2 THE COMMUNITY FOUNDATION SAN LUIS OBISPO COUNTY Opportunity to Thrive Initiative Grant Guidelines and Application Procedures Mission Statement The Community Foundation San Luis Obispo County provides leadership and encourages charitable giving for the issues that improve and enhance lives in the county. History Since 2008, the Community Foundation San Luis Obispo County has committed to supporting programs which help families and individuals move towards self-sufficiency. Through the Community Resource Center Initiative, the network of local family resource centers have been strengthened, and provided resources to help their clients with direct assistance to overcome barriers to family stability. Through the Dunin Opportunity Fund, the Community Foundation has provided resources to support individuals striving for self-sufficiency and who are in need of transitional assistance to overcome a short-term or one-time barrier. These funding opportunities have been consolidated into one initiative, Opportunity to Thrive. The intent of this grant program is to provide funds that may be used as direct assistance for families and individuals in transition and who are working with human service providers or family resource centers in a case management relationship. One-year grants will be provided to local nonprofit organizations which currently work with populations striving for stability and self-sufficiency and have the capacity to assess and administer an assistance fund. Core funding for this initiative is from the Helen & Ronald Dunin Memorial Endowment. Through a testamentary gift, former Mayor of the City of San Luis Obispo, Ron Dunin, and his wife Helen established an endowment to help people who are attempting to help themselves. As immigrants, the Dunins were fortunate to receive help and support during critical times in their lives. Their goal by creating the fund was to give back to the community and assist San Luis Obispo county residents in need. General Information for Applicants The Community Foundation San Luis Obispo County supports charitable organizations whose programs primarily benefit San Luis Obispo County residents. An organization must have a current tax-exempt status from the Internal Revenue Service under Section 501(c)(3) to apply. The Community Foundation San Luis Obispo County will follow these basic procedures in relations to grant requests: The Community Foundation San Luis Obispo County will screen and fairly review all completed grant applications. The Community Foundation San Luis Obispo County will notify all agencies of the award decision in a timely manner. Exclusions In general, The Community Foundation San Luis Obispo County does not make grants to/for: Organizations that discriminate on the basis of age, disability, ethnic origin, gender, sexual orientation, gender identity, race or religion. Individuals. Fraternal Organizations, unless in support of a specific program open to or benefiting the entire community. Governmental Organizations Indirect Administrative costs greater than 15% of the awarded funds Fundraising events such as annual campaigns, walk-a-thons, tournaments, fashion shows, auctions or dinners. Organizations with religious affiliations unless the program is open to the entire community without regard to religious beliefs. Organizations and programs designed to elect candidates to public office. 2
3 Organizations located outside San Luis Obispo County unless for a specific program benefiting residents of-and conducted within-san Luis Obispo County. Pay off past debts or existing obligations. Endowment funds. Scholarships, fellowships, travel grants and technical or specialized research. Funding Criteria & Priorities It is the intent of the Foundation to evaluate proposals and applicant organizations based on the following: Funding Criteria: Organizations with regular/ongoing contact with target populations as part of usual program work, utilizing a Family Advocate or case manager to develop and track a client s self-sufficiency plan Demonstrated ability to administer and monitor a hardship assistance fund Ability of organization to maximize amount of grant to be used in direct assistance (low indirect administration rate), and complement, not supplant, existing services Organization is able to leverage existing infrastructure and capacity to administer the program It is the intent of the Foundation that the Opportunity to Thrive funds will be used as follows: Funding Priorities: Families/individuals in transition, e.g. low-income families in need of multiple social services, foster youth exiting the system, women leaving domestic violence shelters, families at risk of homelessness, individuals attempting re-entry into the workforce. One-time assistance that will make a big difference in removing a barrier or hurdle to self-sufficiency and stability, targeted to individuals who have demonstrated their attempt to help themselves One-time assistance provided for a leg up, rather than short-term impact or ongoing needs Assistance for as many individuals/families as possible while providing sufficient support to overcome the barrier Assistance which fills a gap in the support network and does not supplant other existing funding Examples of assistance include, but not limited to: security deposit for rental unit, uniform purchase or job appropriate apparel, transportation assistance, car repair, tuition, training fees or materials, equipment or tools, youth development program participation, legal consultation, assistive device for recently disabled individuals, prescription medication, credit counseling, tattoo removal, goods/furniture to set up household, preventing utility shutoff. Funds are not intended for ongoing living expenses such as food or housing. The amount of assistance to any one individual or family is not capped, but the intent is to serve as many as possible through the fund Funding and Review Cycle The Community Foundation San Luis Obispo County Grants Committee will consider grant requests once in The maximum grant award allowed will be $10,000. An optional applicant workshop will be held on Wednesday, May 25, 2016 at 3:30 pm at the Community Foundation office, 550 Dana Street, San Luis Obispo. RSVP to Deadline for submission of full proposal is July 15, 2016, 5 pm. Notification and awards will be made on or about September 15, 2016 Follow-up and Reporting Requirements Successful grantees will be required to provide detailed program and client outcome reports as to the use of the grant award. Copies of press releases and/or media contacts resulting from the grant award shall be included. A financial report outlining grant expenditures is required. 4 THE COMMUNITY FOUNDATION SAN LUIS OBISPO COUNTY Applying for an Opportunity to Thrive Grant Narrative maximum 4 pages 1. Please provide a brief statement on how the grant funds will be used. Include program name, amount requested, target population, region served, estimated # of clients to be assisted, and intended grant outcomes. 2. Information on your request, including: a) Provide one representative case history of your organization clientele describing client need and how one-time assistance would complement existing services and make a big difference in removing a barrier or hurdle to self-sufficiency, stability or permanency. b) Describe your client population: demographics, socio-economic status, issues and needs. c) Describe your organization s method for client intake, assessment, transition/intervention planning, and average length of time on case load. (Include sample forms and/or assessment and planning instruments as appendices.) d) Describe your organization s competency with the target population. Who will be responsible for administering the Opportunity to Thrive Fund grant and what are their qualifications? e) Describe existing aid program, if any, how aid level is determined and monitoring of aid distribution and utilization occurs. Provide one example each of something that would be covered by this fund and something that would not be covered. Include estimated average and maximum amount of funding assistance per client. f) Describe existing, if any, resources your organization has to provide assistance outside of the usual scope of services. This may include cooperative relationships with businesses, nonprofit organizations, or grants/contracts. g) Describe program evaluation methods and client outcome measurements. 3. Please provide information about your organization(s), including: a) Mission Statement and Core Values, brief history, programs, and organizational structure. List of Major Contributors Please include an original and five copies of the following appendices 3. Client intake/assessment and planning tools 4. Client progress/tracking and/or outcome assessment tool 5. Line-item budget for grant requested, including any in-kind contributions, leveraged or matching funds (see sample format) 6. Fiscal Year 2015 financial statement (P&L Statement & Balance Sheet) 7. Fiscal Year 2016 organizational operating budget, including all sources of funding Application Due Submit an original and five (5) copies of your application along with the appropriate required appendices.
