1 PFCCS COURSE AN OVERVIEW CYNTHIA TINSLEY, MD LOMA LINDA UNIVERSITY CHILDREN S HOSPITAL PEDIATRIC CRITICAL CARE FUNDAMENTAL COURSE The PFCCS course was developed by the Society of Critical Care It is a two-day (16 hours) course It is aimed to an audience of non intensivisits Nurses Physicians Respiratory Care Practitioners DESCRIPTION Helps prepare the student to recognize and care for patients with acute deterioration Develop ability to manage critically ill children for the first 24 hours of stabilization Before transfer to the care of a pediatric intensivist Before transfer to a PICU 1
2 GOAL OF THE COURSE AT THE COMPLETION OF THE COURSE THE STUDENT WILL BE ABLE TO : Prioritize assessment needs for the critically ill or injured infant and child Select appropriate diagnostic tests Identify and respond to significant changes in the unstable pediatric patient Recognize and initiate management of acute life-threatening conditions Determine the need for expert consultation and/or patient transfer and prepare for optimal transfer COURSE Fundamental principles of critical care is tailored to the pediatric population Initial management before critical care assistance or transfer Goals Avoid morbidity Prevent mortality COURSE AUDIENCE Not a substitute for critical care consultation or patient transfer Targeted to the non-intensivist or the healthcare professional who is certified in : Basic Life Support (BLS), Advanced Pediatric Life Support (APLS), Pediatric Advanced Life Support (PALS) Prolonged care is not the primary emphasis 2

8 CARDIOVASCULAR AND SHOCK CONTENT Define Shock Identify the 4 major categories of shock and their typical presentation Determine the initial steps in the evaluation and management of a child in shock Discuss the goals of fluid resuscitation for pediatric patients in shock Describe the physiologic effects of vasopressors and inotropic agents CARDIOVASCULAR AND SHOCK CONTENT Things you will learn What are the management priorities in patients with shock? What are common pitfalls in the management of pediatric shock? Why is cold shock difficult to reverse? CARDIOVASCULAR AND SHOCK CONTENT Types of Shock that are reviewed 1. Hypovolemic shock 2. Cardiogenic shock 3. Distributive shock 4. Septic shock 8
9 RESPIRATORY CONTENT Explain anatomic and developmental differences that make infants and children prone to disorders of the upper airway Identify common disorders of the upper airway Identify respiratory failure Initiate appropriate diagnostic and therapeutic intervention for the various causes of acute lower airway disease MECHANIC VENTILATION CONTENT Identify the indications, advantages and disadvantages of non invasive positive pressure ventilation Learn the characteristics of the different mechanical ventilators modes Select ventilator settings for the initiation of mechanical ventilation Describe how to adjust ventilator settings based on patients needs SEDATION, ANALGESIA AND NEUROMUSCULAR BLOCKAGE CONTENTS Explain the various benefits and side effects of commonly used sedating agents Describe the benefits of therapeutic analgesia and side effects in critically ill patients Know reversal agents for benzodiazepines and opioid agents Describe the indications and potential risks of neuromuscular blocking agents 9
10 TRANSPORT OF THE CRITICALLY ILL CHILD CONTENT Describe factors that influence the method and mode of transport within and between hospital facilities Discuss transport considerations and practical issues Identify the equipment necessary for a safe transport POST OPERATIVE MANAGEMENT CONTENT Describe postoperative pain management Summarize the management, fluid electrolyte, and nutritional support Identify and recognize the potential postoperative complications TRAUMATIC INJURIES IN CHILDREN-CONTENT Rank priorities for the management of the injured child Employ life saving therapies in the injured child Describe how children s patterns of injury differ from those of adults. 10
11 NEUROLOGIC EMERGENCIES CONTENT Describe the normal physiology and of the intracranial vault and cerebral blood flow Review the emergent neurologic assessment of the pediatric patient Summarize the acute management of seizures and status epileptics Describe how to manage intracranial hypertension FLUIDS AND ELECTROLYTES CONTENT Summarize basic concepts in the maintenance of intravascular fluid homeostasis Discuss the common electrolyte disturbances, their recognition and management Describe common endocrine emergencies including those related to glucose metabolism and discuss their management COURSE CREDIT Upon successful completion, the participant will be awarded 14.5 contact hours of continuing education credit. Loma Linda University Medical Center staff development is authorized by the California State Board of Registered Nursing to offer this course for 14.5 contact hours.
