2 A MESSAGE FROM THE CEO & BOARD CHAIR With Gratitude to Our Community and Friends For more than 8,402 seniors in the Bay Area, On Lok is family. Whether they receive all-inclusive healthcare and day services through our Program of All-inclusive Care for the Elderly (PACE) or keep mentally and physically active at the 30th Street Senior Center wherever our seniors experience On Lok, they describe feeling cared for, safe and socially connected. This feeling of belonging is fostered every day by employees who are truly dedicated to our mission, as well as volunteers and supporters who donate their time, talents and treasure to enrich the lives of our seniors through art, music, learning opportunities and companionship. Thanks to their contributions, we are able to offer an exceptional array of activities and resources to enable older adults to lead full, independent lives the vibrant lives we all hope and desire for ourselves in our golden years. Additional ways we served our communities include the following: Our interdisciplinary teams delivered high-value healthcare. In 2017, On Lok s hospital readmission rates were 40% lower than Medicare s for the second consecutive year. Our community dining rooms served 323,239 nutritious meals and another 277,119 meals were delivered to over 350 home-bound seniors through our 30th Street Senior Center nutrition and PACE programs. On Lok drivers traveled over 682,000 miles to transport our seniors to 27,768 appointments to receive medical and other social services. More than 900 active older adults attended fitness and fall prevention classes at 16 community centers through the Always Active program. For many participants, these were the main source of social interaction. On Lok Housing provided safe and accessible housing options for low-income seniors in an increasingly difficult Bay Area housing market. Building on these results, we are excited to start 2018 with clear strategic goals to propel our mission forward. As the originators of PACE, we continually strive to set the gold standard in care that empowers seniors and their families to maximize independence and community living. We are relentlessly pursuing opportunities to expand PACE to an ever increasing aging population, while advocating for the protection of the program on behalf of current and future generations of seniors. Building on both our expertise of active, healthy older adults at the 30th Street Senior Center and our leadership in full-time care services through PACE, we are designing new, innovative programs to help more individuals and their families across the continuum of care, especially those who may be only one health crisis away from financial and familial hardship. We are also bridging technology with service to deliver safer and more efficient patient-centered care. At the same time, we are investing in our workforce with a renewed commitment to professional development and work-life balance that distinguishes On Lok as a best place to work. With our minds and hearts in the right place, and your generous support, we are setting ever higher standards in high-quality care for seniors and persons with disabilities. 4 PROGRAM OF ALL-INCLUSIVE CARE FOR THE ELDERLY On Lok s Unique Model of Integrated Care Services On Lok s Program of All-inclusive Care for the Elderly (PACE) is widely recognized as an outstanding model of integrated geriatric care and support services. Today there are 122 PACE programs in 31 states. Day health program Including nursing, personal care, social services, and social activities. Primary medical care Home care Medical specialists Medication Acute hospital care Nursing home care Dental Optometry Audiology Podiatry PACE provides comprehensive and coordinated clinical and social services, enabling seniors to live with dignity in their home and community instead of a nursing home. X-Ray Ambulance Lab Rehabilitative therapies PACE participants are in the care of a dedicated team of physicians, nurses, social workers, physical and occupational therapists, dietitians, transportation staff, recreational activity leaders, behavioral and mental health specialists, home care workers, and specialists in dentistry, optometry, audiology and podiatry. Transportation Personal Coordinated Care Team Home-delivered & congregate meals TIMELINE 1971 In San Francisco, Dr. William L. Gee and social worker Marie-Louise Ansak create On Lok, a community-based model of healthcare for underserved seniors. On Lok means peaceful, happy home in Cantonese On Lok launches one of the country s first senior day health centers, serving hot meals and providing health and social services to seniors living at home On Lok adds in-home support services, then primary care services and case management of acute and chronic health services On Lok launches a Medicarefunded model of long-term care, wherein an interdisciplinary team provides seniors with coordinated care plans and comprehensive medical care and social services.

Jackson Avenue, San Jose 30th Street Senior Center 30th Street Senior Center is the largest multi-purpose senior center in San Francisco, connecting 6,776 active older adults with each other and with culturally sensitive programs for self-development and wellness. Its services include health and fitness programs, nutritious homedelivered and congregate meals, educational activities and case management services. 30TH STREET SENIOR CENTER th Street, San Francisco On Lok Senior Housing On Lok s housing program was created to provide affordable and safe housing options for seniors in San Francisco. On Lok provides senior housing in three buildings: ON LOK HOUSE 1441 Powell Street, San Francisco 1000 MONTGOMERY HOUSE 1000 Montgomery Street, San Francisco DR. WILLIAM L. GEE HOUSE 1333 Bush Street, San Francisco PACEpartners offers customized consulting and technical assistance for all PACE programs across the world. Care at Home is a Medicarecertified, state-licensed home health agency focused on patientcentered care. Since 1997, Care at Home has been providing medical home healthcare and non-medical care services to infants, children, adolescents and adults living in the Bay Area On Lok obtains waivers from Medicare and Medicaid to test a new financing method for long-term care On Lok launches the Program of All-inclusive Care for the Elderly (PACE), a groundbreaking model of coordinated service delivery and innovative financing On Lok opens the newly renovated Dr. William L. Gee Center in San Francisco. The National PACE Association, a nonprofit membership organization comprised of PACE sites, is formed th Street Senior Center becomes a part of On Lok s family of services On Lok s PACE program opens two new centers in San Francisco one in partnership with the Institute on Aging and the other at the 30th Street location.

The Center is located within Eden s 98-unit, low-income senior housing project.

7 30th Street Senior Center In FY , 30th Street Senior Center served over 6,776 independent older adults. Average age: 78 63% of participants are low income. 40% live alone. The Center offered 50 activities a week and 8 programs, including Nutrition, Home-delivered Meals, Always Active, Healthier Living, Diabetes Education Empowerment Program (DEEP), the Aging and Disability Resource Center (ADRC), Bilingual Case Management, and Activities. The Center served 83,931 congregate meals and delivered 71,531 meals to over 350 home-bound seniors. The Always Active program provided fitness and fall prevention classes to more than 900 older adults at 16 community centers in the greater San Francisco area. The Powell PACE Center and 30th Street Senior Center in San Francisco undergo a complete renovation. The East San Jose Adult Day Center is also renovated to make it a fully operational PACE Center.
