2 Alnylam Assist is here to help What is Alnylam Assist? The Alnylam Assist support program offers you a number of personalized services throughout your treatment with ONPATTRO (patisiran). This includes understanding your insurance coverage for ONPATTRO and options for financial support that you may be eligible for, materials to help facilitate conversations with your doctor and family, and assistance with connecting to local resources. Beginning treatment with ONPATTRO Begin working with your dedicated Case Manager Your Case Manager helps you understand your insurance coverage for treatment and explains options that you may be eligible for Get started on treatment Your Case Manager provides you with customized ongoing support throughout your treatment with ONPATTRO 2 Please see Important Safety Information on the following pages and accompanying Full Prescribing Information.
3 Partnering with your dedicated Case Manager Who are Case Managers? Once you decide to start treatment with ONPATTRO (patisiran), you will be partnered with your own personal Alnylam Case Manager. Case Managers are experienced in helping individuals get started on treatment and providing ongoing support. They will tailor their level of contact based on your personal needs. Your Case Manager can help you understand your insurance coverage and your options for financial support that you may be eligible for as well as provide you with educational resources. You can also look to your Case Manager to provide information about infusion centers nearest to you as well as information on transitioning to home infusions if you and your doctor decide that it is right for you. Please note that home infusions may not be covered by all insurance plans. 4 Getting started with ongoing support Understanding your benefits Your Case Manager will review your insurance coverage and answer any questions about your insurance benefits for ONPATTRO (patisiran). Financial assistance Alnylam Assist provides financial assistance options to qualifying individuals. Your Case Manager will work with you to determine eligibility. a I have commercial insurance Our Quick Start Program provides up to 3 doses at no cost while waiting for confirmation of insurance coverage for ONPATTRO Through our Commercial Copay Program, you'll pay $0 out-of-pocket for medication costs I m uninsured or I do not have coverage for ONPATTRO Our Patient Assistance Program may be able to provide you with ONPATTRO at no cost If you have Medicare, Medicaid, or other government-sponsored insurance, your Case Manager can provide you with information about charitable foundations or other organizations that may be able to provide assistance for eligible patients. b a Individuals must meet specified eligibility criteria to qualify for assistance. 5 Ongoing support Our personalized level of support doesn t stop once you start your treatment. We ll continue to provide assistance throughout your treatment whenever you need it. Disease education Education can help you make more informed choices about your health. Your Case Manager can connect you with an Alnylam Patient Education Liaison (PEL) who can answer questions about hereditary ATTR (hattr) amyloidosis and provide valuable resources to help you and your family learn more about the condition. Partnering with your dedicated Patient Education Liaison (PEL) Who are PELs? PELs have backgrounds in nursing or genetic counseling and are experienced in educating individuals and their families about matters related to hattr amyloidosis. PELs can help you in a variety of ways, including providing disease and product education, connecting you to additional resources, and answering questions about treatment.
6 Indication What is ONPATTRO (patisiran)? ONPATTRO is a medicine that treats the polyneuropathy caused by an illness called hereditary transthyretin-mediated amyloidosis (hattr amyloidosis). ONPATTRO is used in adults only. Important Safety Information What are the most important things I should know about ONPATTRO? ONPATTRO can cause: Infusion-related reactions ONPATTRO is given as a drip into a vein (called an intravenous infusion ). Reactions to this infusion may happen during treatment with ONPATTRO. Before each infusion you will be given medicines that help to lower the chance of infusion-related reactions. Tell your doctor or nurse right away if you experience any of the following signs of an infusion-related reaction during treatment: Reddening of the face or body (flushing), skin warm Body aches or pain, including pain in the back, neck, or joints Feeling sick (nausea) Stomach pain Feeling short of breath, cough, or other breathing problems Headache Chest discomfort or chest pain Rash Chills Dizziness 6 Please see Important Safety Information on the following pages and accompanying Full Prescribing Information. Feeling tired (fatigue) Rapid heart rate Facial swelling
7 If you have an infusion-related reaction, your doctor or nurse may slow down or stop your infusion, and you may need to take other medicines. When these reactions stop, or get better, your doctor or nurse may decide to start the infusion again. Low Vitamin A levels Treatment with ONPATTRO TM (patisiran) lowers the amount of vitamin A in your blood. Your doctor will tell you to take a vitamin A supplement every day. You should not take more than the amount of vitamin A recommended by your doctor. Low vitamin A levels can affect vision. If you have problems with your vision (e.g., night blindness) while taking ONPATTRO, talk to your doctor. Your doctor may refer you to an eye specialist. What are the common side effects of ONPATTRO? The most common side effects of ONPATTRO are respiratory infections, such as colds, sinus infections, and nasal congestion, and infusion-related reactions. These are not all the possible side effects of ONPATTRO. Talk to your doctor about side effects that you experience. You are encouraged to report negative side effects of prescription drugs to the FDA.
