3 Letter from the Dean Dear Community Partner: The University of Iowa College of Public Health and its Business Leadership Network (BLN) recognize the importance you place on community and public health in your workplaces, community spaces, and community collaborations. Community engagement is at the core of our mission. We are committed to encouraging and supporting community collaborations that enhance public health practice in communities of all sizes across Iowa. Through our College s Iowa Institute for Public Health Research and Policy, we are inaugurating the Business Leadership Network Community Grant Project to support projects, programs, and activities that build collaborations and support community wellbeing. Please consider submitting an application to the College to receive a small grant award that can serve as seed money or supplemental funds to address a community priority in public health. It is our hope to create a sustained interest in and understanding of the value of a collaborative investment in public health. The College of Public Health (CPH) and the Business Leadership Network have created the CPH/BLN Community Grant Project to make it easy for you to apply, and we are willing to answer any questions you may have. Please take a look at the materials, bring together some partners, and think about how this opportunity might support your community s public health initiatives. 4 Introduction and Eligibility for the Grant Project The University of Iowa (UI) College of Public Health (CPH) is offering small grants to be awarded on a competitive basis to applicants working in small and mid-sized communities that are located within the Business Leadership Network region. Under the auspices of the Iowa Institute for Public Health Research and Policy and in cooperation with the College s Business Leadership Network, the goal is to stimulate collaboration among nonprofit, private, and public entities and the College in identifying and addressing the public health priorities in their communities. Additional support from the UI Provost s Office of Outreach and Engagement has supplemented the CPH community grant program. The purpose of the CPH/BLN Community Grant is to foster collaboration in Iowa s smaller communities in addressing areas of identified community health or public health need. The intent is also to begin or strengthen partnerships with business and industry and to link with the University of Iowa College of Public Health experts and resources in appropriate topical issues. It is a priority to keep the application process simple and concise to encourage communities to submit a proposal. New and innovative approaches to addressing public health priorities in communities are welcome. Key areas of interest include but are not limited to healthy lifestyles, population health, data accumulation, workplace safety/wellness, community needs assessment, children and youth issues, healthy aging, arts and health collaborations, and sustainability of multi-generational, livable communities. For this initial pilot year of the Community Grant Project, the College of Public Health will work with the Business Leadership Network Steering Committee to identify and solicit proposals from communities in the BLN regions. Nonprofit organizations and local government entities are eligible to apply. Community collaboration and partnerships are required, and involvement of local business and industry and CPH partners is strongly encouraged. Organizations such as local community foundations and Resource Conservation and Development (RC&D) regions may be strong collaborators. Cash grant awards of up to $3,000 will be made to communities in the Business Leadership regions. Grants require a minimum 1:1 cash match. Over and above the cash match, in-kind match is also encouraged. Request for Proposals Community Grant Project at Page 4 of 10
5 Benefits of Partnering with the College of Public Health/Business Leadership Network At the University of Iowa College of Public Health we understand that our work is very place based; it happens out in the real world where people live and work. We also recognize the mutual benefits that might come about through collaborative relationships with communities and businesses around the state. The mission of the College is to promote health and prevent injury and illness through commitment to education and training, excellence in research, innovation in policy development, and devotion to public health practice. Our faculty, staff, students, and alumni, together with our Business Leadership Network Steering Committee and other partners at the local, state, national, and international levels, form an active learning community seeking to fulfill this mission. These Community Grants support the goals to bring together community partners, including business and industry, and connect CPH faculty, staff, and students to community initiatives. These could take the form of educational opportunities, community events, or evaluation of new initiatives. A grant could also lay the foundation for such opportunities as gaining additional support for community initiatives from other funding sources and/or showcasing innovation in the community. 6 Community Grant Project Timeline Issue Date September 15, 2015 Proposal Due Date November 2, 2015 Awards Announced December 15, 2015 Grant Project Timeframe January 1, 2016 December 31, 2016 Deliverables Due March 31, 2017 Expectations and Commitments GRANT REVIEW AND AWARD Up to six awards will be made to qualifying entities across the BLN regions shown in the map provided. A committee of representatives from the College of Public Health, the UI Provost s Office of Outreach and Community Engagement, and the Business Leadership Network Steering Committee will review proposals and make funding recommendations. The final decision on funding a proposal rests with the College of Public Health Dean and the CPH Executive Committee. The College of Public Health reserves the right to request more information from applicants at any time throughout the application review process. REPORTING TO CPH Organizations receiving awards will provide a simple quarterly report on progress to the CPH. In addition, a brief final report outlining achievements, outcomes, and options for links with CPH will be required. USE OF FUNDS AWARDED This is a one-time grant award. Grant funds must be expended within 12 months of the start date Grant funds may be used for local programs and activities that address a locallyidentified community health or public health need. This may begin a new initiative, supplement an existing project, or help sustain an existing project. Funds may not be used for personnel costs or infrastructure, such as trails or park benches. Cash match from local funding is required in at least a 1:1 cash match ratio from local private, public, or nonprofit partners and/or collaborators. Above and beyond the requirements for cash match, in-kind match is encouraged. Request for Proposals Community Grant Project at Page 6 of 10

9 Counties/Communities Served PROPOSAL NARRATIVE The proposal narrative is limited to six (6) pages, no less than 1-inch margins, single spaced, font no smaller than 11 pt. The narrative should include the following sections. Note that the Cover Page, Budget, and Collaborator Letters do not count toward the sixpage limit. 1. Abstract that briefly describes the project (1 paragraph) 2. Focus of proposed project, including why it is a community priority (1 page) 3. Plan of work and activities to be undertaken. This can be in paragraph or table form. (1.5 pages, including time frames.) 4. Brief description of your organization, its programs, and key person or people who will be leading the project (.5 pages) 5. Brief description of collaborations, including other organizations/entities in your community (including their organizational status nonprofit, public, private) and current or desired collaborations with the CPH (1 page) 6. Evaluation and outcomes, including up to 5 indicators of the success of the project (1 page) 7. Plans to sustain collaborations and/or initiatives after the end of the grant period. (1 page) BUDGET Include a budget and line-item narrative that includes funding from: 1. CPH cash award request 2. Local cash match of at least 1:1 ratio 3. Local in-kind match, if any Provide totals for each source of funding and the total project amount. Proposal Evaluation Criteria All proposals will be evaluated by a committee composed of Business Leadership Network Steering Committee Members and representatives from the College of Public Health and the UI Provost s Office of Outreach and Engagement. Recommendations of this evaluation committee will be forwarded to the Dean and the CPH Executive Committee for final determination.
