3 June 30, 2017 To my fellow Texans and community neighbors, Baylor Scott & White Medical Center Brenham is honored to serve Washington County and surrounding communities operating within an integrated healthcare system driven by the Baylor Scott & White Health System s mission to serve all people providing personalized health and wellness through exemplary care, education and research as a Christian ministry of healing. Baylor Scott & White Health, the organization formed from the 2013 merger between Baylor Health Care System and Scott & White Healthcare, is today the largest not-for-profit health care system in the state of Texas. The organization now includes 48 hospitals, more than 1,000 patient care sites, 5,500 affiliated physicians, and 44,000 employees, as well as the Scott & White Health Plan, Baylor Scott & White Research Institute and Baylor Scott & White Quality Alliance-- a network of clinical providers and facilities focused on improving quality, managing the health of patient populations, and reducing the overall cost of care. Our Board of Directors, physicians, nurses and staff take our responsibility to the community very seriously. We are dedicated to providing high-quality healthcare to each person in need, regardless of their ability to pay and our number one aspiration is the pursuit of quality and patient satisfaction. Our dedication goes beyond the walls of our hospitals and clinics as we serve our community to promote the health and wellbeing of individuals and families and improve the overall health of the population. Our community benefit efforts are centered on improving access to healthcare and empowering citizens to make healthy life choices. Enclosed is our Community Benefit Report for 2017, summarizing our community benefit activities, ranging from immunizations and health screenings to support groups and free health education classes throughout Washington county and surrounding communities. We continue to place particular emphasis on supplying resources for the underserved and uninsured residents in the community. As part of the report, you ll see that conducts regular community health needs assessment and for this reporting year, community benefit programs have focused on the following areas of need: addressing the causes of obesity, and chronic disease management and prevention with a particular focus on cancer. These priority needs have been incorporated into overall hospital strategies to better manage the health of the community and which will shift as new needs are identified in coming years. Mission Statement, an affiliated hospital of Baylor Scott & White Health System (BSWHS), is committed to fulfilling its mission: we exist to serve all people providing personalized health and wellness through exemplary care, education and research as a Christian ministry of healing. II. III. IV. Effective Dates of the Plan The annual report of community benefits provided is for the fiscal year ended June 30, 2017 ( Fiscal Year 2017 ). Description of the Community Served serves children and families across the Brazos Valley area with the majority of its patient population residing in Washington County. The hospital identifies as its community served to include the counties of Washington, Burleson, Waller, Grimes, Brazos, and Austin. Programs Addressing Identified Community Health Needs Programs Addressing Chronic Disease Management (including Cancer): Chronic Disease Self-Management Program Description: Baylor Scott & White Health consistently looks for opportunities to educate the community on health issues to support our mission and vision. This program benefits people with different chronic health problems. Subjects covered include: 1) techniques to deal with problems such as frustration, fatigue, pain and isolation, 2) appropriate exercise for maintaining and improving strength, flexibility, and endurance, 3) appropriate use of medications, 4) communicating effectively with family, friends, and health professionals, 5) nutrition, 6) decision making, and, 7) how to evaluate new treatments. 5 Description: The hospital provides a congestive heart failure (CHF) inpatient program for patients after their primary diagnosis that aligns with the American Heart Association model. Diabetes Education Description: BSWH provides diabetes education seminars and presentations open to the public or for a specific group in need to educate the community about the signs and symptoms of diabetes and how to prevent diabetes from happening. Financial Donations for Health Improvement Description: BSWH often donates funds to local charitable, not for profit organizations whose efforts to improve community health align with the identified health needs in our community. We do so to support their efforts to improve the overall health of the community through education, prevention, health advocacy, or disease management education. Funds that go to improving the health infrastructure of our community are counted after subtracting the fair market value of participation by employees or the organization. Better Breathers Club Description: Better Breathers Club is a support group offered in partnership with the American Lung Association for individuals with COPD, pulmonary fibrosis and lung cancer, and their caregivers. Led by a trained facilitator, these in-person adult participants learn better ways to cope with lung disease while getting the support of others in similar situations. Better Breathers Clubs meet regularly and feature educational presentations on a wide range of relevant topics, including: o How COPD affects the lungs o Breathing techniques o Exercise o Talking with your physician o Medications and other treatment options o Medical tests o Supplemental oxygen o Home healthcare o Lung transplants o Air pollution Fit & Strong Description: Fit & Strong is an evidence-based, multiple component exercise program. It combines flexibility, strength training and aerobic walking with health education for sustained behavior change among older adults with lower extremity osteoarthritis (OA). This eight-week program helps participants to improve: o Lower extremity stiffness o Lower extremity pain o Lower extremity strength o Aerobic capacity o Participation in exercise and caloric expenditure o Self-Efficacy for Exercise Page 4

6 Mammography Screenings Description: Free breast cancer screening by mobile mammography for eligible indigent women between ages 35 to 64. BSWH staff contacts, prescreens, and completes financial apps for all the patients prior to the event dates. We have a small staff so the preparation process for these mammogram events is worked on over months. Clinical assessment and follow up was provided as appropriate. Seniorcise Description: This ongoing educational class for seniors includes stretching and strengthening in standing and sitting positions to improve flexibility, cardiovascular, balance, and strength. Washington County Community Clinic Description: BS&W Brenham collaborates with Washington County and Faith Mission of Brenham to improve access and provide services to the low-income and medically underserved residents of Washington County. This is done through in kind support of resources (supplies, staff, labs, etc.) to the community clinic throughout the year. Wellness Kids Day Description: Annual wellness event held for kids before the start of the school year. Health information, screenings, vaccines and more are provided free to all participants. Wellness Lunch n Learns - Community Based Description: Regular lunch & learn events are held at the hospital and are open to the public. Each session includes educational information about preventive health care, lectures, or presentations held by BSWH physicians and staff about health related topics like understanding various conditions and diseases, when to seek treatment, and the treatment options available. Community Based Wellness Programs Description: Baylor Scott & White Health offers free and/or reduced cost services and programs geared towards enhancing the well-being of individuals in the community. Texercise Description: A statewide health promotions program of the Texas Department of Aging and Disability Services (DADS) that encourages individuals and communities to adopt healthy lifestyle habits such as regular physical activities and good nutrition. Description: WELLNESS PROGRAMS: Fit & Strong Fit & Strong is an evidence-based, multiple component exercise program. Description: COMMUNITY HEALTH EDUCATION-School Based BSWH recognizes the importance of teaching about health and health professions to students in our school systems. Programs and services in this category are geared towards students K-12. VI. VII. VIII. Charity Care and Government-Sponsored Indigent Health Care Provided For Fiscal Year 2017, provided $ 2,944,068 in unreimbursed costs of charity care and government-sponsored indigent health care. Government-Sponsored Health Care Provided For Fiscal Year 2017, provided $ 2,444,458 unreimbursed costs of government-sponsored health care. Other Types of Community Benefits Provided is committed to improving the quality of life for the many citizens living and working in its area and was pleased to allocate a total of $313,602 to the following community benefit activities. A. Community Health Improvement Services $ 66,600 These are costs associated with programs and services that extend beyond patient care activities and include services directed to both individuals and larger populations in the community. These services include such things as educational information about preventive health care and treatment for specific diseases, informational health lectures to help understand various conditions and diseases as well as available treatment options, opportunities that promote healthy activity and wellness initiatives, health fairs and screenings, and support groups. B. Financial and In Kind Donations $ 219,207 Baylor Scott & White provides community benefit grants and support to other non-profit organizations through dedicated community benefit funds as well as provides donation of staff time, medical supplies or equipment to other non-profit organizations. Other Community Benefit $ 27,795 Baylor Scott & White provides other services with the goal of improving the overall health of the community including ongoing assessment of community health needs and evaluation of community health programs and services. IX. Total Operating Expenses and Calculation of the Ratio of Cost to Charge As required by Section (a) (4), reports $31,459,296 in total operating expenses for the prior audited fiscal year ending June 30, As required by Section (1) (5), the ratio of cost to charges was 26.63%. Please see the attached worksheet for the full calculation. X. Report of Community Benefit Provided During Fiscal Year 2017 In a commitment to fulfill its mission, benefit to the community, conservatively estimated, was $5,702,128 for Fiscal Year Baylor Scott & White Medical Center - Brenham is filing its Annual Statement of Community Benefits Standard (Statement) as a consolidated system with the other affiliated hospitals of BSWHS (excluding those that qualify as Medicaid disproportionate share hospitals). Through community benefit activities, BSWHS - affiliated hospitals provided: quality patient care and subsidized services otherwise not available in the community; medical education, training for medical technicians, hospital chaplains, nurses, and future physicians; and medical research that will speed the time between scientific finding and its application to improving medical care.
