1 Position Description Position Details: Title: Registered Nurse (Case Manager) Department: Renal Reports to: Charge Nurse Dependent and Acute Haemodialysis Location: Dependent and Acute Haemodialysis Units Date: December 2007 Purpose of Position: To provide contemporary renal nursing care based on a comprehensive assessment that is patient centred, culturally sensitive and evidenced based. A case management model of care is utilised to ensure patients receive an optimum level of continuity of care. Key Accountabilities: 1) Provide haemodialysis patients and their family/whanau with ongoing skilled nursing care and follow up, through comprehensive case management in the community to optimise quality of life and minimise complications associated with haemodialysis. 2) Teach, guide & support patients (and their family/whanau) with end stage renal failure (ESRF) to effectively manage their kidney disease and the changes in their lifestyle. 3) Co-ordinate and plan for individual patient care across multiple services and providers to ensure appropriate management of patients on haemodialysis. 4) Provide skilled nursing assessments to assist members of the multi-disciplinary team in implementing and adjusting patient care appropriately. 5) Communicate professionally, effectively and appropriately with patients, their family/whanau, colleagues, referrers and other people who interact with the service. 6) Participate in self, peer and unit reviews to continually improve nursing practice, service provision, patient outcomes and quality initiatives. 2 PERSON SPECIFICATION Education & Qualifications General & Specific Requirements: Essential A staff nurse will be able to demonstrate in their practice the following requirements for their level: Registered nurse Minimum 5 years post graduate experience with 3 years minimum experience in haemodialysis nursing. (preferably a level 3 or 4 nurse) Willing to work towards or already completed core topics or equivalent qualification in: Applied science for RN s Speciality knowledge and practice (Renal Nursing) Advanced assessment and clinical reasoning Clinical teaching Desirable Experience in Case Management CRITICAL COMPETENCIES: Competency Description 1. Monitors progress and addresses problems to achieve outcomes. Manages workload/flow, and takes account of changing priorities. 2. Treats them as first priority and improves service. 3. Bicultural Approach Understands the significance of the Treaty of Waitangi. Displays cultural sensitivity/appropriateness and a willingness to work positively with organisational strategies to improve opportunities and outcomes for Maori. 4. Self Management Sets high standards and strives to achieve stretching goals. Displays drive and energy and persists in overcoming obstacles. Copes with stress, is resilient to change and understands personal limitations. Is proactive and displays initiative. 5. Communication/ Actively listens, drawing out information and checking Interpersonal Skills. understanding. Expresses information effectively, both orally and in writing, adjusts language and style to the recipients and considers their frame of reference. Basic Computing Ability to use a personal computer with standard software applications for basic operations such as patient scheduling (PHS), file management, word processing, e- mail, diary management. Relationships: External Internal Committees/Groups Patients and family/whanau General Practitioners Government Agencies Other Healthcare providers Multidisciplinary Renal Team - including ward 71, Haemodialysis, PD,renal transplantation etc. Other multidisciplinary clinical teams both in ADHB & other DHBs particularly Waitemata DHB Auckland District Kidney Society (ADKS) National Kidney Foundation (NKF) Renal Society of Australasia (RSA) Job specifications The Nurse case manager would be responsible for approximately 50 clients in addition to the following: Supporting the team Participating in team education Facilitating medical rounds Monitoring documentation Monitoring adherence to standards of practice Referrals and follow through Performing and teaching physical assessment ANZDATA collection Quality group participation Anaemia, nutrition, bone disease, diabetes, transplant status and vaccination status management Education and training of team members (as required) Target weight management Patient education To work with the team care co-ordinator technician Registered Nurse December 2007 Page 3 of 5
4 Nurse case manager characteristics would be: Able to effectively supervise and delegate Strong interpersonal and communication skills Able to give feedback Familiarity with HDU policies and procedures Preferably full time Be forward thinking Level 3 Ideally undertaking post graduate studies Ability to appreciate diversity Able to coach, facilitate and nurture staff Collaborative in care planning Willingness to develop role Case manager performance development Case Manager performance will be measured against the job specifications and a performance appraisal document will be developed to support this.
