3 3 I. INTRODUCTION A. The Project Humboldt State University seeks proposals from qualified firms to provide electrical engineering services for a temporary cluster of modular buildings at the Campus Events Field. The modular buildings will provide surge space for relocating programs impacted by the Library and Theater Arts seismic retrofit projects. II. PROJECT INFORMATION A. Project Description The project includes electrical and low voltage data and fire alarm infrastructure to support the University in the lease, set up, connections, and eventual removal of multiple relocatable modular buildings and associated foundations, skirting, and accessible ramp systems on the Campus Events Field at Humboldt State University. The project, as currently envisioned, will include a total of 14 relocatable buildings, comprised of (10) 24 x40 Classrooms, (3) 36 x40 Classrooms, and (1) 12 x40 Bathroom building. All buildings are to be in accordance with DSA standards, but the project Construction Documents do not require DSA plan review and approval as normal for K-12 & Community Colleges. DSA will provide only a limited review of accessibility issues for compliance with CBC Chapter 11B. All relocatable buildings are expected to be installed in May 2018 and removed in September The electrical infrastructure, including any transformers and/or main service panels may be considered permanent and left in-place after the removal of the modular buildings. However, the actual final condition is to be determined as part of the design process. B. Scope of Electrical Engineer s Services Refer to Attachment 3, Exhibit A: Scope of Services for Electrical Engineer for a description of the requested services. C. Project Schedule January 4, 2018, Thursday, 10:00am: Pre-Proposal Meeting (Optional) January 4, 2018, Thursday, 5:00pm: Last Day for Written Questions to be Received January 11, 2018, Thursday, 3:00pm: PROPOSALS DUE, by January 12, 2018, Friday: Letter of Intent to Award Issued, Services Begin January 26, 2018, Friday: Preliminary Design Complete February 9, 2018, Friday: 50% Construction Documents Complete February 23, 2018, Friday: 95% Construction Documents Submit for Plan Check March 2018, Respond to Plan Check Comments, Obtain Approvals & Permit April & May 2018, Construction Phase Services Installation September 2019, Construction Phase Services Removal

Codes, Regulations & Requirements The following codes and regulatory requirements are applicable to this project: 2016 California Building Standards Code, Title 24 as adopted by the California Building Standards Commission, including all supplements and errata as effective on the date of submittal for plan check review Public Contract Code, Sections et seq. (CSU Contract Law) California Code of Regulations, Title 8 (CAL/OSHA Standards) California Code of Regulations, Title 19 (Public Safety) California Environmental Quality Act (CEQA) All other applicable codes, regulations and standards. E. Public & Regulatory Agencies The following regulatory agencies have jurisdiction: Humboldt State University: Building Official State Fire Marshal: Fire / Life Safety Authority Division of the State Architect: Accessibility Compliance F. California State University Guidelines & Reference Standards The following guidelines and reference standards as adopted by the California State University System shall be referenced and utilized for design of the project: CSU Seismic Requirements, Nov. 1, o CSU Telecommunications Infrastructure Planning (TIPs) Standards, Feb o Edition pdf CSU Mechanical Review Board (MRB) Peer Review. III. SCOPE EXCLUSIONS A. Exclusions The scope of services is limited to that described in the request for proposal and enclosed exhibits. Any requests for expansion of the scope of work requested shall be directed to the University s design manager. Any changes to the scope of work requested shall be approved, in writing, by the University prior to execution of such work. IV. FEES & CONTRACT A. Contract Documents The contract utilized for this project is the California State University Service Agreement (Attachment 1) which includes the following documents: Rider A Agreement General Provisions, (Attachment 2), Exhibit B Fee Proposal Form (Attachment 3), and Exhibit C Service Provider Hourly Rate Schedule. The California State University System Office of General Counsel prevents the Service Agreement and Rider A from being modified.

Fees Firms shall propose all-inclusive lump sum fees, to be paid monthly, in arrears, based on percent complete rather than hours expended for each item listed in the Fee Proposal Form (Attachment 3). Invoices shall be submitted to the University s designated design manager utilizing a format approved by the University and which properly reflects the fee proposal sub items associated with the milestone tasks required for the scope of services requested. V. PROPOSAL REQUIREMENTS To be responsive, a Proposal must be submitted in the format identified below. Responses must be clearly identified for each of the requirements listed below. The proposal shall generally adhere to the following format for organization and content. Provide one copy of Request for Proposal documents in digital format via . The following information shall be included in this order: Cover Letter: Provide a brief cover letter that references the RFP and confirms understanding of project. Include explicit acknowledgement of addenda, if any. o Include the firm s legal name and contact information for single point of contact assigned responsibility for this project. o Provide name, title, and signature (electronic signature accepted) of the corporate officer authorized to bind the firm contractually. o Indicate the name and CA Electrical Engineer license number for the proposed electrical engineer of record. o Indicate the ability of your firm to execute the required CSU contract documents including all insurance requirements. o Provide a project schedule that is based on the one included with this RFP. Identify strategies for meeting these milestone dates and delivering a successful project in an efficient and professional manner. Qualifications: Provide a brief description of the firm s qualifications and experience relevant to the proposed project. Extensive documentation and entire portfolio is not required. Contact information for relevant professional references may be included. Fee Proposal: Provide fixed fee proposal using the attached Fee Proposal Form, Exhibit B, (Attachment 3). The fee proposal shall be inclusive of any costs that might otherwise be considered reimbursable expenses. Any and all travel, mileage, accommodations, reproductions, and delivery shall all be included in the fee proposal. Any future claims for reimbursable expenses will be rejected except if by explicit written prior approval by the University. Schedule of Hourly Rates: Provide breakdown of hourly rates by employee classification, including terms that would be used to determine fees if additional services are requested. This will become Exhibit C as part of the Service Agreement.
6 6 VI. SELECTION PROCESS The University will review responsive proposals based on qualifications, relevant experience, project approach, ability to meet timeline, and proposed fee. Once the firm is selected, the University will begin the process of awarding the contract and issue a Notice to Proceed. VII. PROPOSAL LOGISTICS A. Both the question and the answer to all substantive inquiries will be posted to the University s Contracts & Procurement website B. Optional Pre-Proposal Meeting All firms intending to submit proposals are invited to attend an on-campus pre-proposal meeting. This meeting is optional and extended for the benefit of proposers only; attendance will not impact the University s judgement regarding the qualifications of a proposing team. University staff will present overall scope of services, answer questions and lead a comprehensive tour of the existing conditions. The optional pre-proposal meeting will be held at 10:00 a.m., Thursday, January 4, 2018 at the Facilities Management conference room located northwest of the intersection of 14 th and B Streets and on the Humboldt State University campus in Arcata, California.
