1 Isle of Wight NHS Primary Care Trust Individual Placement (Job) Descriptions for Foundation Year 1 Placement F1 - Cardiology The department The Cardiology department works from a newly built (2005) Cardiac Centre with Coronary Care Unit, CCU step-down ward and adjacent Cardiology department housing investigation facilities and consulting rooms. There are 3 Cardiologists based at St Mary s though all have regular invasive sessions at Queen Alexandra s Hospital in Portsmouth, with whom we have close ties for interventional services. We also have links with Southampton and regularly have students attached to the department from there. The type of work to expect and learning opportunities Where the placement is based Clinical Supervisor(s) for the placement Main duties of the placement Typical working pattern in this placement FY1s in Cardiology are ward-based during working hours and are fully supported and supervised by FY2 and SpR colleagues and with daily consultant ward rounds. There is some involvement with clerking but acute admissions to CCU are generally seen first by the FY2. There is plenty of teaching, from Consultants and SpRs, including protected time, and opportunity to see cardiac investigations in the nearby department. Responsibilities include ward work following the daily rounds, prescribing, and completion of discharge summaries (shared with other junior colleagues). Emphasis is placed on building history-taking skills and recognising cardiac physical signs, and on ECG interpretation. Regular spells in the Medical Admissions Unit (MAAU) allow general clerking experience alongside that gained in Cardiology. Coronary Care Unit and CCU Step-down ward (includes 4 acute stroke beds) and MAAU where applicable. Usually one of the Consultant Cardiologists; Drs Mark Connaughton, Dallas Price, Dominic Lamb. Ward rounds, jobs arising from rounds, discharge summaries, prescribing, some clerking. Also involvement in acute admissions rota on MAAU and attendance at regular teaching sessions. 4 Placement F1 General Medicine (with Care of Elderly team) The department The medicine department at St Mary s has 17 consultants, 11 of whom are on the on call rota. An associate specialist, 10 SpR/ST3, 11 ST1/2/FY2 and 11 FY1s make up the rest of the general medical establishment. The specialists include endocrinologists, cardiologists, gastroenterologists, care of the Elderly physicians, rheumatologists and respiratory physicians. The type of work to expect and learning opportunities Where the placement is based Clinical Supervisor(s) for the placement Main duties of the placement Typical working pattern in this placement All F1 Doctors in hospital posts will generally be ward based during the normal working day and expected to deliver the daily medical care of all the patients on their ward irrespective of specialty. They will also obtain on call experience. The overall educational objectives of the F1 year are to provide the trainee with the knowledge, skills and attitudes to be able to Take a history and examine a patient Identify and synthesise problems Prescribe safely Keep an accurate and relevant medical record Manage time and clinical priorities effectively Communicate effectively with patients, relatives and colleagues Use evidence, guidelines and audit to benefit patient care Act in a professional manner at all times Cope with ethical and legal issues which occur during the management of patients with general medical problems Educate patients effectively Become life-long learners and teachers St Mary s Hospital, Newport, Isle of Wight. Rehab unit: Dr Gladdish. The F1 doctor is responsible with other staff for the ward care of patients and the maintenance of the patient s medical record. They will attend the consultant ward rounds and have the opportunity to be part of the multi-disciplinary team meetings. They are expected to attend the structured teaching programmes provided by the department. The doctor will be responsible for such other specific clinical duties as allocated by consultants including performing other duties in occasional emergencies and unforeseen circumstances. All the FY1 take part in the ward cover rota, 5pm-8pm weekdays and weekends. They also take part in the medical on call rota in the MAU and will be involved with the generic clerking of patients being admitted and the ongoing care of the patients in the unit. Each day starts with all junior doctors meeting the Lead Clinician for Medicine and General manager at 9am so that any staffing issues are identified early and can be acted upon.

All F1 Doctors in hospital posts will generally be ward based during the normal working day and expected to deliver the daily medical care of all the patients on their ward irrespective of specialty. Department of Psychiatry at St Mary s Hospital which has various components. In-patient services are based in Sevenacres Unit with general adult psychiatry admissions in Osborne Ward, psychiatric intensive care in Seagrove Ward and psychiatry of old age admission in Afton Ward. In addition there are various community sites; Woodlands Unit for rehabilitation and Shackleton Unit for assessment of dementia patients. There is Chantry House, where the general adult psychiatry team is based for the Acute and Recovery Team. The Rehab and Recovery Team is based in South Block at St Mary s Hospital, providing assertive outreach, community rehabilitation and community drug and alcohol service based at various locations on the Island. Dr Naeem for Osborne Ward and Dr Harwood / Dr Dixey for Afton Ward. Clerk in patients for general adult psychiatry and old age psychiatry. Assist Consultant / Staff Grade / Core Trainees or Specialty Trainees on the day to day management of the patients. Write discharge summaries. The post holder will be fully supported and supervised by the different Consultants and other members of the senior medical team. There will be ample opportunities for clerking in patients, requesting and carrying out basic medical investigations (blood tests etc). The post holder will be expected to attend weekly teaching sessions on Wednesdays with opportunities to do case presentations, journal presentations as well as presentations on any audit they may wish to carry out. In addition they can also attend psychotherapy teaching sessions and case based discussion groups which take place at Chantry House on Wednesday afternoons. Mostly ward rounds. 9 Placement The department The type of work to expect and learning opportunities Where the placement is based Clinical Supervisor(s) for the placement Main duties of the placement F1 - Respiratory and General Medicine The Respiratory Department is led by 2 consultants who both do general respiratory medicine and general internal medicine. The ward takes general and respiratory acute admissions form the Medical Assessment Unit. The department provides all secondary respiratory care for the local population of 140,000. Because of the relatively elderly population there is a high prevalence of respiratory disease, such as COPD and lung cancer. FY1 doctors are ward-based, looking after patients with general medicine and respiratory conditions (approx 50:50), providing good exposure to common conditions. The ward is also shared by a Gastroenterology consultant, and the junior doctors cross cover for each other, so also gain exposure to GE conditions. There are opportunities to learn procedures such as pleural aspiration and chest drain insertion which are taught and supervised by the consultants. Trainees are encouraged to attend bronchoscopy lists, and to learn about pulmonary physiology in the pulmonary function lab. On-call includes admitting general acute medical patients (average 15/day), and covering out-of hours ward problems. St Mary s Hospital, Newport, Isle of Wight. Dr Andrew Woolley. Two FY1s are responsible for general ward work for the respiratory ward, supported by 2 SHO grades/ct1 and 2 SpRs/ST3s (subject to on-call commitments/ leave). Approx 20 patients are covered by team, as well as cross-cover for a further 10 patients under the care of a gastroenterologist on the ward. 11 Placement F1 - General Surgery The department General Surgery is structured in 3 teams/firms 1- general surgery with an interest in colorectal surgery 2- general surgery with an interest in gastrointestinal surgery 3- general surgery with an interest in breast surgery The in-patient beds for the firms are shared between two wards, Whippingham and St.Helens (total 56 beds). There is also a 9-bed day surgical unit, access to a limited number of paediatric beds, and 6 intensive care beds. Some emergency patients in ENT and Maxillo Facial Surgery are admitted by Foundation Doctors under supervision of the relevant specialty Consultants. GP communication through discharge summaries. At present F1s also accept referral calls from GPs on emergency referrals. Technical training in theatre for minor procedures (self initiated). 13 Placement The department F1 Rehabilitation Medicine (Stroke) Adult Medicine and Rehabilitation - Both posts are based on the Stroke Unit, but the FY2 also supervises the FY1 on the General Rehabilitation Unit and cross cover the FY1 on the Stroke and Rehabilitation Units should any of them be on call at the MAAU. The stroke unit consists of the following services : (a)acute stroke Unit of four beds located at the Coronary Care Unit (CCU) step down, (B) Stroke Rehabilitation Unit (16 beds) located at a defined Geographical Unit in the North Hospital and (C) Transient Ischaemic Attack (TIA) clinic located within the Stroke Rehabilitation Unit building. A fast track TIA service is run daily between 1200 and 1400 hours to assess and investigate patients with ABCD2 score of 4 or more. Stroke Rehabilitation is the process of assisting a person disabled by stroke to return to an optimal level of health, activity such as walking and participation such as employment within the limits of the persisting stroke impairment. The type of work to expect and learning opportunities The work on this Unit includes: The Foundation Doctors are based on the Unit and are involved ion the day to day care of patients with Stroke and other conditions presenting like stroke until they discharge or transfer to other Units. Participation is the assessment and investigation of patients referred with TIA Continue the care of patients admitted to the Acute Stroke Unit between Consultant ward rounds. 16 Placement F1 - Orthopaedic and Trauma The department The Department of Orthopaedic and Trauma comprises of 5 Consultants, five Associate Specialists, 2 Specialist Registrars, 5 F2 doctors and 2 Orthopaedic Nurse Specialist. There are specialist interest in Hip, Knee, Shoulder, Feet and hand surgery. All consultants are actively involved in management of the acute trauma. The department serves Isle of Wight population of approximately 140,000. However in summer time this population doubles because of number of Tourists. The department has close links with tertiary services in Southampton, Portsmouth, Salisbury and Royal National Hospital in Stanmore. 18 Placement The department F1 - Urology Urology department is made up of One consultant urologist, two associate specialists, two specialty doctors, one FY1 and two FY1 s. The urology on offer ranges from urinary incontinence to oncology (Testicular, prostatic, bladder and renal). There are a number of cancer fast track services available such as the one stop haematuria clinic and the new testicular lump service. Whilst working in urology the F1 will be involved with the generic clerking of patients being admitted and the ongoing care of the patients in the unit. As well as assessing the patient to see if they are fit for surgery three times a week in pre assessment. They are expected to attend the structured teaching programmes provided by the department. The doctor will be responsible for such other specific clinical duties as allocated by consultants
