1 POSITION SUMMARY Provides various personal care and related paraprofessional services in accordance with an established plan of care. Provides for the personal needs and comfort of patients. Performs direct patient care for patients in accordance with an established plan of care and agency policy and procedures. Provides care consistent with all relevant professional standards and state and federal requirements. Reports to: ield Service Supervisor / Clinical Services Manager Non exempt Status QUALIICATIONS 1. High School graduate or GED required. 2. Meets minimum age requirement per state law. 3. Successful completion of a Home Health Aide certification course. 4. Ability to contribute as a team member and ability to work under supervision. 5. Has valid driver s license and use of an insured auto or access to adequate transportation. 6. Current irst Aid certification. 7. Read, write, speak and understand English and/or sign language as needed for the job. 8. Knowledge of the regulatory requirements at the state, federal and local level. 9. Absence of criminal background which would prohibit them from working as a state tested nursing assistant. 10. Performs work under the supervision of the Case Manager Nurse or Branch Nursing Director. 11. Is responsible for observation and documentation of changes in the patient s condition and the care or service provided. 12. Performs personal care/homemaking activities contained in a written assignment by the team leader which includes, but is not limited to the provision and/or assistance of the following patient care: PERSONAL HYGIENE 1. Using the bathroom, bedpan or commode. 2. Bathing. 3. Oral care and care of dentures if applicable. 4. Care of skin and hair. 5. Ambulation. 6. Eating. 7. Dressing. 8. Shaving. 9. Prevention of decubitus ulcers. 10. Care of the incontinent patient. 11. Bed making. 12. Takes vital signs including B/P, pulse, respiration and temperature. 13. Care of hearing aides, eyeglasses, prosthetic devices, personal equipment and belongings. 14. Perineal care.
2 PERSONAL HYGIENE continued 15. Care of patient with a urinary catheter (no irrigation). 16. Nail care (no clipping for diabetic patients). HOMEMAKER TASKS Infection Control a) Hand washing b) Disposal of body wastes Meal Preparation/Nutrition a) Special diet, i.e. diabetic, cardiac, renal, low sodium b) Grocery purchase planning & shopping c) Errands d) Drop off and pick up of prescriptions Laundry a) Sorting b) Washing utilization of laundry supplies & usage of machine c) Drying & ironing d) olding & putting away INECTION CONTROL Maintains asepsis in hand washing, linen handling and cleaning of equipment (thermometers, bedpans, urinals) in accordance with infection control policy and OSHA guidelines and utilizes personal protective equipment as indicated. TREATMENTS AND PROCEDURES 1. Performs assigned activities that are taught by an RN or PT for a specific patient. 2. These include but are not limited to: a. Assisting with the change of ostomy appliances (no irrigation). b. Reinforcement of dressings and bandages. c. Assisting with the use of devices for aid to daily living cane, walker, wheelchair or lift device. d. Assisting with range of motion exercises. e. Assisting with self administered urine tests for sugar, acetone or albumin (may not interpret results). f. Measuring input and output. g. Measuring and preparing special diets. h. Reinforces sterile dressings and bandages. i. Changes clean, non-sterile, dry dressings for stable chronic wounds. j. Assists with the change of colostomy bag for stable, chronic stomas. k. Observes, records and reports changes in the patient and reports findings to the case manager. l. Assists with application of arm sling, arm splint or leg splint.
3 THE HHA MAY NOT PERORM THE OLLOWING PROCEDURES: 1. Sterile dressing changes or any task that require a sterile technique. 2. Irrigation of body cavities such as enemas, colostomy or wounds. Except in the administration of a tap water enema as allowed by the state department of health. 3. Administer suppositories. 4. Gastric lavage or gavage. 5. Catheterization. 6. Remove impaction or perform bowel programs using digital stimulation. 7. Care for tracheostomy tube except cleaning around the stoma. 8. Administer oral medications, eye drops or medicated skin ointments. Shall not administer over-the-counter medications to be ingested or over-the-counter eye preparations to be instilled. 9. Administer parenteral or IV medications. 10. Application of heat by any method. 11. Administer NG or gastronomy tube feedings or refill feeding bag with the new feeding contents. 12. Shall not administer food or fluids into feeding tubes. 13. Suctioning of the nose, mouth or tracheotomy. 14. Irrigation fluid to IV s, oley catheters or ostomies. 15. Any services not included in the patient care plan that has been developed by the RN. REHABILITATION 1. Rehabilitative services may be provided by an HHA only after receiving instructions by an RN or therapist and after performing a successful return demonstration including the following: a. Ambulating, range of motion. b. Assists with transferring and alignment including use of foot board, bed cradle, overhead frame, Hoyer lift and special mattresses. c. Assists with the use of wheelchairs, side rails, restraints and other safety devices. PROESSIONALISM 1. Presents self professionally and positively represents the agency. 2. Language, demeanor and all interactions are professional. 3. Is professional in appearance clothing, grooming (hair, nails etc.). 4. Demonstrate teamwork and effective communication.
