2 A Guide to our Care Groups We provide hospitals and community services to our 365,000 residents and the 16 million visitors who come to the Morecambe Bay area on holiday each year. Traditionally the Trust managed hospital services but in April this year 400 staff providing community services in southern Cumbria joined the Trust from Cumbria Partnership NHS Foundation Trust, and on October 1st they were joined by 200 staff providing community services in north Lancashire, who were formerly employed by Blackpool Teaching Hospitals NHS Foundation Trust. In the following pages you can find out more about what our clinical services do, the services they provide and the numbers that tell their story. 3 Medicine Care Group 40% of all patients arriving in our Emergency Department are triaged within 15 minutes. OPEN We opened the Trust s Diabetes Centre in 2017 dedicated to providing specialist outpatient services for people living with Diabetes in North Lancashire area whilst supporting the education of staff and patients on self-care. 35% Reduction in agency spends compared to previous years for recruitment of Medical Staff. 4 Critical Care & Surgery Care Group 25-30,000 No of surgical procedures carried out annually. 68% of surgical beds were saved thanks to a series of improvements in ambulatory care and the expansion of the Surgical Emergency Ambulatory Care Unit at the Royal Lancaster Infirmary and Furness General Hospital. This means that a huge number of unnecessary patient admissions were avoided. In the last12 months the Care Group has also improved recruitment, and continues to work to address staffing levels especially in Anaesthetics, Theatres, Nursing and Breast services. 7 Integrated Community Care Group Currently our Integrated Community Care Group deliver more than 15 distinct health and proactive preventative care services to people in their own homes, local communities and into the local bed base across South Cumbria and North Lancashire. From October 1 the care group will welcome more than 200 additional staff who operate community services in North Lancashire. 8 Integrated Community Care Group 11 No of Integrated Care Communities (ICC s) Bay Wide made up from teams of health and social care professionals who provide proactive preventative care, through care planning to improve the health and well-being of our citizens whilst supporting sustainability in the health economy. Holistic assessments Through the ICCs the integrated teams work directly with their communities mobilised for health and wellbeing to ensure safe services are delivered by the right person with the right skills. Holistic assessments are undertaken to identify and work through issues that may decrease the patient s ability to self care. The teams will support a persons independence and the ability for our citizens to remain at home. The District Nursing Service Specialises in the complex management of patients in the community. Each team is led by a district nursing sister/charge nurse who is responsible for leadership, assurance of standards of care delivery, maintaining patient safety and effective relationships with general practices, secondary care providers, social care and other local care providers. The range of clinical need and care is wide and varied and becoming more complex due to the acuity and dependency of patients. The core purpose of the District Nursing service is to support people to maximise their health and well-being in their own home and to avoid hospital admissions when it is safe to do so. 60,000 No of face to face contacts our District Nurses make per year across South Cumbria
