3 HAITI EARTHQUAKE RELIEF DONOR ADVISED FUND (DAF) Summary August 20, 2010 DAF #1 Ecole St. Famille LES CAYES -- Rebuilding Six Classrooms has commenced students to benefit -- Total Cost is $122, The DAF has committed $50,000 District 7020 has committed $20,000 SOS has committed $20,000 Rotary Club of Montego Bay, Jamaica, has committed $10,000 Local Rotarians have committed $2,000 Still needed - $20, La Sainte Famille Elementary School

4 Page 2 DAF #2 Jaipur Limb Project PIGNON -- The Training Center and fitting will be in Pignon -- To provide lower limb prosthetics -- There are about 4,000 in need of prosthetics The cost details are being developed in conjunction with UK Rotarians and the Jaipur Limb Program DAF #4 Clinic for women and children KENSCOFF - Haiti Mountain Center -- Provide primary health care to women and children -- Will serve about 5,000 annually -- Total Cost is $80,000 The DAF has committed to $80,000 in two installments DAF #5 College Catherine Flon CARREFOUR -- Re-building the College -- 5,000 students in Grades from K to Total Cost is $400,000 The DAF has committed to $100,000 Shenandoah University is committed to raising funds There is still funding needed College Catherine Flon, Carrefour, Haiti more photos on the next page

5 Page 3 DAF #6 - L Academie Chretienne macombre/leogane LEOGANE -- Immunizations for 2, Immunizations for Tetanus, Diphtheria, Pertussis and Measles -- Total Cost was $50,585 DAF provided $30,000 The University of Florida provided the balance Project is completed and children immunized DAF #8 - Henche Centre HENCH -- Provide a Medical Vehicle for midwives to get to their patients -- Total cost is $63,000 DAF committed to $10,000 RC of Western Henrico committed to $10,000 RC of BonAir committed $250 RC of Henrico North committed $150 Hermitage Interact committed $850 Bon Secours Health Systems Global Missions will provide the balance

7 Page 5 DAF #13 - Containers from Canada -- Rotarians in Canada have raised significant furniture and supplies for the Schools in Haiti -- The request is to purchase the containers to ship the furniture in -- Total cost is $21,000 The DAF has committed the $21,000 DAF #17 - Ecole Normale des Cayes CAYES -- Rebuilding the school Students who are also used to tutor elementary school students -- Also used for community activities as auditorium, conferences, etc. The school will provide 16 scholarships for two years to current students, as well as six scholarships to new students who were victims of the earthquake in appreciation for Rotary help. -- Total Cost is $120, DAF is committed to $60,000 District 7020 is committed to $20,000 Still need $40, Ecole Normale, Les Cayes, Haiti

8 Page 6 DAF #18 - Microcredit program for Women MIREBALAIS -- This will provide funds to 300 women to start a small business -- Rotary will provide training and oversight with interest of 1.5%. -- Half of the interest will go back into the program to help other women -- The other half will cover operating cost -- Total cost is $52,500 DAF likes the project but seeking further details DAF #20 - Latrines Mirebalais Provide 60 bacteriologic Latrines Total cost is $52,914 DAF likes the project but seeking further details DAF #22 - Latrines CARREFOUR/MON REPOS Provide 10 bacteriologic Latrines to a school Total cost is $23,142 DAF likes the project but seeking further details DAF #24 - Latrines CARREFOUR/MON REPOS Provide 10 bacteriologic Latrines for a tent city Total cost is $23,142 DAF likes the project but seeking further details

9 Page 7 DAF #25 - Work program to clean up the highways CARREFOUR/MON REPOS -- Provide salaries to pay 22 workers in 13 areas of the city to clean up the debris for 13 weeks. -- Total cost is $22,620 DAF has committed $22,620 DAF #30 - Frobel Primary School FROBEL -- Rebuild the school with 350 students -- Fix the structure and the finish the walls -- Total cost is $26, DAF is committed to the $26, in two installments DAF #31 - Savanette Primary School SAVANETTE -- Rebuild the school with 450 students -- This will complete eight classrooms -- Total cost is $33, DAF is committed to $33, DAF #37 - Training for Midwives PIGNON This project is to train potential midwives up to their potential and Will result in a decrease in maternal mortality. Total Cost is $34,540 DAF has committed to the $34,540

11 Page 9 The Good Samaritan School, Ile a Vache, Haiti DAF #41 - Generator for a super kitchen PORT DE PAIX -- The super kitchen is already in place but needs a generator for it to operate -- The kitchen will serve some 8,000 meals per day -- Total cost is $19,300 DAF is committed to $10,500 RC of Del Mar with Outside the Bowl committed to $8,800 RC of Del Mar and Outside the Bowl have already provide the super kitchen Generator for Super Kitchen

12 Page 10 DAF #42 - Super Kitchen Have not chosen location as yet -- They use containers to convert into a super kitchen which can provide food for the community. -- Total cost is $116,500 RC of Del Mar and Outside the Bowl is committed to $66,500 The DAF is still developing the details DAF #45 - Lycee Philippe Guerrier LES CAYES -- Fit out the chemistry lab at the school -- This is a District 7020 project -- Total cost is $26,127 DAF is committed to $3,418 District 7020 is committed to $22,709 DAF #46 - Institut Des hautes Etudes Commerciales et Economiques PORT AU PRINCE -- Repair the computer lab of the University level school of Economy and Finance -- Total cost is $50,000 This project is beyond the focus of the DAF but is a good project. Still needs funding DAF #47 - L ecole du Sacre Coeur PORT AU PRINCE -- Rebuilding the elementary school with 16 classrooms -- Total cost is $464,233 This is beyond the parameters of the DAF but is a good project for a number of clubs and or districts to coordinate efforts to assist. Still need funding
