AccessHC is a not for profit organisation with caring at its centre, and employs over 280 staff. The Access HC mission is to build healthier lives together with our communities and deliver excellent health services for all. Access HC is a new organisation but one with a continuous historic pedigree from its beginnings in It has four major medical clinics and eight other sites. Access HC is bedding down a series of recent mergers and other changes which have equipped it to
2 look optimistically towards the future. It has the ambition to become an excellent primary health service for all people, whilst ensuring that disadvantaged people have equitable access to all services. Its funding comes from Fee for Service activities, as well as from the Federal and State Governments. Access HC has an important strategic relationship with Swinburne University to advance innovation in health care. This role reports to the anager of Podiatry, Dietetics and Diabetes Education. The purpose of this position is to provide clinical leadership in both the Podiatry team and the Allied Health Division, to ensure high quality, evidence-based and efficient services are delivered to the clients of Access HC. Key responsibilities are divided into 4 main areas; To provide podiatry services to a high level of expertise and experience, and to act in an advisory capacity in relation to podiatry clinical issues, within Access HC. The position will have a clinical load and be responsible for providing podiatry services to clients, especially clients who have complex clinical needs. The Senior Clinician will provide services across a range of funding sources, including CHSP, HACC, CH, Fee-For-Service, edicare and NDIS. To provide discipline-specific clinical leadership in the Podiatry Team at Access HC, to ensure the development and maintenance of professional clinical standards. This includes provision of clinical supervision to podiatrists, Allied Health Assistants and students. To provide clinical leadership in a specified area within the Allied Health Division (e.g. Diabetes & Wound Care) and to lead integrated (multidisciplinary) clinical practice across the allied health disciplines. To assist the anager in leading service development and quality improvement activities at Access HC. Position Functions Key areas of accountability Client assessment and care (65%) Provide holistic assessment of clients which addresses their physical, emotional and socio-economic needs. In consultation with the client and /or carer develop shared health goals and document care plans. Provide podiatry services for a wide range of clients, including people with complex disabilities, acute and chronic conditions, frail older clients, clients from CALD backgrounds, and clients with special needs. Provide timely feedback / handover to other service providers regarding client assessment, goals and treatment. Identify where clients require additional support and health care services, and make appropriate referrals to other service providers.
3 Work within multidisciplinary teams, to ensure integrated service delivery. Act as a go-to person for the Podiatry team and the Allied Health Division, around working with clients with complex needs. To meet targets and KPIs as set by anager, Podiatry, Dietetics and Diabetes Education. Record keeping and reporting Create and maintain client files and records of assessment and treatment in accordance with organisational procedures Record all relevant statistical information and other data as required by AccessHC including billing procedures. aintain security of client files and data in accordance with AccessHC procedures Clinical leadership/service Development (35%) To assist the anager, Podiatry, Dietetics and Diabetes Education to develop the annual Podiatry Team Work Plan and Professional Development Plan and Individual Work Plans (in line with the Access HC Strategic Plan.) Under direction of anager, to provide clinical supervision to Access HC podiatrists, Allied Health Assistants and students. To assist the anager in ensuring the Podiatry team adheres to professional clinical standards, scope of practice guidelines, key performance indicators, work instructions and Access HC s Policies and Procedures. To provide expertise in the Allied Health Division around podiatry services in general, and an area of specialisation (e.g. Diabetes and Wound management) To assist the anager, Podiatry, Dietetics and Diabetes Education to lead service development and quality improvement activities, both within the Podiatry team and Allied Health Division (e.g. development of IDEAS clinics at Access HC; liaison with wound clinics at local hospitals; development of the roles of Allied Health Assistants in the Podiatry team; and development of the following funding sources across all levels of podiatry service at Access HC - edicare, Fee for Service and NDIS.) Lead integrated (multidisciplinary) clinical practice across the allied health disciplines.
