1 Patient and Family Engagement A Roadmap to Success Thomas Workman, PhD Project Director, PFE Contractor in Support of Partnership for Patients 2.0 Goals for the session Define patient and family engagement (PFE) from CMS perspective Understand the core principles of PFE Understand CMS vision for PFE in the Partnership for Patients 2.0 and beyond Operationalizing the five PFE Metrics Introducing the Roadmap s 6 strategies Understanding how to use the Roadmap to implement PFE activities Definition of PFE for the Partnership for Patients 2.0 Persons, families, their representatives, and health professionals (clinicians, staff, leaders) working in active partnership at various levels direct / point of care; organizational design, policy, and procedure; organizational governance; and community / policy making across the health care system and in collaboration with communities to improve health, heath care and health equity. 3 1

2 Core Principles of PFE PFE happens at multiple levels. Family is defined broadly and by the individual. The goal is moving toward active partnership. PFE requires individual and system behavior change. PFE is about identifying and responding to patient and family identified needs and desired outcomes. 4 The Benefits of Person & Family Engagement Person& family partnership improves outcomes. Person & family partnership improves patient satisfaction and patient experience Person & family partnership creates patient centered systems of care 5 PfP Vision for PFE 2.0 Hospitals and other health care providers achieving quality and safety goals by fully engaging patients and their families, determining what matters most to them in every situation, and partnering with them to make improvements to all aspects of care. 6 2

3 The PfP PFE Metrics Point of care PFE 1: Planning checklist Prior to Admission, hospital staff provides and discusses a planning check list with every patient that has a scheduled admission, allowing for questions or comments from the patient or family PFE 2: Shift change huddles / bedside reporting Hospital conducts shift change huddle and bedside reporting with patients and family members in all feasible cases 7 The PfP PFE Metrics (cont.) Policy & Protocol PFE 3: PFE leader or functional area Hospital has a person or functional area, who may also operate within other roles in the hospital that is dedicated and proactively responsible for PFE and systematically evaluates PFE activities PFE 4: PFAC or representative on quality improvement team Hospital has an active Patient & Family Engagement Committee OR at least one former patient that serves on a patient safety or quality improvement committee or team 8 The PfP PFE Metrics (cont.) Governance PFE 5: Patient and family advisor on board Hospital has at least one or more patient(s) who serve on a Governing and/or leadership board and serves as a patient representative 9 3
