2 The theme of Relay For Life of Orangeburg and Bamberg for 2017 is Heroes of Hope! Relay will be held on April 28, 2017 at the Orangeburg Fairgrounds from 6:00 PM 12:00 Midnight. 4:00 PM -- Gates will open for general admission (see page 18 for admission guidelines) 6:00 PM -- Opening ceremonies begin 7:00 PM.-- The survivor lap The care giver lap will be immediately following the survivor lap. 9:00 PM -- The gates for admission will close at prior to the luminary service. 10 PM.-- The luminary service In order for our Relay to go smoothly, the following is a few of the guidelines that all teams will follow. Children under the age of 16 must be accompanied by an adult that is 25 years or older. Individuals will show a valid picture ID for admission. Individuals 16 and under will be provided an arm band at the gate for admission, and must be worn at all times during Relay. Any team wishing to have a tent site must have $300 turned in by Bank day, Saturday, April 22, Logistics forms need to be turned in by March 21, High School and College teams must have an advisor attend team captain meetings and present at Relay. Team shirts will be distributed based on raising $100 per team member registered online. New Business sign form must be turned in by March 21, 2017 in order to have adequate time for signs to be made. Pictorials forms and pictures must be turned in by April 18, 2017 in order to have adequate time to be scanned for the Luminary Service. Luminary bags are due by Bank Day, April 22, 2017 Survivor, handicap and team captains parking will be on the Whaley Street entrance. General parking will be at the main entrance on Magnolia Street. Gates will open at 9:00 AM, April 28, 2017 for tent set up. All cars must be off the track and parked in their designated spot by 4:30 PM. No alcohol, tobacco products or pets will be allowed 2

Register online at relayforlife.org/orangeburgsc If you cannot register online, call In order for your team members to receive a team member shirt, they must be registered online by March 22, Team shirts will be distributed based on the team raising $100 per team member registered online. 3

11 AMERICAN CANCER SOCIETY RELAY FOR LIFE LUMINARY / PICTORIAL Luminaries (candles in decorated bags with sand) outline the track at the Relay For Life and will be lit in memory or in honor of loved ones and friends who have had cancer. With your $10 donation per luminary bag, the name of the individual that you have designated will appear on a luminary to be lit on Friday evening. A $25 donation will allow a picture to be displayed during the luminary/pictorial service and a luminary bag. 12 LUMINARY/PICTORIAL FORM 1. NAME: Enter the first and last name of the person for whom the luminary is being donated, in honor or memory of. One name per bag. Please have the first and last name written on the bag also. Even if a nickname (Pop, Mom, Grandpa) is the decorated name. 2. LUMINARY ONLY $10: Check if only a luminary is being purchased (No pictorial) 3. PICTORIAL and LUMIINARY $25: Check if a pictorial and luminary is being purchased. A pictorial is a picture that is submitted and displayed via power point during the luminary service. 4. For pictorial please attach photo that does not have to be returned. This picture needs to be of a quality enabled for scanning. The first and last name of the person in the picture should be written on the back for identification. 5. DONATED BY: Enter the name of the person making the donation. 6. TELEPHONE: Enter the telephone number of the person making the donation. 7. TEAM/MEMBER: Enter the Relay For Life team and member selling the luminary/pictorial. This is very important in case we need to contact for further information. 12

14 Sponsorship Commitment Form This form is completed when a corporation or company agrees to sponsor the Relay For Life of Orangeburg Event. This form should be completed and returned by Mar 7, Business name: Enter the name of the corporation or company who has agreed to become a sponsor. 2-5 Addresss: Enter the address of the corporation / company 6. 16 TEAM ACCOUNTING SHEET The Team Accounting Sheet is used by team captains to complete and submit donations for their team to American Cancer Society Relay For Life of Orangeburg & Bamberg. Please submit to Peggy Fogle our accounting chairperson at team captains meetings. If you have coins, please have them rolled. If possible, please convert your coins to cash. 1. Relay For Life of: Enter Orangeburg & Bamberg. 2. Team Name: Enter your team name. 3. Team Captain: Enter the team captains first and last name. 4. Date: Enter the date you submit this form. 5. Phone: Enter the phone number the team captain Enter the team captain s address 7. Team member name/team fund raising activity: Enter the name of the team member to whom the donation is to be credited 8. Grand Total: Enter the amount of the total donations to whom the donation is to be credited. 9. Grand Totals: Add the Grand total for all participants listed and enter that amount here. 10. Team Captain s Signature: The team captain s signature is placed here. 11. Accounting Chairman s Signature: After verifying the total donations, the accounting chair will sign here. 12. Date: The accounting chairman enters the date the form is submitted here. The accounting chair will return the pink copy of this form to be maintained for team records. 16

18 Orangeburg & Bamberg Relay For Life Rules 1. NO pets, firearms, tobacco products, electronic cigarettes, alcohol, profanity/vulgar language allowed on the premises 2. Appropriate attire is required, this includes proper wear of clothing, i.e. NO sagging pants 3. GATES WILL CLOSE AT 9:00 P.M. WITH NO RE- ENTRY. Chaperones must be 25 years of age or older and stay with their party 7. One Chaperone to every 5 under age attendees 8.
