2 Overview PreventionFIRST! announces the availability of funds for the FY2018 Hamilton County Family Services and Treatment Prevention Program grant. This is a substance use/misuse prevention-based grant opportunity. Funding Opportunity Title: Hamilton County Family Services and Treatment Prevention Program Funds are available for either coalition mobilizing, planning or implementation Eligible applicants: The application must be submitted by a community-based coalition or group of community members committed to developing a community-based substance use/misuse prevention coalition. Letter of Intent (LOI) Due: November 12, 2018 Notification of Accepted LOI Project: On or before November 19, 2018 Required Grant Writing Workshop: November 30, 2018 (Selected applicants will be invited to attend based on accepted Letters of Intent) Required Learning Collaborative Grantee Meetings: March 8, 2019 and August 2, 2019 Final Grant Applications Due: By close of business (COB) on December 17, 2018 Money Available To Be Spent: After January 4, 2019 Total Available Funding: Award Levels: $10,000 (There is no match required for this grant) Up to $1,000 for Mobilizing Up to $2,000 for Planning Up to $3,000 for Implementation Length of Project Period: January December 2019 PreventionFIRST! 2
3 Reporting Requirements: Two reports are due to PreventionFIRST! for this grant once funds are administered. The reports will provide an update on the goals, action plan and areas of improvement, or technical assistance needed. A mid-year report is due June 14, 2019 and a final report is due December 13, 2019 to PreventionFIRST! Local Coalition Development Manager Amanda Conn Starner at aconnstarner@preventionfirst.org. 4 Funding Opportunity Description The Hamilton County Mental Health Recovery Services Board has contracted with PreventionFIRST! to support local coalition development in Hamilton County. This grant is intended to improve the capacity of a current coalition or group looking to implement prevention. Substance use/misuse prevention is defined as preventing the onset of alcohol and other drug (AoD) use, misuse and substance use disorders. AoD prevention is a planned sequence of activities that, through the practice and application of evidence-based prevention principles, policies, strategies and programs, is intended to inform, educate, develop skills, reduce risk behaviors, affect environmental factors and/or provide referrals to other services. This grant is not intended to merely fund programs that do not move the capacity of a coalition or prevention-intended group. Movement toward positive community change focused on substance use/misuse prevention in a particular community needs to be demonstrated. Goal 1: Establish and strengthen collaboration among community sectors and county entities to support the efforts of community coalitions to prevent and reduce substance use/misuse among youth.* (*youth is defined as individuals 18 and younger.) Goal 2: Reduce substance use/misuse among youth and, over time, among adults by addressing the factors in the community that increase the risk of substance use/misuse and promoting the factors that minimize the risk of substance use/misuse.* (*substances include, but are not limited to, narcotics, depressants, stimulants, hallucinogens, cannabis, inhalants, alcohol, and tobacco, where their use is prohibited by federal, state, or local law.) Grantees are required to work toward these two goals as the primary focus of their county-funded effort. The funds have been made available through the Family Services Treatment Levy since November 2009 and outcomes are reported back to the Hamilton County Mental Health Recovery Services Board. 5 Letter of Intent Requirements A letter of intent (LOI) must be submitted by the lead agency, coalition or organization for the proposal. The letter should be no more than two single-spaced pages. You must indicate in the letter if you will be applying for a mobilizing, planning or implementation grant; up to $1,000 will be awarded for a mobilizing grant; up to $2,000 for a planning grant and up to $3,000 for an implementation grant. The letter should include: A description of the project Describe how the project will build the capacity of your group Zip code(s) served Brief statement of why your intended project is needed in the community Local data indicating substance use/misuse problems in your community Describe how community partners will collaborate Letters of intent must be submitted by COB Friday, November 12, 2018 to PreventionFIRST! Grant Application Requirements Applicants are eligible for up to $3,000. Annual awards are contingent upon the availability of Hamilton County Family Services and Treatment Prevention Program funds, the continued ability of the coalition(s) to demonstrate eligibility, and grantee(s) progress in meeting grant requirements, as well as all required data and reports. There is no match required for this grant. Applicants will need to choose whether they will be using the funds for prevention coalition development focused on either 1) mobilizing 2) planning 3) implementation. Please choose one. A mobilizing grant would permit a community group to collect quantitative and qualitative data to identify the substance use problem in the community. This type of grant would allow a group to identify existing prevention resources that would be collaborators for a coalition. Strategies can include development of vision, mission, communications strategy, community forums, administer the Student Drug Use Survey, conduct key leader interviews/focus groups/environmental scans, leadership and membership recruitment. A planning grant would be used to plan for eventual implementation of environmental strategies in the community, based on your local substance use/misuse identified problem. It is important to explain the root causes of the problem through the collected data. By choosing a planning grant, the group can begin to plan for the intervention and outcomes sections of the logic model process. This would entail carrying out a community-based planning process that identifies environmental strategies based on the 7 Strategies for Community Change by the Community Anti- Drug Coalitions of America (CADCA) in order to achieve outcomes. An implementation grant allows for a group to ensure the strategies and activities chosen are evidence-based and/or evidence-informed practices moving toward PreventionFIRST! 5
6 positive community change related to prevention with policies, maintain multi-sectors in the plan that is culturally competent and implemented with fidelity. If you have received funding from the Hamilton County Family Services and Treatment Prevention Program grant in the past, you are eligible to apply again if you have demonstrated prudent fiscal management and completed action plans. If selected to receive a grant, funds will be awarded for one year, from January 4, 2019-December 31, Eligibility Requirements The Hamilton County Family Services and Treatment Tax Levy of 2009 authorizes grant funding for community coalitions in Hamilton County. Applications submitted must meet these requirements prior to submitting their grant application: Eligibility Requirements: Requirement 1: The project must have as its principal mission the prevention and reduction of youth substance use/misuse. Requirement 2: The applicant must be a public or private nonprofit, governmental, or faith-based organization. Requirement 3: The project must serve communities and their residents within Hamilton County. Requirement 4: The project must include environmental prevention strategy in submitted action plan. This requirement needed for only planning and implementing applications. (See attachment 3 for environmental strategy examples) Requirement 5: The project must assist with improving capacity of an existing community coalition or begin mobilizing one if a coalition does not currently exist in the community. How to Document: Applicants must include their organization s mission statement. Mobilizing grants must demonstrate their plan for developing their mission statement, if not already in place. Applicants must attach a copy of the appropriate status letter from the IRS. If the applicant is not a public or private nonprofit, governmental, or faith-based organization, a fiscal agent must be used and a copy of the fiscal agent s appropriate status letter from the IRS must be included. Applicants must attach the zip code(s) to be served. Applicants must include in action plan ways the project will incorporate environmental prevention strategies to achieve outcomes at the end of the grant. Applicants must include in narrative how the coalition or organization will build their capacity as a result of this grant. PreventionFIRST! 6
7 Requirement 6: The project must include attendance at required learning collaborative grantee meetings. Learning Collaborative Grantee Meetings: March 8, 2019 and August 2, 2019 at PF! Local Coalition Development Manager Amanda Conn Starner: Formatting Requirements Applicants who do not comply with the following requirements will have points deducted in the scoring process. Text must be typed in black, single-spaced with one column per page Font of Times New Roman 12, with all margins at least one inch each Pages should not have printing on both sides Paper must be white and 8.5 by 11 inches in size Pages should be numbered consecutively from beginning to end following the required outline Any attachments, other than those required, should not be included General Instructions There are a series of questions and topics that you must address in written narrative form. The information and responses you provide in these sections form the Project Narrative portion of your application. If you meet the eligibility requirements, your Project Narrative will be evaluated and scored. In developing the Project Narrative section of your application, use the instructions outlined below, which have been tailored to this program. Project narrative meets page length requirements depending on type of grant chosen. Project must list which of the 12 community sectors are represented in your grant project. Sectors can be found here: Respond to every question in each category of the project narrative individually. Data must be submitted about the substance use/misuse problems in the community. (Examples of data sets that can be submitted include Student Drug Use Survey (SDUS), police data re: substance abuse related crimes for youth, fire dept./ems ambulance run data number of individuals in community transported for substance use or misuse, school expulsion or substance abuse violation data and hospital data.) Attend at least one training as part of allocated grant funds that is prevention based either hosted by PF!, Interact for Health, Ohio Department of Mental PreventionFIRST! 7
8 Health and Addiction Services, Prevention Action Alliance, Statewide Prevention Coalition Association, Alcohol and Drug Abuse Prevention Association of Ohio. There is no page limit for the one-year budget and budget narrative section; however, you must use the table provided. Applicants must include documentation to verify that the applicant or fiscal agent is a tax-exempt organization. Applicants must submit organization s mission statement. Outline of Grant Application Applications must follow this checklist and order of narrative questions when compiling your application. Failure to follow the outline may result in deducted points during the scoring process. Each section, and the required information for each, is detailed below. Cover Sheet (first page of application) (See attachment 1) Project Narrative which should include organization s mission statement Detailed Action Plan including timeline of activities and who will be involved in the project; attached template must be used(see attachment 2) Budget & Budget Narrative; attached templated must be used (See attachment 4) Supporting Documentation Eligibility Requirements Copy of organization s mission statement Copy of organization s tax exempt status letter from IRS or copy of fiscal agent s appropriate status letter from IRS Attach listing of which zip codes will be served in Hamilton County Project Narrative If you chose a mobilizing grant, answer these project narrative questions. Answers to these question are not to exceed three (3) pages. MOBILIZING (Funding up to $1,000) 1. Describe why your group wants to start a coalition. 2. Or, if you are missing data, how do you plan to obtain youth substance use/misuse related data? Responses should address, but are not limited to, the following: a. Data related to youth consequences (i.e., social indicators) such as crime data, juvenile justice/social services referrals, school failure, impaired driving rates, emergency room data, substance use/misuse treatment admissions, teen pregnancy rates, dating violence and sexual assault statistics PreventionFIRST! 8

11 Budget All applicants must provide a one-year budget and budget narrative using the budget table included: (See attachment 4) Unallowable expenditures Award dinners & fundraising events Capital construction or improvement Operating/indirect expenses such as utilities, rent, etc. Salaries Religious organizations for religious purposes Political causes, candidates, organizations or campaigns Grants to individuals Food/beverages that exceed 5 percent of the total budget Debt retirement Alternative school activities (i.e. after prom parties) Cash incentives or Visa/Mastercard gift cards Allowable expenditures Evidence-based prevention programming and/or training Supplies/photocopying/postage & mailing Marketing/promotion cost for the specific project Local travel Food/beverages up to 5 percent of the total budget Small stipends for consultants, not to exceed 10 percent of total budget Small incentives such as prevention products, gift cards to specific retail establishments such as Starbucks, Kroger permitted, however, the cards cannot exceed 10 percent of total budget Award Notices PreventionFIRST! will notify you through that your application has been received. If you are approved for funding, you will receive a Notice of Award (NOA), signed by PreventionFIRST! The NOA is the sole obligating document that allows the grantee to receive the county funding for work on the grant project. Administrative Requirements Grantees must comply with all terms and conditions of the grant award. Grantees will be held accountable for the information provided in the application related to performance targets. Failure to meet stated requirements or goals and objectives may result in suspension or termination of the grant award or in reduction or withholding of payments. PreventionFIRST! 11
