1 HILTON COUNTY M.D, Contract between Association of Black Cardiologists and Fulton County Requested Action: Approve contract with Association of Black Cardiologists as a collaborative, multi-agency participant of Phase IV of the REACH grant. Discussion: The amount of the contract is $95,700. The contract period expires September 30, The Association of Black Cardiologists will organize, coordinate and initiate churches, barbershops and beauty salons as wellness centers. The Association of Black Cardiologists will assist in the implementation of the community action plan as well as in the planning, development, coordination, implementation and facilitation of community forums to include focus groups, community meetings and briefing sessions. The Association of Black Cardiologists will participate in monitoring and evaluating the success of programmatic activities relating to development of cardiovascular wellness. Association of Black Cardiologists was approved in the grant application during Phase IV of the REACH grant by the Centers for Disease Control and Prevention. This contract is 100% grant funded. No County match is required.
2 Contract between Association of Black Psychologists and Fulton County Requested Action: Approve contract with Association of Black Psychologists as a collaborative, multi-agency participant of Phase IV of the REACH grant. Discussion: The amount of the contract is $89,714. This contract expires September 30, The Association of Black Psychologists will assume the responsibility for organizing, coordinating and implementing the African American Male Empowerment Network. The contractor will also assist in the implementation of the community action plan as well as assisting with planning, developing, coordinating, implementing and facilitating community forums to include focus groups, community meetings and briefing sessions. The contractor will participate in monitoring and evaluating the success of the programmatic activities relating to the African American Male Empowerment Network. The Association of Black Psychologists was approved in the grant application as a contractor of Phase IV of the REACH grant by the Centers for Disease Control. This contract is 100% grant funded. No County match is required.
3 Contract between Divine Universal Sisterhood and Fulton County Requested Action: Request Board of Commissioners to approve contract with Divine Universal Sisterhood as a collaborative, multi-agency participant of Phase IV of the REACH grant. Purpose: To continue to mobilize and organize communities' resources in support of effective and sustainable programs which will eliminate health disparities of racial and ethnic minorities in Fulton Discussion: The amount of the contract is $39,445. This contract expires September 30, Universal Sisterhood will assume responsibility for planning, coordinating and implementing the community nutrition education activities related to the project. Contractor will administer all facets of the community nutrition education component of the community action plan in concert with the technical assistance provided by the Fulton County Department of Health and Wellness Office of Nutrition. Contractor will assist in implementing the community action plan. Contractor will assist in planning, developing, coordinating, implementing and facilitating community forums to include focus groups, community education sessions, food demonstrations, preparation of heart healthy recipes and coordination of annual events centering on healthy aging. Divine Universal Sisterhood was approved in the grant application as the co-investigator for Emory University during Phase IV of the REACH grant by the Centers for Disease Control. This contract is 100% grant funded. No County match is required.
4 Contract between Sisters Action Team and Fulton County Requested Action: Request Board of Commissioners to approve contract with Sisters Action Team as a collaborative, multi-agency participant of Phase IV of the REACH grant. Purpose: To continue to mobilize and organize communities' resources in support of effective and sustainable programs which will eliminate health disparities of racial and ethnic minorities in Fulton Discussion: The amount of the contract is $74,180. This contract expires September 30, Sisters Action Team will be responsible for planning, coordinating and implementing the women's empowerment group meetings and activities related to the project in collaboration with the Association of Black Psychologists. Contractor will assist in planning, developing, coordinating, implementing and facilitating community forums to include focus groups, community meetings and briefing sessions. Contractor will participate in monitoring and evaluating the success of programmatic activities relating to women's empowerment groups in collaboration with Emory University, Rollins School of Public Health. Sisters Action Team was approved in the grant application as a contractor of Phase IV of the REACH grant by the Centers for Disease Control. This contract is 100% grant funded. No County match is required.
5 M.1 Contract between Torrance Stephens, Ph.D. and Fulton County Requested Action: Approve contract with Torrance Stephens, Ph.D. as a collaborative, multiagency participant of Phase IV of the REACH grant. Discussion: The amount of the contract is $70,192. The contract period expires September 30, Dr. Torrance Stephens shall participate in the ongoing implementation meetings for Phase IV of the REACH grant. Dr. Stephens will be a representative to the Key Coalition Board established under the project and he will participate in the deliberations and decision making process of the Board. Dr. Stephens will act as the project's principal evaluator and Dr. Stephens will oversee and direct all aspects of the evaluation activities. Dr. Stephens will assist in any epidemiologic studies conducted for this project and will assist in the statistical analysis and evaluation of research data gathered for this project as well as any outside data being evaluated for this project. Dr. Stephens was approved in the grant application as the co-investigator for Emory University during Phase IV of the REACH grant by the Centers for Disease Control. This contract is 100% grant funded. No County match is required.
