We provide services in more than 50 locations, ranging from the inner-city and suburbs to regional and remote parts of Australia. Our vision is for full citizenship for all people living with a mental illness in Australian society. Our mission is to improve mental health and wellbeing in local communities across the country. We believe that recovery is an individual process and that with the right kind of support everyone can live a life based on their own strengths, values and goals for the future. We use an approach called the Collaborative Recovery Model (CRM) to support people through our services. The CRM assists individuals to identify their personal strengths and values, to set goals and then helps them make progress towards achieving them. 2 Position overview The Enrolled Nurse will provide nursing services across the Street to Home service leading the primary healthcare response. This position forms part of the Street to Home multidisciplinary team of health professionals, street outreach and peer workers to provide culturally, appropriate, assertive outreach assessment and case management services to homeless people sleeping rough. The Enrolled Nurse will work with the Street to Home team to provide primary health services with consumers sleeping rough in public space, identifying and treating any immediate health issues and referring to appropriate health service for follow up. The Enrolled Nurse will engage people who are homeless and disadvantaged in a process where they can access and learn how to understand and maintain optimum health and wellbeing for self, partners and children. The Enrolled Nurse will provide leadership and work with the Street to Home team to provide a range of services that address the health needs of individuals and families including children presenting or in contact with Street to Home street patrols, outreach program and in post exit support and facilitate access to mainstream and specialist health services by advocating and providing information and referral to individuals and families. Neami Street to Home service provides culturally appropriate, assertive outreach and case management with people who are homeless and sleeping rough in the Adelaide CBD within a Housing First approach. The Enrolled Nurse will participate in regular foot patrols to identify people rough sleeping through assertive street work patrols and will provide support for people accessing the Street to Home clinical services based at the Hub. The assertive outreach model is aimed at providing integrated services that focus on consumer identified goals, improved health outcomes and transition from homelessness to safe, secure and sustainable long term housing. We persist and work with people over the medium to long term supporting people to maintain their tenancies once they secure housing. The Enrolled Nurse will be expected to work to the Street to Home Service Model as per the contract, with a housing first approach and actively participate in The Adelaide Zero Project - ending street homelessness in the inner city. Period of employment Maximum term contract until 30 June 2019; subject to a 3-month probationary period. Accountability The Enrolled Nurse will be accountable to the Service Coordinator Clinical. Conditions of employment The terms and conditions of employment will be in accordance with the Nurses Award 2010 Enrolled Nurse Level 2 (above Award salary dependant on experience). A number of benefits are available to staff, including generous salary packaging with rewarding NFP fringe benefit tax exemptions. Criminal record checks are mandatory for all new appointments. Neami National will cover the cost of an Australian check. Where a new employee has lived outside of Australia for 12 months or more within the last 10 years, the cost of an International check will be borne by the applicant (~$142.00). Working with Children check and Vulnerable Persons check required before commencement of work (employee responsibility). You must maintain a right to work in Australia, in the position and location of employment during your employment with Neami. 4 Section C: key responsibilities Provide Professional High Quality Consumer Care Aimed at Improving Consumer Health Outcomes Provide competent and responsive health care with people who are homeless and disadvantaged appropriate to age, gender, culture, sexual orientation and disability Assess consumer s needs, plan, implement and coordinate appropriate changes in condition and care Assist in the delivery of an effective street patrol and outreach nursing service in collaboration with the GP, Registered Nurse and Street to Home Service Manager and Coordinators Engage consumers, using a strengths-based approach to complete a health status measurement and a needs assessment. Using the Collaborative Recovery Model (CRM) protocols work collaboratively with consumers to identify their needs, set goals and develop a plan to meet those goals Facilitate group processes which engage individuals and families around their health and wellbeing which assist them a) understand their health and wellbeing as individuals and in relation to family, friends and children b) learn options for being proactive in their self-care and care of dependent others c) know where to seek assistance Engaging with pregnant women who are homeless and with team members assist preparation for birth and parenthood Provide health education, counselling and / or therapeutic / rehabilitation programs to improve the health outcomes of individual consumers or groups Monitor consumer care plans and participate in clinical auditing and/or evaluative research to ensure appropriate consumer care outcomes are achieved on a daily basis Demonstrate and promote risk minimization approach to practice and support implementation and maintenance of systems to protect consumers and staff Supports the Registered Nurse to provide SOWs and Peer Workers with skills to identify clients at risk. I.e. identification of seizures, treatment of seizure, when to call Ambulance. Identify respiratory distress and when to call an Ambulance Provide services that are consistent with the Neami Clinical Governance Framework Work within the parameters of Neami s policies and procedures in addition to any professional codes of conduct Ensure incident and critical incident reporting occurs in accordance with guidelines Clinical service delivery Provide professional practice in accordance with the National Competency Standards for the Enrolled Nurse Comply with the Code of Ethics for Nurses and adheres to relevant legislative requirements pertaining to health care and the nursing profession. Work within the scope of the Code of Professional Conduct for Nurses Provide quality consumer care using an evidence based approach Participate in regular audit processes to ensure compliance with applicable regulatory bodies is maintained Regular monitoring of consumer related documentation to ensure compliance with applicable legal and regulatory bodies Undertake special projects or reports required by the Service Manager on a wide range of issues 4 of 9

5 Provide services to consumers from a variety of cultural and ethical groups Participate fully as a team member Establish and maintain effective communication within interdisciplinary teams to ensure consumers receive quality nursing care Foster a learning environment by participating in reciprocal guidance and coaching with other staff to share clinical knowledge and develop and inform your own practice by drawing on the expertise and insights of other staff Participate in regular supervision and an bi-annual performance review Actively participate in reflective practice through team meetings, decision-making processes, service planning sessions, supervision, and staff development activities Raise and seek to resolve any areas of conflict or dispute with other staff in an open, honest, and respectful manner Support Neami National efforts in reducing our impact on the environment and work towards a sustainable future Working with community partners Plan and coordinate services including those of other disciplines or agencies as required to meet individual and/or group health care needs Involve carers, family and friends as identified by the consumer in the individual service plan for the consumer Will participate as directed and undertake activities outlined in local activation protocols when a Code Blue or Code Red event is activated under the Rough Sleeper Code Blue Plan Statewide and the Rough Sleeper Code Red Plan Statewide Maintain records and resources Collect, collate and maintain data on consumer contact With team members collect information on community resources Regularly report to the Manager regarding achievement of work plan Ensure all consumer data is entered into H2H data base Complete all documentation related to service delivery within a timely manner 5 of 9
