1 Application Checklist Name of applicant (Typed): Sheldon Graduate Student Department: Hypothetical The entire application must be TYPED and all application materials must be STAPLED in the following order: 1. Application checklist (this page) 2. GPSA Travel Award Application Form (four pages) (starting with the GPSA Travel Grant Summary). Every section on the application must be completely filled out. The form for the current academic year must be used (see dates at the top of the page). 3. Student cover letter (one page maximum). This letter must be signed and must include information about: a) purpose of travel, b) dates of travel, venue/place, c) relevance (benefit) to applicant and university, and d) summary of proposed work. 4. Letter from faculty advisor or committee chair signed on official department letterhead (one page maximum). This letter must be signed and must include information that: a) describes the value of the student travel/participation, b) verifies that there is no other travel money available from the department, individual research grants, or professional association hosting the event or inviting the student s participation, or c) specifies money available for student travel. 5. Evidence of participation from the event sponsors such as: copy of the registration acceptance letter/ confirmation of participation. Applicants name must appear on letter/ confirmation of participation. In field work cases a separate document must be added stating the nature of field work, i.e. where you will be going, what you will be doing, an additional letter from your advisor or person you are meeting with while in the field, and any other supportive evidence.
