC and the wider university strategy. Throughout October, Dubree reviewed the plan with all major nursing leadership groups, including the chairs of the staff nurse councils, and other key stakeholders, such as School of Nursing. The Executive Board will also seek input from frontline nurses at unit boards, staff meetings and other venues. It s important that we have a vision of the direction we need to go in, but I don t have all the answers, Dubree said. I hope people will work with us to come up with the best ideas. I want to know what energizes our nurses, what concerns them and how they see themselves contributing to these goals. THE DISH ON THE D.N.P. PROGRAM The Doctor of Nursing Practice (D.N.P.) program offered by School of Nursing is now in its second year, and many VUMC nurses are taking part. Graduates of the program will be expert practitioners who apply and translate evidence into practice directly with patient populations or within systems of care. Rather than conduct research like their Ph.D. colleagues, they may partner with members of research teams The program is directed by Donna McArthur, Ph.D., F.N.P.- B.C., F.A.A.N.P., professor of Nursing. The promise our D.N.P. students is bringing to practice, the leadership roles they have and the potential for impacting health care is phenomenal, she said. Marlee Crankshaw, R.N., M.S.N., nurse manager of the Neonatal Intensive Care Unit and Newborn Services, is in the second year of the program. She said the D.N.P. is a perfect fit for her. With my D.N.P. degree, I expect to take the research done by the Ph.D.s and put their findings into practice at the bedside. More than half of the D.N.P. students are from Tennessee, but the remainder are located as far away as Alaska and Maine. PROGRAM specialties, from family nurse practitioner to informatics, and range in age from late 20s to early 60s. The program is structured so that students meet on campus once per semester for an intensive experience with their cohort and faculty. The remainder of the coursework is completed via Webenhanced modalities. To balance her nursing duties, family life and the D.N.P. program, Crankshaw said she has turned into a weekend warrior. When I get home on Friday night, I begin a marathon focus on my D.N.P. work all the way through Sunday night, she said. Of course, I still have to juggle mother, grandmother, wife and homemaker hours amidst all the work. Each night, no matter how tired or late it is, you have to be disciplined enough to devote time to reading, assignments and online work with your group colleagues. Though she hasn t completed the program, Crankshaw said it has already changed the way she practices nursing. Each semester I have brought something with me to my role as manager. I look at things differently now I find that I am now looking for evidence for practices and the bigger picture. The capstone of the D.N.P. program is a scholarly project. Students identify a health care problem or issue from their practices they want to answer. The types of projects include quality improvement, implementation and evaluation of evidence-based practice guidelines and designing new models of care, McArthur said. In July, the program received a three-year Advanced Nursing Education Grant from the Health Resources and Services Administration. This will allow the program to expand to include a B.S.N. to D.N.P. trajectory and electives in global health and education. continued from page 3 VUMC Nurses in the D.N.P. OCTOBERFEST POSTER PRESENTATIONS The third day of Octoberfest was designated Evidence-Based Practice Research Day and included research and evidence based practice poster presentations in the North Lobby of Light Hall. Thirty-three nurses presented research on topics including the new nurse resident buddy system, urinary tract infection prevention strategies, and patient education for healthy hearts. 5
