1 Position: EFT: Reports to: Role Purpose: Mental Health Practitioner Part Time (negotiable) Coordinator McAuley House The Mental Health Practitioner will provide best practice recovery-oriented clinical treatment and support strategies to McAuley clients. The incumbent will work to support a multidisciplinary approach to clients including taking a lead role in monitoring and assessing risk concerns for clients, risk of harm to self and/or others, mental deterioration, liaison with area mental health networks, clinicians and GPs Internal Relationships: External Relationships: Program Coordinators, Case Workers, women, staff and volunteers Relationship with North Western Melbourne PHN. Maintain and build strong relationships with referral services (GP, Psychiatrists, other Medical Practitioners), and other services and stakeholders involved with McAuley Community Services for Women PD Mental Health Practitioner Page 1

McAuley Community Services for Women is a ministry of the Institute of Sisters of Mercy of Australia and Papua New Guinea, impelled by the Gospel of Jesus Christ and enriched by the spirituality of Catherine McAuley, the founder of the Sisters of Mercy. Our mission is to provide accommodation, services, advocacy and support for homeless women, and women and their accompanying children, who experience family violence. Service Delivery Clinical Care coordination Assertive Outreach, linkage and follow up Intake/triage of new clients and wait list management Provision of support to clients on waitlists Provision of education sessions to clients Participation in clinical supervision, team meetings and case discussion of clients Participation in service review and service maintenance tasks Contribute to a safe and nurturing environment for women and children Prioritise the clinical needs of each client within the caseload monitor and assess risk concerns, including risk of harm to self and/or others; identify and assess escalating behaviour/mental deterioration Report on client goals and individual treatment plans to their GP or relevant health care practitioner as required Consult with direct line manager/on Call service about referrals for any client incident Additional tasks as delegated by the team leader PD Mental Health Practitioner Page 4

Administration Maintenance of electronic case notes, assessment forms, wait lists and evaluation forms Participation in evaluation of programs Uploading data onto relevant database Reporting back to referrers as required Marketing and promotion of the service to local service providers Adherence to all policies and procedures 5. Demonstrated ability to meet the expected standards of performance and competencies in relation to the requirement of the position description. Qualifications in AOD, social work, welfare, psychology or related health profession, including general registration under section 6 of the Psychologists Registration Act 2000 (VIC); Australian Association of Social Worker Guidelines, and/or as per AHPRA guidelines Demonstrated experience and knowledge of working with clients with drug and alcohol issues and/or forensic clients Previous experience working in a care coordination, case management or outreach role PD Mental Health Practitioner Page 5
