It s easy to use My Health to get to myuhc.com, your UnitedHealthcare member website. From there, you can quickly check claim statuses, find doctors and get answers. Covered spouses/domestic partners can also access My Health at myhealth.jpmorganchase.com. Need to register? 1 Go to My Health. For employees at work: My Health via me@jpmc From the internet: myhealth.jpmorganchase.com 2 Go to My Medical Plan Website and click Register Now. Create a HealthSafe ID, a new and improved registration process that makes it easier to move across all UnitedHealthcare websites using the same login: Identify yourself. Enter your name, birthdate, ZIP code, member ID and Group/Account number. 3 Create a username and password. Set up account recovery preferences. Agree to Terms of Use, the Privacy Policy and the Consumer Communications Notice. Confirm your contact information. Registration confirmation. Go green! Choose online delivery for documents such as health statements. It s faster, and a more efficient delivery method for you. Go to myuhc.com through My Health > My Medical Plan Website and select Account Settings, then Mailing Preferences. 4 Note: If you are an employee and you access myuhc.com through My Health, in the future you will not need to enter your username and password, as you ll be logging in via Single Sign-On. It isn t always easy to find a doctor who is right for you. The UnitedHealth Premium designation program provides simple tools and useful information to help you make more confident decisions. The program uses claims data to evaluate doctors and see if they meet national and local standards for quality and cost efficiency. Find a network doctor or hospital. It s easy, thanks to myuhc.com. 1 Log 2 Click 3 Click on to myuhc.com through My Health > Find a Doctor (under My Medical Care). on the Physician & Facilities tab or the Find a Doctor button. the Find a Provider button to search for providers in your network. 4 Search for a doctor by People, Places, Tests and Imaging, Services and Treatment, and Care by Condition. Look for doctors who are highly rated for quality and cost by the Premium designation program. Choose smart. Look for the blue hearts. 5 Have a doctor come to you using a smartphone, phone, tablet or computer for $5 or less with Virtual Doctor Visits. See page 9 for details. Did you know the doctor or facility you choose may save you money? UnitedHealthcare s network is the largest in the country. If you seek care from a health care provider in the network, you pay significantly less than if you receive care from non-network providers. When you need health care from a doctor or hospital, knowing how your plan works can prevent surprises and save money. 1 2 Choose any doctor or health care professional in our network. Your Medical Plan pays a larger percentage of medical expenses when you see a network health care provider. Note: You can see any doctor you d like without a referral; you do not need to designate a primary care doctor. Visit myuhc.com through My Health > Find a Doctor for our network directory. Doctors who are rated by the Premium designation program for quality and cost help ensure you receive the right care at the right time saving you time and money. If you re sick, consider Virtual Doctor Visits. To register, go to me@jpmc > My Health > My Medical Plan Website. Select Virtual Visits and choose one of our partners: Amwell or Doctor on Demand. Or, go directly to myuhc.com. Know what your Medical Plan covers before your visit. Review your plan materials to learn what your plan covers and what it doesn t. (You can access the materials on My Health.) Your plan does not have traditional copayments for doctor visits. Remember, eligible preventive care is covered 100 percent when you see a network provider, and you pay 10 percent of the cost of network primary care visits (e.g., internist, family practitioner, pediatrician, OB/GYN, if listed as a primary care physician in our provider directory, or convenience clinics) with no deductible. 3 Use the cost estimator before receiving health care services. Imagine shopping for health care the way you would for a new phone comparing options and service providers, estimating your prices and deciding where to buy. All of this vital information is available to you with the cost estimator on myuhc.com through My Health > Estimate Medical Plan Costs. ` Learn about your procedure and compare treatment options. ` Select the right quality provider for you. ` Easily estimate out-of-pocket costs for your specific procedure. ` Choose your provider location based on your needs and even get directions. Virtual Doctor Visits. Let the doctor come to you with a Virtual Doctor Visit from your phone, tablet or computer. 9 Your Medical Plan comes with an account called a Medical Reimbursement Account, or MRA. The MRA is funded by JPMorgan Chase when you take action and complete designated wellness activities, and will help pay your deductible, coinsurance, and other eligible medical and pharmacy out-of-pocket expenses. Your Medical Reimbursement Account (MRA). Whether you are enrolled in Option 1 or Option 2, your Medical Plan includes an account called an MRA to help pay your share of eligible medical and pharmacy expenses. 1 2 How the plan works. Your Medical Plan pays 100 percent of eligible network preventive care and 90 percent for network primary care doctor office visits and Virtual Doctor Visits, without having to pay a deductible. A deductible is the amount you must pay for other types of services before the plan begins paying. After you pay the deductible, the Medical Plan pays a large percentage of expenses, and you pay the remaining amount. This is called coinsurance. Your MRA will help pay your medical deductible and coinsurance, plus eligible prescription drug out-of-pocket expenses. The money in the MRA may be enough to cover all your costs for the year. This means you would pay nothing out of pocket when receiving care. And if a serious medical event happens, you can rest easy. Once the deductible is paid and your coinsurance payments add up to the plan s coinsurance maximum, the plan pays 100 percent of all eligible expenses for the rest of the year. Network preventive care is covered 100 percent and does not affect your MRA. MRA funds can carry over: If you have dollars remaining at the end of the year, they will carry over so you can use them next year, provided you are enrolled in the Medical Plan next year. 3 4 If you spend all of your MRA funds, use your Health Care Spending Account (HCSA). Your HCSA is funded by you and can pay eligible medical and pharmacy expenses after your MRA funds are gone. Vision and dental expenses can also be paid using your HCSA (but not your MRA). MRA and HCSA claims can be paid through an automatic claims payment process or a Health Care Account Card (debit card), depending on the payment method you selected at enrollment and the type of expense. If you have the Health Care Account Card, you can choose to use another form of payment (such as a personal credit card or checking account) and then be reimbursed by submitting a claim form, which is available on myuhc.com through My Health > My Medical Plan Website. You can carry over up to $500 in unused HCSA funds for use in the following plan year. If you did not elect an HCSA or there are no funds available, you will need to pay your share of expenses out of your pocket. Want to make your MRA dollars go further? Doctors and hospitals may charge different rates for the same services depending on where you go. So your choice makes a difference. Go to My Health > Estimate Medical Plan Costs. 8

10 Using Your Benefits. Learn more about the services available as part of your Medical Plan benefits and your options for care day or night. Your check-ups. Take advantage of your eligible preventive care benefits. Preventive care helps keep you healthy. It can include physical exams, screenings and immunizations. Eligible preventive care is covered 100 percent when you see network doctors. It s important that when your doctor submits a preventive care visit to us for payment, they appropriately code the claim as preventive so your medical plan covers it 100 percent. When you and your covered spouse/domestic partner participate in certain preventive care activities, you may earn funding for your MRA. Your Virtual Visits. Get reliable answers anytime. Let the doctor come to you when you connect to a doctor in minutes anytime, from just about anywhere using a smartphone, phone, tablet or computer for $5 or less per Virtual Visit. Doctors can make diagnoses, provide guidance and call in prescriptions to your local pharmacy. For details, go to me@jpmc > My Health > Virtual Doctor Visits. You can also go directly to myuhc.com to start a Virtual Visit. Your 24-hour nurseline. Get reliable answers anytime. You can call anytime, day or night, and speak with a registered nurse. For example, you may call mynurseline if your child wakes up at 3 a.m. with a high fever and you re not sure where to seek help. The nurse can provide assistance or help you select a new doctor, facilitate making an appointment and help you coordinate the sending of your medical records. You can also call to: Research treatment costs in your area. Get resources to manage an illness or injury. ` ` Locate network doctors and hospitals in your area that meet criteria for quality and efficiency of care. ` ` Understand medication interactions. Your health coaches. Better health is just a call or click away. Our telephonic coaching and online Missions can help you with wellness goals and dealing with the stresses of daily life. And remember, when you use our health coaching services, you and your covered spouse/ domestic partner may earn funding for your MRA. You can get help to: Exercise more. Eat healthier. A resource for expecting mothers. Maternity Support supports expectant mothers throughout their pregnancy. To enroll, call between 8 a.m. and 8 p.m. CT, Monday through Friday, or visit the Healthy Pregnancy program website at myuhc.com through My Health > My Medical Plan Website for more information. You or your covered spouse/domestic partner must enroll by the first trimester (unless you become covered under the plan at a later stage of pregnancy) and complete program requirements to be eligible to receive MRA funds. Type of medical care Why would I use this care center? What type of care would they provide? 1 What are the cost and time considerations? 2 Virtual Visits When you have a non-emergency health need but don t want to drive to your doctor s office or sit in a crowded waiting room with a sick child, Virtual Visits allow you to talk with a doctor anytime, anywhere using your smartphone, tablet or computer. Allergies. Bronchitis. Coughs. Diarrhea. Fever. Pinkeye. No appointment necessary; available 24/7. Virtual Visits are $5 or less. The doctor can send some prescriptions to your local pharmacy. Great to use while traveling. Doctor s Office You need routine care or treatment for a current health issue. Your primary care doctor knows you and your health history, can access your medical records and refer you to a specialist, if necessary. Routine checkups.* Immunizations.* Preventive services.* Manage your general health. * Eligible expenses are fully covered without coinsurance or a deductible. Primary care doctor office visits require coinsurance but no deductible if in network. Normally requires an appointment. Little wait time with scheduled appointment. Convenience Care Clinic You can t get to your doctor s office, but your condition is not urgent or an emergency. Convenience care clinics are often located in malls or retail stores offering services for minor health conditions. Staffed by nurse practitioners and physician assistants. Common infections (e.g., strep throat). Minor skin conditions (e.g., poison ivy). Flu shots. Pregnancy tests. Minor cuts. Earaches. Requires coinsurance but no deductible if in network, similar to primary care office visit. Walk-in patients welcome with no appointments necessary, but wait times can vary. Urgent Care Center You need care quickly, but it is not an emergency, and your primary care doctor may not be available. Urgent care centers offer treatment for non-lifethreatening injuries or illnesses. Staffed by qualified physicians. Sprains. Strains. Minor broken bones (e.g., finger). Minor infections. Minor burns. Requires coinsurance, usually higher than for an office visit. Subject to a deductible. Walk-in patients welcome, but waiting periods may be longer as patients with more urgent needs will be treated first. Emergency Room (ER) The ER is for the treatment of life threatening or very serious conditions that require immediate medical attention. Do not ignore an emergency. If a situation seems life threatening, take action. Call 911 or your local emergency number right away. Heavy bleeding. Large open wounds. Sudden change in vision. Chest pain. Sudden weakness or trouble talking. Major burns Spinal injuries. Severe head injury. Difficulty breathing. Major broken bones. Requires coinsurance, usually higher than for urgent care centers. Subject to a deductible. Open 24/7, but waiting periods may be longer because patients with the most serious lifethreatening emergencies will be treated first. 10

We ll work with you and your doctor to help you understand your care options. We ll also let you know about special programs to help you make health care decisions. Condition management. Get help managing chronic health conditions more effectively. Programs and nurse consultations are available to help you and your family deal with conditions such as: Asthma. `Chronic ` obstructive pulmonary disease. Coronary artery disease. Diabetes. Heart failure. When you use condition management services, you and your covered spouse/domestic partner may earn funding for your MRA. Centers of Excellence. Centers of Excellence are medical facilities with special expertise in bariatric surgery, cancer, congenital heart disease, infertility, neonatal and organ transplants. We can connect you with leading health care facilities and providers who offer information on where you can get care, coordinate your care with your treatment team, schedule appointments and more. When you use a Centers of Excellence (COE) for infertility treatment, the plan will pay $20,000 more than if you were to use a non-coe facility. Your total lifetime benefit using a COE is $30,000. There is a lifetime benefit of $10,000 at a non-coe facility. To receive any benefit, you must contact UnitedHealthcare and work with a coach before receiving infertility treatment. To receive any bariatric benefit, you must contact UnitedHealthcare and work with a coach before having bariatric surgery. Treatment Decision Support. When you re facing a health care decision, it s easy to get overwhelmed. Let us help take the stress and worry off your mind, and give you the answers you re looking for. When you call, you ll speak with a registered nurse who is specially trained to answer the kinds of questions you have about your condition and treatment options: ` ` Learn more about a diagnosis. `Understand ` treatment options and the risks and benefits of each. ` ` Select a treatment facility. ` ` Work more effectively with your doctor. ` ` Know what to expect from surgery. ` ` Find a resource for a second opinion. 13 Managing Your Benefits. Have questions about a claim or how a service was billed? This section helps you understand the process. Remember, you have myuhc.com through My Health > My Medical Plan Website, but if you need more help, give us a call. Your claims. How your medical claim is paid. UnitedHealthcare has negotiated rates with network providers. So your out-of-pocket costs will be lower when you use a network provider. Your doctor s office will bill UnitedHealthcare. Then, we pay your doctor directly when medical services are paid by your Medical Plan. If you have elected automatic claim payment from your MRA and HCSA, we will use the funds in your accounts to pay the amount you owe for medical services. If you have chosen the Health Care Account Card (debit card), your doctor will bill you the amount you owe, and you can use your card to pay your share of costs from your MRA and/or HCSA after the Medical Plan has paid its share. If you went to a non-network provider, they may require partial or full payment at the time of service. Also, some non-network providers may not bill UnitedHealthcare directly, leaving you responsible to submit a medical claim form to UnitedHealthcare. If you submit a claim for a non-network expense, you will be reimbursed directly from your Medical Plan, MRA and/or HCSA (if applicable). Standard claim process for network benefits STEP 1 STEP 2 STEP 3 STEP 4 STEP 5 STEP 6 You present your Medical Plan ID card at a network doctor s office. Provider submits claim to UnitedHealthcare for cost of the visit. The claim is processed according to the Medical Plan, and the provider is paid the amount the Plan owes. The provider is also informed of the amount you owe (your deductible and/or coinsurance, if applicable) and you are sent a health statement from UnitedHealthcare showing your share of the expense. If you have elected an automatic claim payment, UnitedHealthcare will use your MRA and HCSA funds (if any) to pay the provider. The provider will send you a bill if your MRA and HCSA do not cover the amount you owe. If you have elected the debit card, your provider will bill you and you can use the debit card to pay the amount you owe from your MRA and/or HCSA, if applicable. For both automatic claim payment and the debit card, you can confirm the amount paid by checking your UnitedHealthcare health statement. Did you see a non-network provider? If you receive care from a doctor who is a non-network provider, you may need to fill out a medical claim form. You can find claim forms on myuhc.com through My Health > My Medical Plan Website, under Claims and Accounts > Claim Information. Our network doctors normally take care of claim forms for you. Note: You must submit a claim by the end of the year following the one in which the service was received for payment to be made. For HCSA claims, you may submit for reimbursement up until March 31st of the year following the year in which service was received. 12

Health statements are sent to you monthly when you or one of your covered dependents use your Medical Plan and have financial responsibility. You can see all claims processed for that period, your MRA balance, plus remaining amounts for your network and non-network deductibles. If you don t have any claims in a month, you will not receive a health statement. If you have claims but no financial responsibility, you ll receive a health statement quarterly. When you choose to receive your health statement online, you ll receive an whenever a new statement is posted to myuhc.com through My Health > My Medical Plan Website. You can view your current and past health statements or current claims activity at any time at myuhc.com through My Health > My Medical Plan Website. Sample health statement. Member ID. Your unique number that protects your Social Security number. Statement period. Your benefit plan activity during a period of time. Message center. Messages that promote better health awareness.
