7 Community Benefit. Working for you. As a not-for-profit health system, Sutter Health is dedicated in reinvesting in the communities we serve. By assessing needs through coordinated and collaborative Community Health Needs Assessments (CHNAs) in counties within primary service areas, Sutter Health hospitals create implementation plans to strategically address the unmet needs of residents. Then through Community Award Investments and other collaborative efforts, plans are put into action. Watch our investments at work. 7

Offering sponsorship awards, allows us to support the great work that community based organizations provide everyday and gives Sutter Health the opportunity to expand its mission of enhancing the well-being of people in the communities it serves. Sponsorship Application Timeline Winter - For events happening January May 2018 August 1, 2017: Mandatory Webinar available August 1, September 29, 2017: Proposals due October 2, December 29, 2017: Awardees announced January July 2018: Awardees report measurable outcomes Spring - For events happening June December 2018 January 15, 2018: Mandatory Webinar available January 15, March 9, 2018: Proposals due March 12, May 31, 2018: Awardees announced June February 2019: Awardees report measurable outcomes

9 Sponsorship Guidelines As you can imagine Sutter Health receives hundreds of requests for funding, and although all good efforts are worthy of support, we unfortunately cannot fund all requests. Therefore, the following criteria and restrictions have been established to help us narrow the focus to provide the greatest amount of impact on areas of priority as identified through needs assessments, as well as help us fulfill mandatory reporting requirements. Applicant Criteria - Only 501c3 organizations may apply - Only online applications accepted - Agency must commit to measuring the results/impact of their event/activity through an online survey. Sutter Health, reserves the right to determine which affiliates sponsor each request. A request may tie two or more affiliates into the sponsorship. All decisions are final. Requesting organizations are not able to go to other contacts within Sutter Health or individual departments seeking support. 9

10 Sponsorship Guidelines (continued) Our sponsorship commitments extend beyond our Community Investment Awards* (grant funding opportunities) and when sponsoring, we look for: - Organizations that engage a broad constituency of support, including but not limited to civic, community and regional leaders, as well as foundations, nonprofits and the business community. - Events that celebrate successful health-related programs with proven impact. - Events that provide visibility to Sutter Health representatives, either with a presence at the event or an opportunity to highlight Sutter s support of the organization. - Events and efforts that foster economic development and vitality, which is imperative to the overall strength and health of our greater community. * If you have received a Community Investment Award (grant), sponsorship funding is on a limited case-by-case basis. 10

11 Frequently Asked Questions (FAQ) What format do I submit my proposal in? Online only via the sponsorship proposal form for your area. Please ensure that you complete the correct form for the county in which you provide services. How do I apply if my organization has multiple events that cover both the Winter and Spring timelines? Please complete one application for all events year around during the first period because organizations will receive no more than one award per year. You must include requested dollar amounts for each event. If my event/organization has multiple sponsorship levels should I include all options in the proposal? Yes, but please be sure to still request a specific dollar amount. How do I report measurable outcomes for my event(s)? After your event has concluded, measurable outcomes are collected via an electronic survey provided during the announcement of your award. Please note that failure to report outcomes may jeopardize future funding opportunities. 11

12 FAQ continued If I received a Community Award Investment (grant), can I apply for sponsorship? Yes, you may, however please note that we do not intend to duplicate investment efforts and all requests from existing grant recipients will be considered on a very limited, case-by-case basis. What is the difference between a Community Award Investment and a sponsorship? Community Investments (grants) are based on our community health needs assessment and focus on programs that increase access for the underinsured and underserved. A sponsorship allows Sutter to collaborate with community based organizations for their events or programs that creates opportunities for Sutter to support the community.
