1 Tay Ward Perth Royal Infirmary Information for patients, relatives and carers Introduction Welcome to Tay Ward. Telephone: Tay Ward is part of the Medicine for the Elderly Unit within the Perth and Kinross Health and Social Care Partnership and is accessed via Taymount Terrace at Perth Royal Infirmary. Tay ward provides rehabilitation for patients aged 65 years and over, working closely with many different health professionals that make up the multidisciplinary team to provide appropriate care and support. Discharge Planning begins on admission to Tay Ward. The team anticipates goals that need to be reached by working and discussing with you/your relative. The team has weekly case conferences to discuss progress and a target of a planned date of discharge is made and reviewed regularly. This is to ensure that all your needs are met for discharge to the most appropriate setting. Members of the Team Consultants: There are four consultants who rotate through Tay Ward. You/your relative will always have a main consultant but may be looked after by another consultant as they rotate. Ward consultants are: Dr Donna Clark Dr Dirk Habicht Dr John Harper Dr Neil Henderson They are responsible for the medical care of patients and are supported by junior doctors based on the ward. Consultants are present on the ward every day where they are updated on all patients and twice weekly they conduct ward rounds to review patients. Page 1 of 6
2 They also work closely with the multidisciplinary team and yourselves, to make decisions about future care needs. Social Work: Social workers work closely with patients, relatives and the multi-disciplinary team to carry out an assessment based on the patient and relatives needs and outcomes they would like to achieve for discharge. If it is not possible for a patient to return home, the social worker will help arrange an alternative option. Occupational Therapist: What do we do? Occupational therapists and technical instructors provide occupational therapy services. We work alongside doctors, nurses and other members of the team to help you overcome difficulties caused by illness, disability and memory problems. Occupational therapy aims to Work with you to improve, maintain and restore your skills with everyday tasks, for example, shopping, housework and preparing meals. Help you to be as independent and as safe as possible in your own home. Help you to maintain important roles in your everyday life, for example, parent, friend, spouse and carer. Look at all your current strengths and skills to see how these can be used in all of the above. Physiotherapist: The service is run Monday to Friday. Our aim Our aim is to help make you as independent and functional as possible during your hospital stay, preparing you for a successful and supported discharge. What we do We provide treatment in or around the ward or may ask you to attend the Physiotherapy gym. We: Carry out an assessment specific to your condition. Provide exercise programmes to improve your mobility, strength and pain control. Page 2 of 6
3 Assess you for and supply you with a suitable walking aid. Advise and educate you on how to prevent falls. Offer support and guidance for your carers or relatives. Work closely with the nurses, doctors and occupational therapists. Nursing staff: The Senior Charge Nurse of the ward is Shirley Wilcox. If you have any queries or concerns she will be happy to discuss them with you. Through the majority of a patient s time in hospital they will be cared for by nurses. We endeavour to work alongside the patient and their family/carers and the multi-disciplinary team to provide person-centred care. Nursing staff will help the patient/relative with all aspects of care whilst promoting health and well being and helping to achieve, maintain or recover independence. Any questions or problems can be discussed with nursing staff. Healthcare Assistant Healthcare assistants work alongside nurses, supporting the trained nurse role. Healthcare assistants will assist and encourage you to regain your independence Psychiatry of Old Age Liaison Team (POALT): The Psychiatry of Old Age Liaison Team (POALT) are involved with the assessment, review and follow up of patients who present with mental health problems such as dementia, delirium and depression, and the stress and distress that may be experienced with these conditions. General ward information Visiting times and ward number: Our direct telephone number is Tay Ward visiting times operate between 3 5pm and 7 8pm If you cannot visit within these times, we are flexible and you can visit out with them. 4 Meal times and food allowed/not allowed: Our meal times are: Breakfast: Lunch time: Evening meal: 8.15am 9.15am 12.30pm 1.30pm 5.15pm 6.15pm We operate protected meal times on the ward. Relatives are welcome to assist if they wish however nursing staff are on-hand to assist patients if they require help. There is a fridge located on the ward where patients can store food brought in by relatives. Please see the list below for food that is allowed in hospital, providing their diet allows and food that is not allowed in hospital. Food allowed in hospital: Pre-wrapped biscuits Fresh fruit and dried fruit Bottled or canned drinks Individual cartons of fresh juice Yoghurts Sweets and chocolate Crisps, nuts and crackers Cereal bars Individual pre-packed cakes without cream Food not allowed in hospital: Cooked meats and meat products Fish paste and fish products Fresh or synthetic cream Uncooked eggs Take-away meals Page 4 of 6
5 Communication: We ask that you avoid phoning at meal times. We would appreciate if there is one spokesperson per family who can phone the ward and pass on information to the rest of the family. You can always speak to any staff on the ward during visiting times and they will be happy to answer any queries you may have. Infection Control: The policy on the ward is 2 visitors to a bed. If there are a large number of visitors attending then we can arrange at times for the patient to go to the day room, however it must be remembered that the day room is for all patients to use. We also ask that visitors do not sit on patient s beds or chairs. There are designated visitor chairs. If you are feeling unwell we ask that you avoid visiting until you are clear of any symptoms. We also ask that visitors use the hand cleansing gel on entering and leaving the ward to help stop the spread of infections. Toiletries/clothing to bring in for patient: Clothes day wear Footwear with good soles preferably not slippers Nightclothes Underwear Toiletries Shop and newspaper facilities: There is a WRVS trolley service that visits the ward Monday Friday, where patients can purchase newspapers, toiletries and snacks, provided they have money. If you or your relative would like to buy anything from the WRVS trolley we ask that a small amount of money is kept with yourself or the patient. Valuables: NHS Tayside does not accept responsibility for the loss or damage to personal belongings unless it has been officially handed over to NHS Tayside and a receipt has been issued. Patients are asked to sign a personal disclaimer form in acceptance of this. Page 5 of 6
6 Laundry: Patients washing will be put into clothing bags by staff. This will be located in the patient s wardrobe. If a patient has any soiled linen this shall be put into a disposable clothing bag, where it can be put straight into a washing machine and removed from the bag when finished. We would appreciate it if this was picked up and washed on a regular basis. Hairdressing facilities: Tay Ward does not have its own hairdresser, however you or your relative/carer can organise for a mobile hairdresser to come to the ward.
