1 YALE UNIVERSITY WOMEN S ORGANIZATION SCHOLARSHIP Summer Session 2018 May 31, 2019 The Yale University Women s Organization (YUWO) offers Scholarships to women associated with the Yale community, whose formal education has been interrupted or delayed and who wish to resume their studies or enhance their careers. An applicants Yale association may be through her own affiliation or that of her immediate family (past or present). Current members of YUWO and their families are not eligible. Each applicant is judged upon merit and need. The scholarship may be used only for course tuition and academic fees for the academic year starting Summer Session 2018 and ending May Please note: You MUST have Adobe Reader XI installed on your computer in order to be able to save your completed application. (Prior versions of Reader will not permit the completed form to be saved.) You can download Adobe Reader XI here: The application form should be downloaded to your computer and saved. Open the form USING Adobe Reader XI, complete it, and save it again. You can then attach it to an to submit it. Please submit the completed application form and all supporting material by one of the following means: 1) a pdf of the application and supporting material to Margaret Mann at boyermann@gmail.com. Include: (1) reasons you desire further study, (2) career/educational goal, (3) a brief statement about the program or courses you will take, and (4) how this award will affect your plans. Please include any other relevant information that you feel may uniquely qualify you for this scholarship. 2. Provide two letters of recommendation, preferably from your supervisor at work and from an advisor or professor at the academic institution, if you are currently enrolled. The letters must be typed and signed on letterhead stationery. The letters can be sent as a pdf, directly from the recommender s , to Margaret Mann at boyermann@gmail.com or mailed to Margaret Mann, YUWO Scholarship, 50 Underhill Rd, Hamden, CT Submit documentation of Yale affiliation. This may include your Yale ID, Spouse s ID, or other clear documentation of Yale affiliation. 4. Please enclose a copy of your 2017 W-2 form(s), your spouse s W-2 form(s) if filing jointly, and the first page of your 2017 tax return. Black out your Social Security number. The W-2 and 2017 tax return must be received by April If you are currently in school, submit a copy of your transcript.
