2 Overview An electronically delivered copy of a patient s ambulance care record Delivered into a GP practice s TPP SystmOne (v5.01 and above) task box Only for non-conveyed patients registered with a TPP SystmOne GP Practice The epcr is not a referral it is for information purposes only (ambulance staff will notify practices of a referral in the normal way - by phone, etc.). Used with all TPP SystmOne GP practices (340+ in Norfolk, Suffolk, Essex, Cambs, Beds and Herts) Average of two ambulance epcrs per TPP SystmOne GP practice per day Concise and legible and recorded permanently in the GP s TPP system. GP practice GP Notifications user guides available Minimal change to GP practice s internal processes No cost to East of England TPP SystmOne GP practices GP practices (TPP SystmOne) will have a professional and timely document detailing ambulance care for their non-conveyed patients

3 Background During the past two years EEAST has implemented the Emergency Care Solution (ECS) on Panasonic Toughbooks Now on the majority of Trust vehicles (all DSAs and many RRVs) An electronic Patient Care Record (epcr) is created in the ECS for the majority of patient contacts (the rest are on paper) whether they are conveyed or not

5 Treated at home (non-conveyed) Calls to emergency services are recorded in a Computer-Aided Dispatch (CAD) system, which captures information such as patient demographics and the urgency of the call The demographic data links with the epcr, which is automatically available to access by the paramedic on the Toughbook During face-to-face patient assessment, further information is captured e.g. incident, clinical and treatment, creating a patient epcr The patient is asked if they consent to share their information. If they do and they are not conveyed and their GP practice uses the TPP SystmOne clinical system (v5.01 and above) - a copy of their epcr is sent electronically into the GP practice s TPP SystmOne ECS task box The GP practice can then action (e.g. to Read Code (XaPCc Seen by an Ambulance Crew ) and update the record to appear in the Journal)

9 Benefits for the GP practice A comprehensive and professional record of ambulance care that is legible, consistently presented and uses common terminology Very timely as will be in a practice s TPP SystmOne system soon after (nonconveyed) ambulance care has been given Supports the clinical pathway and treatment/management decisions Improved GP knowledge about ambulance care activities Improved community knowledge (if on TPP SystmOne and have a legitimate relationship with the patient) about ambulance care activities Improved practice knowledge about frequent callers Supports practice QOF figures Potential reduction in inappropriate visits, care or treatment Patient s ambulance care record permanently in practice s records Will not have to scan ambulance A3 paper records into practice s system

11 Deployment plan The Trust plans to use GP Notifications with all practices in Norfolk, Suffolk, Essex, Cambs, Beds and Herts which use TPP SystmOne version (v5.01 and above). We will: o engage with CCG/GP/IT leads (or CSU/GP/IT leads, as appropriate) o present EEAST s GP Notifications deployment plan and GP practice and patient benefits to CCG/GP/IT leads and TPP, GP and practice managers groups and/or TPP user groups o issue EEAST s GP Notifications user guide and information packs to the aforementioned leads (for TPP SystmOne practices) o Confirm go-live dates to aforementioned leads (for confirmation to local GP practices) o practice go-lives o set up feedback meetings about the benefits of receiving EEAST s GP Notifications

14 FAQs Why are the GP Notifications only for non-conveyed patients? EEAST is working with the National Programme for IT (NPfIT) to enable GP Notifications to be sent to TPP SystmOne GP practices for conveyed patients (for instance those patients who we take to A&E). We will inform all TPP SystmOne practices when this service is available. Why are GP Notifications only for TPP SystmOne practices? Currently the only interface between EEAST s Emergency Care Solution (ECS) is the TPP SystmOne (V5.01 and above). However the Trust is working with the ECS suppliers and EMIS and IN Practice Vision to enable the interfaces with other GP clinical systems as well. Again, we will inform all GP practices when this service is available.
