3 Purpose of the role : To provide a client/family/whanau centered, safe, responsive, effective and outcome focused social work service to both inpatient and outpatient areas of Child, Women and Family Services. To provide a social work service that is in accordance with the policies of Waitemata District Health Board and the Practice Standards of the New Zealand Social Work Registration Board. KEY TASKS EXPECTED OUTCOMES Receive and process referrals to social work Referrals are received and responded to within 24 hours Make accurate and timely assessments of the psycho-social issues, safety and risks impacting on client (parent/caregiver, child and family/whanau) Assessments are accurate, meaningful and timely in accordance with priority guideline timeframes. Assessments are clearly documented in writing in Concerto and/or All social work practice is ethical and of a high quality standard. Demonstrate effective clinical reasoning by ensuring that care is planned and interventions provided to meet needs identified in the social work assessment and/or risk assessment. Demonstrate a contribution to the continuum of care by working in partnership with clients, key stakeholders and other services both within Waitemata DHB and the wider community. Demonstrate ability to work effectively within a clinical team by developing and maintaining effective collegial relationships with other client file The social worker is registered with the New Zealand Social Work Registration Board and adheres to the ethics and standards of the Board Provides unbiased care to women seeking termination of pregnancy Safety and risk factors prioritised, and addressed in a responsive, timely and professional manner Written care plans demonstrate care objectives and social work actions to address issues of assessment, and risk Clinical documentation is clear, accurate and in accordance with Waitemata DHB policies Demonstrates an ability to work across relevant child, women and family service inpatient and community settings to ensure seamless service Referral to other providers occurs to ensure a safe and effective continuum of care Works as part of the on-call roster for social work to ensure continuous services on weekends and public holidays Relationships are established and maintained across all relevant areas of child, women and family services Waitemata District Health Board - Social Worker Approved HOD Allied Health Page 3

Participate fully as a member of the social work and multidisciplinary teams and contribute to the ongoing development of social work services. Demonstrate in practice the principles of the Treaty of Waitangi Partnership, Participation and Protection. To also demonstrate culturally safe practice that encompasses a diverse range of beliefs, values and practices. Maintain an up to date knowledge of networks, services and other resources available and beneficial to clients and/or their families/whanau. Meet the clinical competencies specific to the service needs, position and profession. Maintain such clinical and statistical records of social work interventions and outcomes, according to Waitemata DHB organisational and SWRB Core Competence Standards and requirements for Annual Practicing Certificate. Undertake continued professional development and supervision. EXPECTED OUTCOMES Social work contributes openly and constructively with other clinicians in the care of clients/clients/family/whanau An inter-professional approach is evident in care planning Support is provided to team members who experience traumatic events while providing care e.g. still birth, death of a child Responsibilities to the Child Women & Family social work team and to the broader multidisciplinary team are fulfilled Contributes to clinical, management and other meetings relevant to social work service delivery Leadership is demonstrated appropriate to expected level of practice Social work activities are in line with service Quality Plans and/or Strategic plans Social work activities are consistent with the service vision for social work Maori and others receive and /or have access to services appropriate to their culture, in accordance with Waitemata DHB policies and SWRB Core Competence Standards Knowledge is current and appropriate to the needs of clients and/or their families/whanau Knowledge is maintained across a broad spectrum of networks including disability sector, well child providers, schools, family violence prevention agencies and other community NGO s. 5 KEY TASKS Participate in the supervision of social work students on placement as required EXPECTED OUTCOMES SWRB requirements for Annual Practicing Certificate Self motivation is evident in relation to personal and professional development There is active participation and achievement of objectives identified as part of your annual performance development review Social work students receive safe and comprehensive learning while on placement Client safety is not compromised during student placement Workload is maintained during student placement Statutory & Treaty of Waitangi obligations Ensures the professional and political integrity of WDHB by carrying out all functions in compliance of the Treaty of Waitangi and by demonstrating a serious commitment to keeping the treaty alive. 6 PERSON SPECIFICATION POSITION TITLE: Social Worker Qualification Experience Skills/Knowledge/Behaviour Minimum Bachelors Degree in Social Work Registration with the Social Workers Registration Board (SWRB) Current Annual Practicing Certificate issued by the SWRB Social work experience with children, youth and families Experience of working with Maori clients, families and other cultural groups Experience of working in a multidisciplinary team environment Experience in the area of disability and/or long term conditions Counseling skills, particularly in the area of grief and loss and adjustment to health diagnosis and change Skilled in solution-focused therapy and demonstrated ability to work from a strengths based practice approach Knowledge of District Health Board systems and the health environment Preferred At least 2 years social work experience Member of Aotearoa New Zealand Association of Social Workers Qualification in Professional Supervision Experience of working with a broad range of providers across the continuum of care, for example, Iwi Organisations, Community Support Agencies and Primary Care Providers, Oranga Tamariki, etc. Experience in working with grief and loss Experieince in the area of disability and/or long term conditions Knowledge of Child Protection legislation, systems and processes Knowledge of Child Health issues, particularly in the areas of child development and/or preventable diseases Knowledge of the family centred care approach, attachment and family systems theories Waitemata District Health Board - Social Worker Approved HOD Allied Health Page 6
