26 Modality Attwood Green IMPORTANT PATIENT MESSAGE Welcome to Modality Attwood Green Please be aware that we are moving to a new clinical computer system. The move will take place between 9 th 19 th November 2018, this is called the migration period. Unfortunately during this period patients will be unable to use the Patient Access online access facility. This will not affect your registration as a patient with this surgery, as your current medical record will be automatically transferred to our new system which is called SystmOne. If you are currently registered for online access with Patient Access we will need your permission to activate your details on the new online system, SystmOne Online. If you would like us to activate your account please Modality.mag@nhs.net using the subject Online Access. In your please confirm your full name, date of birth and Patient Access user name. Your new log in details and instructions on how to use SystmOne Online will be ed back to you after the migration period. Should you prefer to activate your account in person, or register for online access for the first time, please visit your surgery after 20 th November 2018 with a copy of this , which can be shown on your mobile device, and our Patient Services Assistants will be pleased to help you. Please note that if you have not previously been registered for online access for appointment booking and prescription requests please bring along photo ID and a proof of your address. NHS 111 You can call 111 when you need medical help but it is not a 999 emergency. NHS 111 is available 24 hours a day, 365 days a year. Calls are free from landlines and mobile phones. 29 Visitor Information Accessibility The main entrance has wheelchair access and lifts. We also have wheelchair accessible toilet facilities. There is a security reception desk situated on the ground floor, if you need any help please notify the security desk and our team will be informed and pleased to assist you. Facilities There is visitor parking behind the building. We ask that you park sensibly and considerately as the parking facilities are for all visitors at the Health Centre. There is on road parking available if visitor car park is full. All cars are parked at the owners risk, so please take note of any on-street parking restrictions. There is a bus stop directly outside the practice and is served by the number 80, 98 and 99 bus which stops on Bath Row. The nearest train station is New Street Station and is about a 15 minute walk. Registration We register patients within 3 miles of the surgery postcode. You can either register online from our website or in person at reception. You may be required to produce proof of identity including photographic identification and proof of address. For more information on how to register at the practice please contact the reception team who will be happy to give you advice and guidance. If you are unable to provide any details for registration please speak to a member of the reception team.
