1 City of London Affiliate Program Background The City of London Affiliate Program allows non-profit minor, and non-profit adult sports groups to register with the City to receive reduced fees and priority bookings. Objectives / Guiding Principles Non-profit minor and adult sports groups are the primary providers of organized sport in the City of London. The City of London along with other Municipalities across Ontario and Canada, recognize that affordable access to recreation facilities is an important factor in ensuring maximum participation in sport and other programs that promote physical activity and a healthy lifestyle. Benefits Sport organizations that receive affiliate status will have the opportunity to book sport facilities (ice, fields, and diamonds) prior to non-affiliate groups or commercial bookings. Sport groups with affiliate status will also receive the reduced affiliate rental rate. Organizations that are Eligible for the Affiliate Status Affiliate status is available to non-profit, volunteer-based organizations whose membership is made up primarily of London residents. Eligible organizations must be guided by a constitution and bylaws. Furthermore, organizations will have an elected board of directors who serve without remuneration, except for reasonable expenses incurred in the carrying out of their duty as Director. Organizations that are Not-Eligible for the Affiliate Status Affiliation status is not available to: Organizations whose primary interest is in promoting professional association or business opportunities. Organizations whose main purpose if to further the aims of a religious organization or to promote religious doctrine. For-Profit Organizations National or Provincial organizations. Social service organizations.

5 Application Checklist Please review this check list prior to submitting your application. The following items that support you application must be submitted with this application form. Minutes of the most recent general membership meeting Financial reports, including income statements and balance sheets Registration lists from the previous season. The registration list must contain: o Age o Postal code o Gender A copy of your Not For Profit Certificate or your registered charitable number Has your organization submitted an application to the City of London Affiliation program in the last 2 years? If yes, please contact Vicki Cloes at ext.2586 for a revised checklist as our files may already contain some of the items required. * Each organization will be required to have all their members sign a waiver (at the time of the organizations registration), which allows the organization to release their participant s personal information to the City of London.
