The presenting authr must be ne f the c-authrs listed n the submitted abstract. The presenting authr is respnsible fr the fllwing: Ntifying all c-authrs f any additins, deletins, and changes t the prgram, as may be cmmunicated by ACMG. Obtaining all f the cnflict-f-interest disclsure and cpyright transfer infrmatin frm c-authrs. Authrs Cnsent and Waiver f Claims Each abstract authr agrees and certifies that he r she: has read all f the rules and agrees t be bund by them, is respnsible fr submissin f the abstract in accrdance with the rules, and

3 waives any and all claims against ACMG and any reviewer arising ut f r relating t the abstract submissin and review prcess, including but nt limited t peer review and the grading f abstracts. General Guidelines The abstract must address scientific questins, detail clinical bservatins, r cntain primary scientific data. Authrs assign cpyright f the abstract t ACMG upn submissin, unless ne f the authrs is a U.S. Federal emplyee (in such case, ACMG des nt hld cpyright). This means that the identical abstract may nt be republished r submitted t anther meeting. All research and studies reprted in submitted abstracts that invlve human and animal subjects must cmply with the guiding principles fr experimental prcedures fund in the Declaratin f Helsinki f the Wrld Medical Assciatin. Interim analysis f a prspective randmized clinical trial will be cnsidered nly if it is perfrmed as planned in the riginal prtcl and is statistically valid. If yur abstract invlves interim analysis, use the Interim Analysis f a Clinical Trial sectin f the abstract frm t explain the details f yur study. The reviewers will have this infrmatin available during their evaluatin. N revisins can be made after the abstract submissin deadline (Nvember 20, 2015). The presentatin at the annual meeting must reflect the submitted abstract. In particular, the abstract title, authrship, and scientific cntent f the presentatin at the annual meeting must match the submitted abstract, althugh updates n results may be added. Abstracts shuld be written in clear and cncise English, s that reviewers are able t fcus slely n the scientific merits f the submissin. We encurage nn-english-speaking authrs t have their abstracts checked fr grammar and spelling prir t submissin. It is assumed that the presenting authr will have adequate cmmand f English t present and t respnd t questins. If that is nt the case, the authrs shuld chse a pster as their preference.

4 Preferred Presentatin Frmat During submissin, submitters must chse the frmats in which they wuld be willing t present/publish their abstract. These chices must be made at the time f submissin and cannt be changed after the review cmmittee has reached its decisin. First, authrs select whether they prefer ral r pster presentatin. Abstracts nted with a preference fr pster presentatin will NOT be cnsidered fr ral presentatin, regardless f scre. Secnd in the event that an abstract is nt chsen fr ral presentatin, it can still be chsen fr pster presentatin. Authrs wh state a preference fr ral presentatin must decide whether they wuld accept pster presentatin. Authrs may either present a pster r request that the abstract be immediately withdrawn. (Please nte that declining t have yur abstract cnsidered fr pster presentatin des nt imprve its chances f being accepted as an ral presentatin. This ptin shuld be selected primarily by authrs wh wuld nt be willing t present a pster.) Abstract Review and Selectin Prcess After the submissin deadline, all cmpleted and eligible abstracts will be made available t the ACMG Abstract Reviewers fr review and scring, and final decisins will be made by the Prgram Cmmittee in December, Abstracts will be evaluated and scred slely n their scientific merits. Incmplete abstracts will nt be reviewed. The same study must nt be submitted as multiple abstracts. Abstracts that are simply different versins f a single study will be rejected. Abstracts will be peer reviewed accrding t the subject categries. Authrs must indicate during nline submissin the apprpriate review categry (ne nly). All categry selectins will be final. There will be NO re-classificatin f abstracts after the abstract submissin site has been clsed. Abstracts submitted t the wrng categry are scred in that categry and usually fare prly.

5 All abstracts submitted will be cnsidered eligible fr ne presentatin: either plenary, ral (simultaneus sessin), r pster. Oral (simultaneus) Sessins are frmal ral presentatins fllwed by a brief discussin. Pster Sessins allw the viewing f a pster illustratin f the abstract. Authrs are expected t pst and remve psters at designated times and t be at their psters t answer questins during the time designated fr pster presentatins. The tp 10% rated abstracts selected fr Pster Presentatin will be highlighted in the Exhibit Hall and ACMG Prgram. Acceptance/Rejectin Ntificatin Ntificatin regarding acceptance r rejectin f abstracts will be sent t the presenting authr in early December 2015 by ; cnsequently, an accurate address is critical. If yur abstract is accepted, the will specify whether it is accepted as ral r pster presentatin. If yu have nt received an ntificatin by December 14, cntact Educatin@ACMG.net. Rejectin ntificatins will als be sent at that time. T ensure that yu are able t receive crrespndence frm ACMG, please make sure that yur sftware can receive mail frm the acmg.expplanner.cm and acmg.net dmains. If after cmpleting yur submissin yu dn't receive a cnfirmatin frm the abstract system, yu must cntact yur system administratr and make sure that bth dmains are added t yur address whitelist. The decisin f the ACMG Prgram Cmmittee regarding acceptance and presentatin f abstracts is final. Once yur abstract has been accepted, we are NOT able t renumber it t accmmdate yur schedule. S if yur abstract is designated as an dd number pster, we cannt renumber it as an even number t allw yu t present n a different day. Abstract Withdrawal Once an abstract is accepted, a written request t withdraw must be submitted n later than February 5, 2016, t Educatin@ACMG.net if the

6 first authr decides t withdraw the abstract fr any reasn. Abstract withdrawal requests received after the deadline will be cnsidered n a caseby-case basis. ACMG reserves the right t withdraw abstracts that are in vilatin f the Cllege s plicies and guidelines, have been deemed scientifically unsund, r have been fund t include inaccurate data, etc. Abstract Submissin Plicies 1. Cnflict-f-Interest Disclsure Plicy ACMG is cmmitted t ensuring the integrity f its scientific, educatinal, and research prgrams. The ACMG Cnflict-f-Interest Plicy requires disclsure f any financial r ther interest that might be cnstrued as resulting in an actual, ptential, r apparent cnflict. ACMG abides by rules frmulated by the Accreditatin Cuncil fr Cntinuing Medical Educatin (ACCME) that require that yu disclse any relevant financial relatinship yu r yur spuse/partner have had within the past 24 mnths. Fr this purpse, relevant financial relatinships are thse frm which yu have received r may receive financial benefit and which are related t the CME cntent. As a cntinuing medical educatin (CME) prvider accredited by the ACCME, ACMG must ensure balance, independence, bjectivity, and scientific rigr in all presentatins at the ACMG annual meeting. By cmpleting this sectin f the nline abstract submissin, yu agree that yu have read the ACMG Cnflict-f-Interest Plicy and that yu understand and supprt its intent. This plicy is nt intended t prevent a presentatin; it is merely intended t penly identify ptential cnflicts s that audience members may frm their wn judgments abut the presentatin with a full disclsure f the facts. Authr Respnsibility Regarding Cnflict-f-Interest Disclsure The presenting authr is respnsible fr btaining disclsure infrmatin frm all c-authrs.

7 All authrs and c-authrs are required t prvide any relevant infrmatin cncerning persnal r prfessinal circumstances and relatinships that might reasnably be expected t affect the audience s view n the presentatin. This includes relatinships with pharmaceutical cmpanies, bimedical device manufacturers, r ther cmpanies whse prducts r services are related t the subject matter f the presentatin tpic. If n relevant relatinships exist, this must be stated as well. What t Disclse Yu must disclse the relatinship and state the name f the cmpany fr each f the fllwing areas in which yu maintain a relatinship. Exact dllar amunts are nt necessary. 8 Off-Label Use Yu will be required t nte whether yur presentatin will include discussin f ff-label use f prducts. If s, yu must prvide a brief explanatin. Other Areas During the disclsure submissin prcess, yu will als be required t indicate yur cmpliance with the fllwing: If yu are prviding recmmendatins invlving clinical medicine, these recmmendatins will be based n evidence that is accepted within the prfessin f medicine as adequate justificatin fr their indicatin and cntraindicatins in the care f patients. All scientific research referred t in supprt f a patient-care recmmendatin will cnfrm t generally accepted standards f experimental design, data cllectin, and analysis. The cntent f the infrmatin with which yu are invlved will prmte quality in health care r advances in science and will nt prmte a specific prprietary r cmmercial interest. Cntent fr this publicatin will be well-balanced and unbiased. If yu have been trained r utilized by a cmmercial entity r its agents as a speaker (e.g., participatin in a speaker s bureau) fr any cmmercial interest, the prmtinal aspects f that wrk must nt be included in the presentatin in any way. Activities that fall utside the definitin f CME/CE; Educatinal activities that serve t maintain, develp, r increase the knwledge, skills, and prfessinal perfrmance and relatinships that a physician uses t prvide services fr patients, the public, r the prfessin (surce: ACCME and AMA) will nt be certified fr credit. CME activities that prmte recmmendatins, treatment, r manners f practicing medicine r pharmacy that are nt within the definitin f CME/CE r, are knwn t have risks r dangers that utweigh the benefits r, are knwn t be ineffective in the treatment f patients. 3. Presentatins and CME/CE activity materials must give a balanced view f therapeutic ptins; use f generic names will cntribute t this impartiality. 10 Hw t Submit an Abstract The abstract submissin site pens September, All abstracts must be submitted by Friday, Nvember 20, 2015, 11:59 pm PST. Submissins that are incmplete by the deadline will be rejected. Abstracts must be submitted nline thrugh the fficial nline abstract submissin system. s and wrd prcessing files nt submitted thrugh the site will nt be accepted. Once yu have submitted the title page infrmatin, a draft f yur abstract will be saved, and yu will be able t return t edit and update it at any time until Nvember 20, 2015, at 11:59 p.m., PST. Yu will receive an prviding a link t yur submissin. Any technical questins regarding the submissin prcess shuld be directed t acmgcfp@tmiexps.cm, (9-5 EST Mnday t Friday). Preparing an Abstract fr Submissin Cntact Infrmatin Yur name, degree, institutin, address, phne number, and address must be prvided. As the crrespnding authr, yu will receive all future crrespndence frm ACMG. The crrespnding authr shuld be the first authr (presenter) f the abstract, unless therwise nted during submissin. C-Authrs Names f c-authrs and institutins must be prvided. Changes will nt be made t the spelling f authrs names after the submissin deadline; please prf yur c-authrs names carefully.

11 Cpyright Plicy All authrs must assign cpyright f the abstract t ACMG, unless ne f the authrs is a U.S. Federal emplyee (in such case, ACMG des nt hld cpyright). Abstract Title The abstract title shuld be brief and clearly indicate the nature f the abstract. The abstract title must be in title case. Capitalize all nuns, prnuns, adjectives, verbs, adverbs, and subrdinate cnjunctins (i.e., as, because, althugh). Except fr the first wrd f the title, lwercase all articles, crdinate cnjunctins (i.e., and, r, nr), and prepsitins, regardless f length. Als, lwercase t when used as an infinitive. Additinally, keep letters lwercase if the lwercase letters have a specific meaning, such as acgh r NIPT. D nt put a perid at the end f the title. Fr example: Cancer Genetics: Translatin f New Cncepts t Clinical Care Use f Prduct Names Nn-prprietary (generic/scientific) names shuld be used and shuld be lwercase. If necessary, yu may include a prprietary name in parentheses directly fllwing the generic name after its first mentin in the bdy f the abstract; the first letter f the name f a prprietary drug shuld be capitalized. ACMG reserves the right t replace prprietary names with generic names t adhere t this plicy. Abbreviatins Use standard abbreviatins. Place abbreviatins in parentheses immediately after the first mentin f a term r phrase; the abbreviatin can then be used thrughut the abstract.

12 Abstract Bdy, Tables, and Figures Abstracts submitted fr the ACMG annual meeting are published nline. It is the respnsibility f the authr t prfread the abstract carefully. Yu are alltted 4,200 characters (including spaces) fr yur abstract. The prgram will nt allw submissin if the maximum number is exceeded. If yu cut and paste frm a wrd dcument, use the Special Characters r Symbls menu t enter Greek letters, symbls, and fr inserting HTML cde such as bld, italic, super and subscripted symbls. Please nte: scrll t the bttm f the special characters menu t lcate HTML cde such as bld, italic, super and subscripted symbls. The abstract may be structured (i.e., abstracts divided int sectins using terms such as Intrductin, Methds, Results, Cnclusins, etc.) r unstructured. D nt use bld type r underline frmatting. Italic type is acceptable. Text may be in multiple paragraphs. Special Greek and mathematical symbls are available in a character map within the submissin system. Use numerals t indicate numbers, except when beginning sentences. Simple tables and graphs may be included. The nline abstract submissin system prvides step-by-step instructins n inserting tables and graphs in the preferred HTML frmat. Any references shuld be nted as citatins within the text and nt as ftntes at the end.

Hw t be cnsidered fr the PerkinElmer Diagnstics Cmpany Travel Award: All geneticists-in-training, wh are bth first authr and presenter f the abstract, are autmatically cnsidered fr an Abstract Award. Therefre, yu must first identify as a geneticist in-training and then select the apprpriate training categry (undergraduate student, medical student, graduate student, resident physician, r pst-dctral fellw) in the nline abstract submissin system. As a geneticist-in-training, yu are required t submit a letter frm the senir investigatr verseeing yur research cnfirming yur status as a geneticist-in-training. The ACMG Fundatin fr Genetic and Genmic Medicine will be giving the Carlyn Mills Lvell Genetic Cunselr s Award t the first authr f the highest rated genetic cunselr-authred abstract submissin fr the scientific prgram during the 2016 ACMG Annual Clinical Genetics Meeting. Applicants must be certified r bard-eligible Genetic Cunselrs r genetic cunseling graduate students enrlled in an accredited genetic cunseling training prgram and d nt have t be an ACMG member. Eligible candidates interested in entering the cmpetitin will indicate s during the abstract submissin prcess. The Carlyn Mills Lvell Genetic Cunselr s Award will be presented t the recipient at the 2016 ACMG Annual Clinical Genetics Meeting. In additin, the recipient will be identified with a paragraph abut the Award in the nsite Prgram and Abstract Bk and in a press release. In recgnitin f their selected presentatin, a $1000 award will be given t the recipient t attend the meeting. Electrnic Signature Cmpletin f all required disclsure infrmatin in the nline abstract submissin system serves as an agreement and is accepted in lieu f a faxed signature.
