1 1. Overview and Instructions Congratulations on being an official participant in the Maine Semester of Youth Service! With acceptance comes a cash award of up to $300 to help youth carry out their community service project. This form has multiple purposes: 1) describe what specific community service activity the youth will carry out to address the issue/problem they selected for action; 2) submit a budget for the cash award and commit to matching it with local resources (see special note below); 3) outline the communications and outreach plan that will result in community awareness of this service by youth and participation by local leaders. REMINDER: Projects must be completed by March 20, 201 in order for youth to participate in the state recognition at the Portland Pirates Hockey game on April 3, 201. (We need 2 weeks to get you the tickets and work with the hockey team staff on listing your accomplishment during game day announcements.) Completion report will be due 3/23/201. In addition, youth groups are required to prepare and participate in the virtual Semester of Service Celebration on April 13 where youth groups will present a report of their project and its results. FUNDING and SPONSORS: Funds for the project awards are from the Maine Commission for Community Service, Youth Service America, and State Farm Insurance. The statewide recognition event is a joint project of Maine AmeriCorps Alums and the Maine Commission for Community Service. BUDGET AWARD NOTE: You must include the project's anticipated expenses in the budget form at the end of this application. The award is for up to $300 and requires a $1:$1 match of in kind or cash. The grant money may be used for any project related planning or implementing costs, but may not be used to purchase food or gifts (including volunteer or participant recognition). Likewise you may not include food or gifts as part of your match. We recommend that you answer all the narrative questions in a word document and then cut/paste them into this form. Community Service Project Proposed Review the information and description of need submitted in your registration form. Describe the planned action the students or youth group will carry out to meet the need identified in the registration. Please note, the need and activity must be linked and have a natural/logical alignment.

(What impact or change do they expect to achieve?) 3 Service Action Plan 1) Describe who will benefit from the service and how many 2) List the milestones and completion dates related to organizing and carrying out this service. (Sample entry: "By 2/28/201, confirm availability of site.") Your description should include a. Any research or learning activities b. Meetings with community partners c. Roles for community partners, students or youth not involved in the planning (e.g., students recruited to help on the day of), and roles for community leaders asked to serve alongside the young people. 3. Local Leaders and Media How will your group include local and state leaders (school board members, state representatives, etc). in your project? Note: The experience of youth and local/state leaders serving together is more powerful than just having the dignitaries speak at start/end points. Be sure the youth have a chance to learn what these people are responsible for doing. This can be accomplished by simply giving the adults conversation prompts. What is the media outreach plan? How will the community be made aware of the goals and actions youth have undertaken as well as the results?

4 4. Budget Please document your anticipated expenses in this budget form. Remember the award is for $300 and requires a $1:$1 match of in kind or cash. At this stage, the donor or provider of in kind does not have to be named but you should indicate the materials or service you anticipate will be donated and its value. Remember that you cannot use the grant award for food items or gifts, including volunteer or participant recognition. 6 If awarded, the recipient of this grant agrees to 1. Provide MCCS requested data and narrative information on the service project and participants no later than Monday, March 23, Collaborate with MCCS to promote any MSYS related events to the local and regional media 3. Provide digital photos or videos and a "photo use consent form" covering the youth participating in MSYS related events. 4. Ensure youth participation in the virtual April 13 reflection and celebration event. This is no fee and uses internet as well as toll free teleconferencing. Please note, the State of Maine requires the Commission execute a "small contract" to release the funds.
