1 5.20 Proposed Project Staff Qualifications and Experience: The committee evaluation will include your proposed staff qualifications including, but not limited to: a. Your proposed on-site management/organizational chart and regional and national support. b. Specific qualifications, educational background, and experience of staff proposed to perform and manage the College s project. The committee will assign up to 25 points based on your firm s responses within the following breakdown: (Combined 0 25 points). Up to 5 Points: Sections 5.201, 5.202, 5.203, & (Organizational Chart and Key Personnel Resumes) Up to 10 Points: Section (Nationally Recognized Testing Laboratory (NRTL) Certification) Up to 10 Points: Section (Post Purchase Support Capacity) Proposers shall attach (Labeled as 5.201) an organization chart of proposed staff to work on the College s account. Explain the organization of the Design-Build Team including the firm(s) reporting/contracting relationships as well as individual staff reporting relationships. Indicate the key individuals within each firm (including subcontractors) and include position title, office staff vs. on-site staff, and the reporting relationship of each individual for the project. Include no more than seven (7) key individuals from the office staff of the Design-Build Team and not more than five (5) key individuals from the on-site construction staff of the Design-Build Team. The firm should illustrate the effectiveness of this organizational chart in coordinating the work efforts of each individual and each discipline in order to effectively complete the project. Explain the accountability of each firm and each individual to execute the project successfully. Attached? Experience: types of project; name, dollar value and scope of project; experience with maritime simulated live fire prop system design-build; duties/responsibilities and position title for each project; specific reference to name of projects and position title for those projects listed in Section (Past Related Performance) Designated Subcontractors - The Design-Build Firm shall submit a list of the names, addresses, and minority type/values of the licensed Subcontractors proposing to use on this project. This list shall be submitted with the intent that the Design-Built Firm shall employ these Subcontractors for the trades noted. Should the Owner voice an objection to any of the listed firms due to past poor performances, the Design-Build Firm shall submit for the Owner's approval and selection, the name or names of other Subcontractors which may be employed in lieu thereof. If the Owner's selection of Alternates should affect this list, the Contractor shall be prepared, upon request, to submit the names of Subcontractors which he proposes to use for the given selection of Alternates upon which the Contract may be awarded. Prior to start of construction, the Design-Build firm shall provide verification that all subcontractors are licensed per all applicable federal, state, county, and local law. 3 5.205 Nationally Recognized Testing Laboratory (NTRL) Certification (Up to 10 Points) Submit with your proposal written evidence that the live fire training system equipment offered is currently certified and labeled by a Nationally Recognized Testing Laboratory (NRTL) for compliance with: NFPA 86, the Standard for Ovens and Furnaces UL 508A, the Standard for Industrial Control Equipment The NRTL must meet the requirements of OSHA 29 CFR ( Third party or national testing laboratories not recognized by OSHA will not be considered.
