1 Let us help you achieve or maintain your highest recognition level GVF works year-round with its partners to develop programs and incorporate TDM and/or sustainability initiatives into their work environment through transportation demand management (TDM). We want to help you be recognized for the investment you are making, and work with you to develop a TDM plan that will track measurable data throughout the year. Complete the application and be recognized at the Bronze, Silver, Gold, Platinum or Diamond level at our TDM Breakfast on September 10, A response to each question is required, and some questions require you to submit additional materials. Each question is required, and in some cases, applicants must submit additional materials. Follow the guidelines provided below when making your selection: HAVE: Check the programs and/or initiatives already implemented or to be completed by December For each selection, you will receive a credit. UNDERWAY: Check the programs and/or initiatives currently under construction or in the process of being implemented. These initiatives will not receive a credit but will be used for the development of a customized GVF TDM Plan. WANT: Check the programs and/or initiatives you would like to work on to improve your recognition level. UNSURE: Check the programs and/or initiatives that you would like more information about and how to proceed. NOT INTERESTED/APPLICABLE: Check the programs and/or initiatives that are of no interest or applicable. 1

Collaborate with GVF to present to your staff and/or elected officials on the importance of considering TDM strategies when reviewing proposals, considering improvements, updating comprehensive plans and/or zoning * 17. Develop mode shift goals from single occupancy vehicles (SOV) to alternative modes of transportation What is your mode shift goal (% of change from SOV)? 7

Conduct resident/employee surveys on commuting patterns and behaviors * 23. Implement a comprehensive plan that incorporates TDM initiatives * 24. Offer commuter benefit program to employees * 25. Organize a commuter challenge for employees and/or residents 9

10 * 26. Partner with third party technology organization to display real-time commuter information, such as transit schedules, next Uber pick-up, bike share * 27. Promote or participate in Safe Routes to School program * 28. Receive a Best Workplaces for Commuters designation * 29. Receive a Bicycle Friendly Community designation 10

11 Platinum Level Agency must perform any SIX (6) of the Bronze Level activities, any THREE (3) of the Silver Level activities, TWO (2) of the Gold Level activities, and TWO (2) of the following activities * 30. Complete sidewalk inventory * 31. Develop of a complete streets ordinance and/or plan * 32. Implement a sustainability plan that is inclusive of transportation related initiatives, including bicycling and pedestrian programs and/or infrastructure * 33. Instead of free parking, offer the pre-tax parking benefit for paid parking as allowed by the IRS * 34. Meet or exceed mode shift goals identified in Silver Level within one year Please provide the % of SOV trips shifted to alternative mode(s) of transportation 11

Offer maximum $255 per month pre-tax transportation benefit and an employer contribution of at least $30 per month, as defined by IRS * 45. Provide a municipal sponsored shuttle for residents * 46. Provide additional trail/trails connections * 47.
