1 Official Rules and Regulations BIRTHDAY PROMOTION MAY 1, 2012 MARCH 31, 2013 OLG SLOTS & CASINOS ONTARIO LOTTERY AND GAMING CORPORATION CONTEST ELIGIBILITY REQUIREMENTS: 1. This promotion is sponsored by the Ontario Lottery and Gaming Corporation (OLG) and is open to Winner s Circle Rewards members who are nineteen (19) years of age or older who are not selfexcluded, trespassed and/or archived as determined by OLG and excludes employees of any of the following: OLG, the Alcohol and Gaming Commission of Ontario (AGCO), and OLG s advertising, promotion, and contest management agencies. No purchase necessary. 2. There is no fee payable to obtain a Winner s Circle Rewards membership OLG is not responsible for any delays in processing membership applications. 3. This promotion takes place at each of the following nineteen (19) OLG Slots and Casino sites: OLG Casino Brantford, OLG Casino Point Edward, OLG Casino Sault Ste. Marie, OLG Casino Thousand Islands, OLG Casino Thunder Bay, OLG Slots at Ajax, OLG Slots at Clinton, OLG Slots at Dresden, OLG Slots at Flamboro, OLG Slots at Georgian, OLG Slots at Grand River, OLG Slots at Hanover, OLG Slots at Kawartha, OLG Slots at Mohawk, OLG Slots at Rideau Carleton, OLG Slots at Sudbury, OLG Slots at Western Fair, OLG Slots at Woodbine, and OLG Slots at Woodstock (individually, the Site ; collectively, the Sites ). 4. Any personal information provided by entrants is collected pursuant to the Ontario Lottery and Gaming Corporation Act, 1999 and is intended to be used for the following principal purposes: marketing and advertising including providing entrants with valuable information and other gaming related correspondence, assisting OLG in developing and improving segment-specific marketing strategies, and assisting OLG in keeping the Winner s Circle Rewards program relevant and rewarding for Winner s Circle Rewards members; understanding entrants interests; responding to entrants concerns and improving service delivery; for OLG s internal business purposes and in connection with any necessary customer conduct reviews; for the purpose of disclosing personal information to the Alcohol and Gaming Commission of Ontario, insurance companies, law firms, and law enforcement agencies for security, health and safety, and legal or regulatory compliance reasons; and to comply with legal requirements. OLG may also provide personal information to its agents, suppliers and service providers in conjunction with providing entrants with OLG s product and services. For further information, call the OLG Support Centre at The Birthday promotion will run at all OLG Slots and Casino Sites during the following month in 2012: May, June, July, August, September, October, November, December and the following month in 2013: January and February (individually the Month; collectively the Months. ). The Birthday promotion will be designated for each of the months, and each month s promotion will be referred to as the [name of month] Birthday promotion. 6. In the event a selected entrant does not meet the eligibility requirements, then OLG has no obligation to award any prize to that selected entrant. 7. Winner s Circle Rewards Gold members will be eligible to receive one (1) $25.00 slot play offer during their birth month (as determined by OLG). Gold members are described as having accumulated 5,000 (five thousand) Winner s Circle Rewards points or more through slot play within a 12 month period (as determined by OLG). Winner s Circle Rewards Silver members will be eligible to receive one (1) $10.0 slot play offer during their birth month (as determined by OLG). Silver members are described as having accumulated 1000 (one thousand) to 4999 (four thousand ninety nine) Winner s Circle Rewards points through slot play within a 12 month period (as determined by OLG). Winner s Circle Rewards 1

2 members will be eligible to receive one (1) $5.00 slot play offer during their birth month (as determined by OLG). Rewards members are described as having accumulated 1 (one) to 999 (nine hundred and ninety nine) Winner s Circle Rewards points through slot play within a 12 month period (as determined by OLG). The 12 month period that determines membership levels begin each year on October 1 st and ends on September 30 th of the following year. 8. Winner s Circle Rewards members who move up in membership level will be eligible to receive the offer designated for that membership level. By way of example, if a Winner s Circle Rewards member moves up to the Silver membership level, he/she will be eligible to receive one (1) $10.00 slot play offer and one (1) electronic draw entry during their birth month. 9. Winner s Circle Rewards members must visit a site during their birth month (as stated in OLG database) and have their Winner s Circle Rewards card swiped/entered at the Winner s Circle Rewards desk or Express Kiosk to receive the slot play offer per OLG procedures and to be entered in the electronic draw system. SLOT PLAY OFFERS AND CHANCES OF WINNING 10. Slot play offers are available to all Winner s Circle Rewards members, as described in Rule # 7 with an active account status (this excludes self-excluded, trespassed, and/or archived members, as determined by OLG) and described in the prize chart below: 11. Eligible members may participate in this promotion at any Site. Limit: one (1) $25.00 slot play offer per Winner s Circle Rewards Gold member or one (1) $10.00 slot play offer per Winner s Circle Rewards Silver or one (1) $5.00 slot play offer per Winner s Circle Rewards level member per birth month across all Sites. Denominations in which slot play prizing is issued may vary by Site. Winner s Circle Rewards members who do not visit any of the Sites, as listed in Rule #3 during their birth month will not be eligible to receive the offer. 12. All Winner s Circle Rewards members must swipe their Winner s Circle Rewards card at the Winner s Circle Rewards Express kiosk or the Winner s Circle Rewards desk during their birthday Month, based on OLG s database records, for their slot play offer as listed in Rule #7 and described in the prize chart below: Membership Level Gold Silver Rewards Prize Electronic entry into the Monthly Birthday Draw and on-site $25.00 Promotional Ticket Electronic entry into the Monthly Birthday Draw and on-site $10.00 Promotional ticket Electronic entry into the Monthly Birthday Draw and on-site $5.00 Promotional ticket 13. Any reference to "Slot Play" requires that the member redeem the Slot Play offer at the Winner's Circle Rewards desk for a Promotion Ticket. Promotion Tickets (Slot Play) do not have cash value, and cannot be redeemed or exchanged for cash. The member will be required to play the full value of the Promotion Ticket at one eligible slot machine (valid slot machines are clearly marked). If a member has more than one (1) Promotion Ticket, each individual Promotion Ticket can be used at a number of eligible slot machine(s) equivalent to the number of Promotion Ticket(s) the member has in hand. 2

3 Promotion Tickets expire as indicated on the ticket expiry date and/or time. The member will be eligible to receive winnings or credits (if any) generated by this Promotion Ticket. 14. Limit of one (1) Slot Play offer per eligible Winner s Circle Rewards member per birth month across all Sites. MONTHLY BIRTHDAY DRAWS AND CHANCES OF WINNING HOW TO ENTER (NO PURCHASE NECESSARY): 15. The Monthly Birthday Draw is open to all active Winner s Circle Rewards members in each membership level (i.e. Gold, Silver, and Rewards) whose birth month is the month of the promotion. 16. Commencing on the first day of every Month at 9:00 a.m. or Site opening time (whichever is later) until the last day of that Month at 11:59 p.m. or Site closing time (whichever is earlier), each eligible member must swipe their Winner s Circle Rewards card at the Winner s Circle Rewards Express kiosk or the Winner s Circle Rewards desk during their birthday month, based on OLG s database records, to receive one (1) electronic entry into the Monthly Birthday Draw. 17. Winner s Circle Rewards members in each membership level (i.e. Gold, Silver, Rewards) will be eligible to receive one (1) electronic draw entry during their birth month to be entered into a draw for a chance to win one (1) of one-hundred seventy three (173) prizes of one-hundred dollars ($ CAD) per month across all Sites. The number of draw winners per Site is based on Site Category as described in Rule # One (1) electronic draw entry per Winner s Circle Rewards member in each membership level per birth month across all Sites. MONTHLY BIRTHDAY DRAW PRIZE(S) 19. Each monthly birthday draw winner will receive one-hundred dollars ($ CAD) in cash. 20. Cash vouchers may only be redeemed at Cage & Coin of the issuing Site. Cash vouchers must be redeemed within twenty-four (24) hours of issuance. 21. Selected eligible Winner s Circle Rewards Gold members as described in Rule #7, that have consented to receive mail will receive a Birthday card sent to the address (as stated in the OLG database) notifying them of their Birthday offer to be redeem per Rule # Selected eligible Winner s Circle Rewards Silver and Rewards members as described in Rule #7, that have consented to receive will receive a Birthday card sent to the address (as stated in the OLG database) notifying them of their Birthday offer to be redeem per Rule #12. MONTHLY BIRTHDAY PRIZE DRAW 24. On or after the last day of the promotional month, draw entries will be electronically drawn by OLG Corporate for each of the nineteen (19) participating Sites, from all eligible entries received per Site, for the Monthly Birthday Draw. 25. The chance of being selected in the Monthly Birthday Draw held at the end of each promotional month is dependent on the number of eligible entries received for that draw at each site. 26. Winners will be contacted on or after the 15 th day of the following month. By way of example, if a member is selected as a winner in the December 2012 birthday draw, the winner will be contacted on or after January 15 th, Selected winners do not have to be in attendance at the time of the draws in order to win. Attempts will be made to award the selected winners with the respective prize by contacting the selected winners. The first attempt will be made on receipt of the winner s for OLG Gaming CRM. The second attempt will be made seven (7) days later. Winners will have sixty (60) days from the first attempt of contact to claim the prize. Attempts will be made to contact the selected winners at his/her telephone number (as recorded in the OLG database) if the member has provided phone consent, from 9:00 am to 10:00 pm or by (as recorded in the OLG database), if the member has provided consent, to arrange a time to visit the WCR desk to claim his/her prize. The selected winner is responsible for returning any telephone or message left by the WCR representative within the time frame stipulated in the message in order to remain eligible to win the prize. If selected winner does not have phone or consent, then a note will be added to the member s account and if, within the time specified above, the member returns to the Site, the staff member will inform the winner of the prize. 28. Once the entrant has been notified, he/she will have up to sixty (60) days to claim the prize at the Site. The prize must be claimed at the Site from which the winner was notified. The prize must be claimed in person. The member must present valid government issued identification in order to claim the prize. If the prize is not claimed within sixty (60) days, the prize will not be awarded and OLG has no obligation to award any substitute prize. OLG reserves the right to verify the full legal name of the claimant with the information on the electronic draw entry. 29. If, at the end of the above mentioned time period, the selected winner has not been contacted or otherwise declared a winner for that prize, he/she will no longer be eligible to receive the prize. GENERAL RULES 30. All participants must present valid government-issued photo identification upon request. 4

5 31. All vouchers/coupons/offers are the property of OLG. Vouchers/coupons/offers which have been obtained or made through fraudulent means or which are illegible, altered, reproduced, mutilated, irregular, or appear to be tampered with are subject to disqualification. OLG will not be responsible for coupons/voucher/ballots, direct mail pieces or s, which are lost, destroyed, deemed unmailable, as determined by Canada Post, misdirected, or delayed for any reason whatsoever, or for the Site s capacity constraints or for the removal or the non-removal of any patrons, or for any incorrect or inaccurate data in the WCR database, any incorrect or inaccurate capture of entry information, technical malfunctions, human or technical error, visual or printing errors, or garbled data or transmissions, omission, interruption, deletion, defect or failures of any telephone, computer line or network, computer equipment, software or any combination thereof. Where such failures cannot be corrected immediately or within a reasonable time frame to allow for a continuation of the promotion, the Site may, at the Site s option, use a manual back up system and/or disqualify the affected participant. 32. All decisions of the judges with respect to any element of this promotion are final. 33. In the event that an entrant does not meet the eligibility requirements, then OLG has no obligation to award any prize(s) to that entrant. 34. It is the participating member s responsibility to verify that the WCR membership number appearing on the coupon/voucher/ballot/offer is the same as the membership number appearing on his/her WCR card. OLG will not be responsible for any errors in the membership number. 35. As per the Winner's Circle Rewards program rules and regulations (contained within the Winner's Circle Rewards application) any Winner's Circle Rewards card used in this promotion can only be used by the named cardholder. The use of any other person's card(s) to obtain an entry will result in any issued entry being null and void and ineligible to win any draws. 36. The only way to receive a direct mail piece is to meet the criteria as determined by OLG, based on OLG database records. OLG will not be responsible for errors in database entries or mail delivery errors. 37. No offer substitutions will be made, save for OLG s right, at its sole discretion, to make an offer substitution of equivalent value. Vouchers and coupons not redeemed prior to the expiry date will automatically become void and will not be redeemable at a later date. OLG has no obligation to award substitute offers to those WCR members who have not redeemed their offer(s). 38. Offers/vouchers/coupons sent by mail or courier to the OLG or any of the participating Sites, or offers/vouchers/coupons not submitted in person at one of the participating Sites, will not be validated for redemption. 39. OLG reserves the right to verify the full legal name and address of the claimant with the information on the selected entry form. To claim the prize, valid government issued photo identification OR valid WCR card together with an acceptable (as determined by OLG) form of valid government issued nonphoto identification will be required. 40. OLG is not responsible for commencing, continuing or completing Birthday Promotion, or awarding prizes in the event of circumstances beyond OLG s control, and OLG may, at its sole discretion, change or alter this promotion, as OLG deems necessary. 41. OLG reserves the right to change or alter a promotion, as it deems appropriate, including making changes or alterations to these rules and regulations relating to the accommodation of participants in accordance with the Accessibility for Ontarians with Disabilities Act, 2005 (AODA). 42. Members with self-excluded or trespassed status or whose Winner's Circle Rewards account become archived during the promotional period, as determined by OLG, will not be eligible for any prizes associated with OLG contests, promotions, tournaments or giveaways. 5

OLG may, at its sole discretion, disqualify an entrant and seek damages and/or other relief, to the fullest extent permitted by law, from all persons responsible for any attempt to tamper with the entry process, interfere with the contest rules, and deliberately damage any kiosk or undermining the administration, security or legitimate operation of this contest. 44. Any personal information provided to or collected by OLG or the Contest Management Agency, including but not limited to entrants contact information, is collected pursuant to the Ontario Lottery and Gaming Corporation Act, 1999 and is intended to be used for the following principal purposes: to conduct and administer the contests by OLG or through a 3rd party Contest Administrator, including but not limited to the awarding of the Prize(s) and referring to such personal information in voice messages or other messages left for a selected entrant, for improving how OLG markets or delivers its product(s) to consumers, and to comply with legal requirements. These Official Rules and Regulations govern the promotion and must be followed and by agreeing to participate in the promotion, all entrants agree to comply with, and be bound by these Official Rules and Regulations. 47. This promotion is subject to the Laws of the Province of Ontario and the Federal laws of Canada as applicable. 48. Prizes, vouchers and coupons are non-transferable.
