4 Post-Acute Care Perception Versus Reality Perception This is where older patients go to live when they are at the end of life When you work in a nursing home you lose your nursing skills When you work in a nursing home you only pass out medicines
5 Post-Acute Care Perception Versus Reality Reality Most centers previously known as nursing homes are now known as post acute service providers discharging 25-30% of their patients back to the community. Post Acute Care nurses are caring for clinically complex patients LVADs & Life Vests Pleurex catheters IVs, PICCs, Central Lines Post Acute providers are actively engaged in bundled payment care initiatives and integrated care paths with acute providers to promote early discharge Most post acute care providers offer opportunities for career advancement. Often, you can choose your specialty or career path and the organization will assist you in training and education needs to help you reach your professional goals!
