1 SIERRA HEALTH FOUNDATION // CLASS XV // FALL 2018 Sierra Health Foundation s Health Leadership Program is delivered by the Institute for Population Health Improvement at UC Davis in partnership with Richard F. Callahan, DPA.

2 EFFECTIVE LEADERSHIP FOR NORTHERN CALIFORNIA AND THE SAN JOAQUIN VALLEY Sierra Health Foundation is a private philanthropy that forges new paths to promote health and racial equity in partnership with communities, organizations and leaders. We are committed to reducing health disparities through convening, educating and strategic grantmaking. Sierra Health Foundation s Health Leadership Program strengthens the skills of leaders in nonprofit organizations and public agencies whose mission is dedicated to improving health and quality of life in their communities. The foundation s years of experience have shown that nonprofits and public agencies not only need strong, dedicated, skilled leaders, but also strong, effective and sustainable organizations to achieve their maximum impact in addressing health disparities and promoting equity. Since 2001, 363 alumni fellows, who are nonprofit and public agency leaders, have gone through the Health Leadership Program, developing their skills to lead their organizations and improve community health and well-being. We re pleased to continue the program in 2018 and invite current and emerging leaders to apply. Successful applicants will be in organizations that work to improve community health and well-being and reduce health disparities across a broad range of issue areas that address the social determinants of health, such as educational equity, employment and economic development, housing, youth development, juvenile justice, clean air, water, land use planning and other environmental justice issues, immigrant and refugee services, access to health services, population health, social services, and drug and alcohol dependency. The 2018 program will continue to focus on improved organizational outcomes and impact, through effectiveness, quality, financial stability, partnership development, public policy, governance and sustainability. The Health Leadership Program is led by the Institute for Population Health Improvement at UC Davis, and is co-directed by Kenneth W. Kizer, MD, MPH, and Richard F. Callahan, DPA.

3 ABOUT THE PROGRAM The Health Leadership Program is designed for up to 25 current and emerging leaders with 12 days of in-class leadership development activities and discussions addressing health and racial disparities through building organizational capacity and addressing inequities through policy and systems change. Class fellows identify the core values and leadership strengths that drive their personal approach to work and working with others. Throughout the program, they work and learn with other organizational leaders, nationally respected leadership trainers and faculty to develop innovative approaches to identifying and tackling complex social challenges, and learn how to coach and engage staff and teams, as well as lead and manage change processes. The program is designed to help leaders strengthen organizational impact and sustainability through dynamic classroom activity and participatory sessions, including experiential learning, presentations with cutting-edge leadership practices, peer-to-peer exchange, connections to program alumni and an individual leadership project, with executive coaching and mentoring support in each session. A web-based platform is used to create a community of sustained interaction between sessions. Class fellows also have the opportunity to develop a valuable, supportive network of nonprofit and government leaders, not only within their class but through membership in the alumni group. Faculty members are an ongoing resource and are available to class members and alumni members for one-on-one consultation. Sierra Health Foundation is a private philanthropy with a mission to invest in and serve as a catalyst for ideas, partnerships and programs that improve health and quality of life in Northern California and the San Joaquin Valley. The foundation is committed to improving health outcomes and reducing health disparities in the region through convening, educating and strategic grantmaking. 4 PROGRAM CURRICULUM Curriculum topics include: Increasing Health Access and Promoting Health Equity Strategy Development Leading Organizational Change Addressing Health Disparities The Connectedness of Race, Health and Systems Getting Traction on Social Change Implicit Bias Receiving Executive Coaching and Mentoring Building Organizational Capacity and Sustainability Addressing Social Determinants of Health 360-Degree Leadership Assessments Connecting with Diverse Communities Organizational Growth for Community Change Aligning Financial Imperatives and Mission Strengths-Based Leadership Strategies for Improving Population Health PROGRAM SCHEDULE The program consists of six modules over seven months. All sessions take place at Sierra Health Foundation at 1321 Garden Highway in Sacramento. Sessions begin with breakfast at 7:30 a.m. or 8:30 a.m. and end after dinner at 6 p.m. Attendance at dinners is required (not provided the final day of each module). 6 WHO SHOULD APPLY? The Health Leadership Program is designed for current and emerging leaders in nonprofit organizations and public agencies whose mission is dedicated to improving community health and well-being and reducing health and racial disparities across a broad range of issue areas that address the social determinants of health, such as educational equity, employment and economic development, housing, youth development, juvenile justice, clean air, water, land use planning and other environmental justice issues, immigrant and refugee services, access to health services, population health, social services, and drug and alcohol dependency. Sierra Health Foundation defines health broadly and generally considers most human-service agencies and policy/advocacy organizations addressing the social determinants of health as health-related. Preferred participants will be in management roles and have a strong commitment to and involvement with their local community. Because leaders lead others in their organizations, and we believe the best learning comes from practice, applicants must supervise or have experience supervising at least three individuals. While more than one leader from an organization may apply, we prefer to include only one from an organization in any current class. SELECTION CRITERIA Participants selected will demonstrate a strong commitment to their organization s mission and the capacity to become vital leaders of their organizations and their communities. Other factors considered include commitment to course requirements, experience and career aspirations, adaptability to change, receptivity to learning, ability and willingness to apply lessons learned, demonstrated ability to work collaboratively, the support of the participant s organization during the learning process and continued service in Northern California or the San Joaquin Valley. Ideally, the class will reflect diversity in gender, age, ethnicity, geographic service area, experience level and organization type. Class size is up to 25 participants. TUITION, FEES AND EXPENSES Sierra Health Foundation will pay tuition costs and most related expenses associated with participation in the program, including lodging and most meals. Participants are expected to pay their own incidental costs. Hotel accommodations are provided for participants living beyond a reasonable commuting distance. Program participants of the same gender share two-bedroom, two-bathroom suites located one mile from Sierra Health Foundation.

Those eligible counties are: Alpine, Fresno Kings Participation in the Health Leadership Amador, Butte, Calaveras, Colusa, Tulare Kern El Dorado, Fresno, Glenn, Kern, Kings, Lassen, Madera, Mariposa, Merced, Modoc, Mono, Nevada, Placer, Plumas, Sacramento, San Joaquin, Shasta, Sierra, Siskiyou, Solano (eastern), Stanislaus, Sutter, Tehama, Trinity, Tulare, Tuolumne, Yolo and Yuba. EXPECTATIONS OF PARTICIPANTS Participants in the program will commit to: Attendance and active participation in all sessions, including meals Having the support of their organization s leadership, for example, the executive director, supervisor or board chair, as applicable, to attend all sessions and apply new practices Active participation in online learning communities Individual study and application of practices from the program Participation in one or more one-on-one, executive coaching meetings Participation in the leadership engagement component of the program Participation in follow-up evaluation of the program Affiliation with the program through follow-up network activities CONTINUING CONNECTION Health Leadership Program Fellows have the opportunity to stay connected with their peers and enhance the principles and lessons presented during the course by participating in the Health Leadership alumni group. The group meets at least once a year to network and continue leadership learning with program faculty and other respected presenters.

8 APPLICATION PROCESS We encourage you to submit your application before the deadline date in case you need help with any of the guidelines below. Applications are due no later than noon on April 16, Submit the application via our online grants portal New users of the portal will need to create an account by clicking on the New Applicant button in the portal as the first step in the application process. Use this link any time you return to your in-progress application.
