1 Accreditation Support for Ohio Local Health Districts Request for Training or Technical Assistance - Round 1 The Ohio Department of Health OVERVIEW With funding from the Ohio Department of Higher Education and in coordination with The Center for Public Health Practice in the College of Public Health at The Ohio State University (CPHP), the Ohio Department of Health (ODH) is providing access to training and technical assistance (TA) for local health districts (LHD) in Ohio to support implementation of action steps toward achievement of accreditation through the Public Health Accreditation Board (PHAB). The overall goal is to demonstrate a measurable increase in readiness to apply for accreditation or re-accreditation. Training and TA will be provided by CPHP, or a subcontractor when appropriate, to select LHDs between March 1, 2017 and June 30, It is anticipated that requests will be accepted in a series of five rounds. Approved applicant agencies will not receive direct funding, rather they will receive support at no cost to them. Requests that are collaborative (multiple agencies) are strongly encouraged and will receive preference. ODH will make decisions regarding requests and funding allocations. This document provides an overview of this opportunity for the first round of assistance and includes the following information: Application dates Eligibility requirements Approved training and TA categories Application instructions Selection criteria and awards Contact for questions APPLICATION DATES It is anticipated that requests for assistance will be solicited in five rounds as outlined below. It is expected that there will be two rounds of funding in 2017 and 2018 and one round in Exact dates may change based on evolving needs; the request announcements will be updated accordingly. 2 ELIGIBILITY REQUIREMENTS Eligibility is open to Ohio s LHDs who have completed the LHD Profile and Accreditation Readiness Assessment survey administered by ODH and CPHP in November This opportunity is open to accredited and non-accredited health districts. Eligibility for Rounds 3 through 5 will be tied to participation in the profile and readiness re-assessment planned for early TRAINING AND TECHNICAL ASSISTANCE CATEGORIES Requests may be made for training or technical assistance. Training is defined as learning a new skill in order to perform a task or increase efficiency. Technical Assistance is defined as specialized advice, assistance, or service that is needed to operate more effectively; this may occur in one-on-one or in a group setting. Requests for assistance must fall into one of the three (3) general categories of activities described below. Please note that these categories may be modified in subsequent solicitation rounds to better accommodate changing or evolved needs. This document will be updated as needed. Category 1: Addressing PHAB prerequisite documentation requirements not currently met. The accreditation process is based on demonstrating compliance with eight prerequisites, as well as Standards and Measures spread across 12 Domains, as described in PHAB s Guide to Accreditation (Version 1.5). Applicants to this category may use this award to demonstrate progress towards completing one of five eligible prerequisites: agency strategic plan, workforce development plan, quality improvement plan, performance management system, and/or marketing and branding strategy. Note that community health assessment and community health improvement plans are not eligible for technical assistance, as there is a separate funding opportunity available to local health departments to support these two documentation requirements. Likewise, emergency response plan development is not an eligible focus given other resource availability for this purpose. Agencies are strongly encouraged to submit requests that are collaborative and/or address similar needs within regional/multiple organizations. Currently accredited agencies may apply for this assistance category as related to their reaccreditation needs. Category 2: Supporting implementation of an LHD s work plan toward achieving accreditation. As part of an overall effort to support organizations that are preparing for accreditation, agencies are required to submit a work plan in early 2017, outlining specific action steps toward achieving that status. For LHDs explicitly considering mergers, cross-jurisdictional sharing, or arrangements of other shared services as avenues toward assuring acceptable documentation that supports accreditation and assures service provision, TA is available to support activities that will demonstrate progress toward work plan Round 1 (Mar 1 Aug 31, 2017) 2
3 achievement (ex. feasibility study). Agencies should include other LHDs who may be actively affected by shared arrangements in their requests. Category 3: Completing accreditation readiness activities and strengthening the public health infrastructure. This category allows LHDs to describe a specific accreditation readiness need not captured in either Category 1 or 2 above and request training and/or TA that would support work on identified deliverables that address those needs. Proposals should list the specific Domain, Standard(s) and Measure(s) the LHD is intending to address and how the proposed work ties directly to their anticipated work plan, if appropriate. Agencies are strongly encouraged to submit requests that are collaborative and/or address similar needs within regional/multiple organizations. Currently accredited agencies may apply for this assistance category as related to their reaccreditation needs. APPLICATION INSTRUCTIONS Requests for training or TA are being gathered through an online survey form. The specific fields of the survey appear at the end of this document, and include the following: o Primary Agency Name(s) and Current Accreditation Status o Primary Agency Contact (complete address, phone and ) o Names of Collaborative Agencies, if applicable o Type of Assistance Requested: Training or TA o Training or TA Category (selected from Category list described above) o Topic Area of Requested Training or TA Need o Description of Training or TA Requested o Description of Need/Justification o Possible Challenges and Identified Solutions (as applicable) o Proposed Milestone Activities/Deliverables with Anticipated Timeline for Completion (end by or before August 31, 2017) o Application Certification Please be certain that all information is provided in a clear and concise manner. ODH will review and make all decisions and notifications regarding acceptance of requests.
