1 Minato City Specified Fertility Treatment Subsidy Program Outline of Program In order to relieve the financial burden of fertility s, Minato City will subsidize a portion of the expenses related to specified fertility s (in vitro fertilization and micro-insemination only) and the sperm biopsy sampling, etc. (surgical procedure to retrieve sperm from the testes) leading up to these specified fertility s that is not covered by medical insurance. (The program is subject to change without notice. Please check the Minato City website, etc. to confirm the most current information.) 2. Amount of Subsidy (1) Specified fertility The maximum subsidy per fiscal year (April 1 of the current year to March 31 of the following year) is 300,000. (2) Sperm biopsy sampling, etc. (surgical procedure to retrieve sperm from the testes) as part of specified fertility The maximum subsidy per fiscal year (April 1 of the current year to March 31 of the following year) is 150,000. * You may apply for the subsidy as many times as you wish until the total amount of all subsidies received reaches the maximum permissible amount (there is no limit to the number of applications per fiscal year.). * If you receive a subsidy in connection with this from a prefecture or Metropolis, the amount remaining after deducting that subsidy is the amount of subsidy for which you are eligible to apply. * You can apply for the subsidy for a total of five fiscal years. However, please note that starting after the wife reaches 43 years of age will no longer be covered after fiscal 2021, so you may not be eligible to apply for the full five fiscal years. 3. Who is Eligible for the Program (qualifications for subsidy) 1 Marital status 2 Residence Medical facility for Subsidy from Metropolis or prefecture Subsidy from other city, town or village Conditions The couple applying for the subsidy must be continuously married from the first day of until the day of application. The couple applying for the subsidy must meet one of the following conditions of residence from the first day of until the day of application. (1) Both the husband and the wife must be continuously registered (2) If only one member of the couple is registered as a resident in Minato City, the member having the higher income must be continuously registered as a resident of Minato City. (However, persons residing outside of Japan are excluded.) Specified fertility s must be carried out at a medical facility designated by the prefecture or Metropolis (including designated or core cities). If the patient/couple is eligible under a Metropolis or prefectural, etc. specified fertility subsidy program, the patient/couple must have been granted that subsidy. The spouse having an address outside Minato City must not have received a subsidy for specified fertility s in another city, town or village (excluding prefecture or Metropolis) in connection with this. Couples in a de-facto/ common-law marriage are not eligible to apply for the subsidy Designated medical facilities can be confirmed on the Ministry of Health, Labor and Welfare website or directly with the medical institution. Required if you are eligible for the subsidy. * It does not matter whether the results in pregnancy or not. * Effective fiscal 2021, if the wife started after she reached 43 years of age, she will no longer be eligible for the subsidy. * There is no limit on income, but income will be verified to confirm eligibility for subsidy from Metropolis or prefecture. Applicants will be required to file an income tax return if they haven t done so (the return will be retained until confirmation can be made; however applicants residing outside Japan are excluded). -1-

Application Process Flow Under this program, if you are eligible for a subsidy under a prefectural or Metropolis, etc. specified fertility subsidy program, one of the conditions is that you must have received that subsidy. Eligibility and documents and procedures required for application may vary. Please confirm this first (there are restrictions on age and income). Information regarding Tokyo Metropolis specified fertility subsidy program may be found here: Website: Tokyo Metropolis Bureau of Social Welfare and Public Health, fertility subsidy * Further information may be obtained at the Minato Public Health Center, Health Promotion Section, Public Healthcare Subsection. Search (1) If you are eligible for the Tokyo Metropolis subsidy program No. Application to Tokyo Metropolis Application to Minato City 1. Obtain Tokyo Metropolis guide and application. (Download from website or request them from the Minato Public Health Center, Health Promotion Section, Public Healthcare Subsection window.) 2. Prepare the required documents and submit your application to Tokyo * Please make and retain a complete copy of the entire set of documents. These documents may also be used for your application to Minato City. 3. Apply to Minato City. Please refer to Page 3 for required documents. 4. You will receive notification of the results of your application (two (2) months after submission of your application). 5. When you receive notification of the results of your application from Tokyo Metropolis, please send/submit them to Minato City (within 30 days of approval by Tokyo ) 6. Required Documents If you are applying for the Minato City subsidy after applying for the Tokyo Metropolis subsidy You may use a copies of the documents you submitted with your application for the Tokyo Metropolis subsidy as part of your application for the Minato City subsidy. Please submit a copy of your notification of the results of your application for Tokyo Metropolis specified fertility subsidy program within 30 days of receipt. The following documents (1-4) must be submitted (document 1 may be downloaded from the Minato City website.) Required documents Application for Minato City Specified Fertility Treatment Subsidy Program (Form 1) Copy of Certificate of Medical Costs for Consultation/ Examination for Specified Fertility Treatments submitted to Tokyo Copy of receipt issued by designated medical facility that you submitted to Tokyo * Please do not submit the original. Copy of notice of approval for the Tokyo Metropolis (etc.) specified fertility subsidy program. If both the husband and the wife are continuously registered residents of Minato City from the start of until the day of application, either party may submit the application, however the bank account designated to receive the subsidy should be in the name of the applicant (if the account is in the wife s maiden name, please enter Maiden name ) * If the designated bank account is in the name of a party other than the applicant, a proxy letter will be required (no special form). The same applies for the total amount in case a Certificate of Consultation/Examination for Intratesticular Biopsy/Sperm Retrieval is being submitted. Please submit within 30 days of receipt of notice of approval. If the approval is not issued/received within the application period, please attach a note stating this fact when you submit your application documents (no special form). I will submit the notice of approval for the Tokyo Metropolis (etc.) specified fertility subsidy program within 30 days from the day following receipt of the notice. In some cases the following documents (5-8) may be omitted Copy of Resident s Register submitted to Tokyo Copy of documents submitted to Tokyo Metropolis certifying that parties are legally married to each other (Family Register, Certificate of Marriage, etc.) Copy of certificate of income, etc. submitted to Tokyo Copy of Certificate of Consultation/Examination for Intratesticular Biopsy/Sperm Retrieval submitted to Tokyo Metropolis Not required if the applicants are registered *Applicants who have moved overseas must attach an official copy of the Family Register entry. Not required in cases where both parties are registered residents in the same household in Minato City and their marital relationship can be confirmed. Not required for applicants subject to the Inhabitant s Tax (whether liable for tax or not) in Minato City for that fiscal year. Required only if applying for the male fertility subsidy. -3-
4 If you are not eligible for the Tokyo Metropolis subsidy program and are applying only for the Minato City subsidy The following documents (1-3) must be submitted (documents 1, 2 and 7 may be downloaded from the Minato City website) Required documents Application for Specified Fertility Treatment Subsidy Program (Form 1) Certificate of Medical Costs for Consultation/ Examination for Specified Fertility Treatments (Form 2) Copy of receipt issued by a designated medical facility. * Please do not submit the original. If both the husband and the wife are continuously registered residents of Minato City from the start of until the day of application, either party may submit the application, however the bank account designated to receive the subsidy should be in the name of the applicant (if the account is in the wife s maiden name, please enter Maiden name ) * If the designated bank account is in the name of a party other than the applicant, a proxy letter will be required (no special form). Please obtain a certificate from the medical facility after completion of one series of s. All costs shown on Document 2 above, Certificate of Medical Costs for Consultation/ Examination for Specified Fertility Treatments; same if submitting a Certificate of Consultation/Examination for Intratesticular Biopsy/Sperm Retrieval. The following documents (4-7) may be omitted in some cases Official copy of Family Register (issued within the past three months). Documents certifying that the parties were continuously legally married to each other (Family Register, Certificate of Marriage, etc.) from the start of until the day of application (issued within the past three (3) months). Certificate of Inhabitant s Tax (whether liable for tax or not); copy of Inhabitant s Tax determination notice. * Fiscal year varies, depending on month of application. Month of application Certificate of Inhabitant s Tax (whether liable for tax or not); copy of Inhabitant s Tax determination notice. January May, 2017 Fiscal 2016 (*1) June December, 2017 January May, 2018 Fiscal 2017 (*2) Not required if the applicants are registered *Applicants who have moved overseas must attach an official copy of the Family Register entry. Not required in cases where both parties are registered residents in the same household in Minato City and their marital relationship can be confirmed. 5 6. Application Period In principle, the application deadline is the last day of the fiscal year (March 31) during which was completed. However, please refer to the chart below for special cases that may apply. (1) In cases where was completed between January and March and application cannot be made by the last day of the current fiscal year (March 31), applications may be submitted between April 1 and June 30, but such applications will be considered part of the next year s subsidy. Issuance by the designated medical facility of the Certificate of Medical Costs for Consultation/ Examination for Minato City Specified Fertility Treatments has been delayed. Please attach a note (no special form) stating this fact and submit your application together with all other required documents during the application period. I will submit the Certificate of Medical Costs for Consultation/ Examination for Minato City Specified Fertility Treatments as soon as it has been issued. b. Issuance of the Tokyo Metropolis, etc. Certificate of Medical Costs for Consultation/ Examination for Specified Fertility Treatments has been delayed. Please attach a note (no special form) stating this fact and submit your application together with all other required documents during the application period. I will submit the notice of approval for the Tokyo Metropolis (etc.) specified fertility subsidy program within 30 days from the day following receipt of the notice. 7. Other Submitted documents cannot be returned. Please also refer to the FAQ (Q & A). All costs related to postage and fees, handling fees, etc. related to obtaining documents and certificates required for the application are the responsibility of the applicant. Forms submitted in person at the window will only be accepted; they will not be examined or checked at that time. Applicants may be contacted by a person-in-charge in case of omissions or other defects in the application. Please be sure to enter a telephone number at which you can be reached during the day. 8.
